Transgender, Mental Health, and Older People: An Appreciative Approach Towards Working Together.

It cannot be assumed by healthcare providers that transgender people routinely receive care and treatment that is of the quality and sensitivity that should be expected. In particular there are concerns from within the transgender community that they experience discrimination and disrespect from both individual practitioners and the healthcare system as a whole. This causes an avoidance of contact that is undesirable for both users and providers of healthcare services. Older transgender people are vulnerable to a range of mental health problems and, like all elderly, increasingly to dementia;

"Living in a Shell of Something I'm Not": Transsexuality, Medical Ethics, and the Judeo-Christian Culture.

A surgeon participating in a 2008 congress on the topic of trans-identified people posed the question of whether doctors would have to defend themselves when "judgment day" comes for having employed surgical means to turn men into women or vice versa? What might be viewed as a certain level of (medical) irrationality surrounding transgender life, coupled with widely documented cases of violence directed at these groups may well partly be attributable to the deeply internalized doctrine of creation in Judeo-Christian culture. Objections, however, to the use of transgender medicine "in the name

Transgender health care in Germany: Participatory approaches and the development of a guideline.

Health care of individuals who experience their gender not, not completely and/or not continuously in line with their sex assigned at birth has been organized for a long time in an area of conflict between the right for self-determination of transgender individuals and the fear of incorrect decisions among mental health professionals. The German Standards for the Treatment and Diagnostic Assessment of Transsexuals, published in 1997, were an attempt to regulate this area of conflicts within the German health system. Meanwhile, in view of the seventh version of the international Standards of Ca

Endocrine treatment of transsexual persons: extensive personal experience.

OBJECTIVE: The Endocrine Society's recently published clinical practice guidelines for the treatment of transsexual persons acknowledged the need for further information on transsexual health. We report here the experience of one provider with the endocrine treatment of transsexual persons over the past 2 decades. METHODS: Data on demographics, clinical response to treatment, and psychosocial status were collected on all transsexual persons receiving cross-sex hormone therapy since 1991 at the endocrinology clinic at Albany Medical Center, a tertiary care referral center serving upstate New Y

"I don't like fitting a box, except the box I am": genderqueer and nonbinary (GQNB) patient strategies for navigating healthcare.

BACKGROUND: While genderqueer and nonbinary (GQNB) people comprise a significant percentage of the transgender community, very little research has explored the unique experiences and needs of GQNB patients. A critical challenge faced by GQNB patients is the degree to which health systems operate inside of a gender binary-as evidenced by the design of health technologies, organizational policies, and culture. Understanding how GQNB patients navigate health systems as they currently exist may give us insight into how to improve the design of those systems. This study aimed to identify GQNB patie

Improving the Gender-Affirmation Process for Transgender and Gender-Nonconforming Individuals: Associations Among Time Since Transition Began, Body Satisfaction, and Sexual Distress.

Sexual distress is associated with a variety of negative outcomes. Unique contributors to sexual distress exist among transgender individuals. The current study examined the impacts of gender-affirming interventions (i.e., hormone therapy [HT], gender-affirmation surgery [GAS]) and body satisfaction on sexual distress among 317 transgender adults recruited nationally to participate in an anonymous online survey. As expected, individuals who had received HT and/or GAS reported better body satisfaction compared to those who wanted these interventions but had not yet received them. Sexual distres

Uterus transplantation in women who are genetically XY.

Uterus transplantation is an emerging technology adding to the arsenal of treatments for infertility; specifically the only available treatment for uterine factor infertility. Ethical investigations concerning risks to uteri donors and transplant recipients have been discussed in the literature. However, missing from the discourse is the potential of uterus transplantation in other groups of genetically XY women who experience uterine factor infertility. There have been philosophical inquiries concerning uterus transplantation in genetically XY women, which includes transgender women and women

Delayed foreign-body reaction and breast deformity after free silicone injections.

Liquid silicone injections for breast augmentation are associated with significant complications. They remain prevalent in transgender and gender-diverse populations facing barriers to regulated surgical care. We present a case of a transgender female with a history of breast silicone injections who developed local complications requiring surgical excision. Clinical breast images and gross pathology are provided. This case highlights characteristic clinical features of silicone-related complications, the role of imaging in diagnosis and the importance of equitable access to safe gender-affirmi

Fertility preservation trends, perspectives, and reflections in gender diverse adults across the life course.

STUDY QUESTION: What is the fertility preservation (FP) utilization rate among transgender and gender diverse (TGD) patients at the Transgender and Intersex Specialty Care Clinic (TISCC) within the Mayo Clinic? SUMMARY ANSWER: The rate of utilization of FP services by TGD patients at Mayo Clinic's TISCC was determined to be 6.62%. WHAT IS KNOWN ALREADY: TGD patients encounter several barriers when pursuing FP, which may contribute to a lower utilization rate of these services. These barriers include financial constraints and lack of insurance coverage, as well as societal and healthcare prov

Prolactinoma course and management in transgender women: case report and systematic review.

OBJECTIVE: Prolactinomas are the most common functioning pituitary neuroendocrine tumors (PitNETs). In transgender women, gender-affirming hormone therapy (GAHT) usually combines high doses of estrogen and anti-androgen therapy, both of which can elevate serum prolactin levels. Whether GAHT influences tumor behavior in patients with pre-existing prolactinomas remains unclear. DESIGN AND METHODS: Case report illustrating the clinical challenges in managing a prolactinoma in the context of GAHT initiation, combined with a systematic review of all published cases and available guidelines of GAHT

Prevalence of gender-affirming hormone therapy in non-binary and genderqueer individuals, a systematic review and meta-analysis.

PURPOSE: Current guidelines for gender-affirming hormone therapy (GAHT) primarily focus on binary transgender (BT) individuals and provide limited recommendations for non-binary and genderqueer (NBGQ) individuals. Understanding hormone use among this heterogenous group will contribute to more personalized counseling and treatment strategies. We performed a systematic review of the scientific literature to assess the prevalence of GAHT in NBGQ individuals and potential clinical context-dependent differences. METHODS: A systematic literature search was performed aimed to assess the prevalence a

Transgender and gender-diverse autistic adolescents are at elevated risk of depression.

Autistic people are more likely to be transgender and gender diverse than the general population. Furthermore, co-occurring trait-level autism and transgender and gender-diverse identity are associated with symptoms of depression and anxiety, and autistic adolescents who identify as transgender and gender diverse have more internalizing behaviors than both non-transgender and gender-diverse autistic adolescents and non-autistic transgender and gender-diverse adolescents. However, no study has yet examined the extent to which transgender and gender-diverse identity predicts specific co-occurrin

Prevalence and potential implications of HPV infection in transgender women with gender reaffirming genital surgery: a systematic literature review and meta-analysis.

BACKGROUND: Human papillomavirus (HPV) is a common infection of the anogenital tract. Although most infections clear, persistent infections with oncogenic types can predispose to cancer. While the natural history of anogenital HPV infection in cisgendered women is relatively well understood, there are significant knowledge gaps regarding HPV prevalence and clinical implications of genital HPV infection in transgender women (TGW) with neovagina(s). METHODS: A systematic review, including meta-analysis, of studies available in the literature with data on HPV prevalence in TGW with neovaginas wa

Social and programmatic vulnerability in the context of transgender people's health: a scoping review of scientific evidence from Brazil.

BACKGROUND: Most transgender people face different conditions of health vulnerability on a daily basis. In the Brazilian context, no research review has been found on such situations in the light of the theoretical conceptualization of multidimensional vulnerability. This research aimed to identify and analyze components of social and/or programmatic vulnerability that interfere with access to health care for trans people in Brazil. METHODS: The scoping review followed the JBI Manual and the PRISMA-ScR list. Brazilian studies published in any language, in the period 2019-April 2023, in the NC

Slipped Capital Femoral Epiphysis in an Adolescent Undergoing Gender-Affirming Therapy with a Gonadal-Releasing Hormone Analog: A Case Report.

CASE: Physeal stability in slipped capital femoral epiphysis (SCFE) depends on integrity of the hypertrophic zone. This in turn is affected by imbalance between circulating growth hormones and gonadal hormones. This case describes the occurrence of SCFE in a Māori (indigenous New Zealander) transgender girl, undergoing gender-affirming therapy with a gonadal-releasing hormone analog (GnRHa). CONCLUSION: Surgeons and clinicians alike should be aware that GnRHa in gender-affirming therapy may confer an additional risk factor, lowering the threshold required for SCFE to occur.
