BACKGROUND: In the 19th century, neurosyphilis was the most frequent cause of dementia in Western Europe. Now dementia caused by syphilis has become rare in Germany. BACKGROUND: β-Lactamases are the main cause of bacterial resistance to penicillin, cephalosporins, and related β-lactam compounds. The presence of the novel penicillin-binding protein (pbp) Tp47 in Treponema pallidum has been reported to be a well-known mechanism for turnover of b-lactam antibiotics. Although, T. pallidum remains sensitive to penicillin, clinically significant resistance to macrolides has emerged in many developing countries. Among patients with primary syphilis, sensitivity of the toluidine red unheated serum test was 55% (6/11) compared with darkfield microscopy and 60% (9/15) compared with Treponema pallidum polymerase chain reaction. Specific histological alterations have been observed in 23 of the 25 patients with early syphilis with laboratory findings suggesting liver damage. Treponema has been detected in the areas of liver lesion in 70 per cent of the patients. Control examination carried out after penicillin therapy showed regression of the alterations. On the basis of the findings authors assert that treponema might induce hepatitis in early syphilis differing from liver damage of other origin.
