Appropriate Selection or Poor Triaging: Assessment of Patient Profiles Presenting to a Neuromodulation for Pain Program at a Tertiary Academic Center.

Spinal cord stimulation (SCS) is effective for some pain syndromes, but inappropriate referrals can frustrate patients and misuse resources. This retrospective study reviewed 370 patients referred to a Neuromodulation for Pain Program (NPP) from July 2022 to June 2024. Of these, 241 were deemed appropriate for SCS. The SCS-appropriate group had higher pain intensity, more neuropathic pain, and lower opioid use. Inappropriate referrals were often from family physicians. Key factors associated with inappropriateness included low pain intensity, lack of neuropathic features, and high opioid use.

A mixed methods study protocol evaluating early screening, triaging, risk assessment and health optimisation in perioperative pathways.

BACKGROUND: Over 10 million operations are carried out every year in the UK, improving millions of lives. While most operations are low-risk, some result in patients having poor outcomes. Preoperative assessment evaluates a patient's health prior to surgery to identify risks and where possible minimise them through optimisation. However, this preoperative assessment often takes place close to the planned date of surgery, meaning there is little time to optimise patients' health. Early preoperative screening is the process of evaluating a patient's health earlier in the surgical pathway. In 202

Triage and care for women with symptoms or diagnosis of pregnancy loss between 14 + 0 and 21 + 6 weeks' gestation.

Mid-trimester pregnancy loss (MTL), defined as a pregnancy loss occurring between 14 + 0 and 21 + 6 weeks of gestation, causes significant physical and emotional distress to women and presents clinical challenges to healthcare professionals. It is acknowledged that in low-resource settings, this guideline might be applicable to births up to 28 weeks or babies weighing less than 1 kg. Risk factors for MTL include advanced maternal age, previous history of MTL, women of Black ethnicity, smoking, excessive alcohol consumption, obesity, and anatomical factors such as a short cervix, congenital ute

Evaluating the Accuracy of the SIL Score for Predicting the Sepsis Mortality in Emergency Department Triages: A Comparative Analysis with NEWS and SOFA.

Background/Objective: Sepsis is a disease with a high mortality rate, which emphasizes the importance of developing tools for the early identification of high-risk patients and to initiate timely treatments to reduce mortality. The SIL score is a scale that uses the shock index and arterial lactate level to identify early on the patients that are at a high risk of in-hospital mortality due to sepsis. The purpose of this study was to validate the SIL score as a tool for estimating the probability of sepsis in-hospital mortality from the triage room in emergency departments. Additionally, the ad…

Benefits and challenges of in vivo EPR nail biodosimetry in a second tier of medical triage in response to a large radiation event.

Following large-scale radiation events, an overwhelming number of people will potentially need mitigators or treatment for radiation-induced injuries. This necessitates having methods to triage people based on their dose and its likely distribution, so life-saving treatment is directed only to people who can benefit from such care. Using estimates of victims following an improvised nuclear device striking a major city, we illustrate a two-tier approach to triage. At the second tier, after first removing most who would not benefit from care, biodosimetry should provide accurate dose estimates a…

Risk-stratification of febrile African children at risk of sepsis using sTREM-1 as basis for a rapid triage test.

Identifying febrile children at risk of sepsis in low-resource settings can improve survival, but recognition triage tools are lacking. Here we test the hypothesis that measuring circulating markers of immune and endothelial activation may identify children with sepsis at risk of all-cause mortality. In a prospective cohort study of 2,502 children in Uganda, we show that Soluble Triggering Receptor Expressed on Myeloid cells-1 (sTREM-1) measured at first clinical presentation, had high predictive accuracy for subsequent in-hospital mortality. sTREM-1 had the best performance, versus 10 other m…

Towards enhanced telephone triage for chest pain: a Delphi study to define life-threatening conditions that must be identified.

BACKGROUND: Improving telephone triage for patients with chest pain has been identified as a national research priority. However, there is a lack of strong evidence to define the life-threatening conditions (LTCs) that telephone triage ought to identify. Therefore, we aimed to build consensus for the LTCs associated with chest pain that ought to be identified during telephone triage for emergency calls. METHODS: We conducted a Delphi study in three rounds. Twenty experts in pre-hospital care and emergency medicine experience from the UK were invited to participate. In round I, experts were as…

DNA methylation marker for the triage of hrHPV positive women in cervical cancer screening: Real-world evidence in Taiwan.

OBJECTIVE: Human papillomavirus (HPV) testing as the primary cervical cancer screening followed by reflex cytology if high-risk HPV is present (hrHPV+) is recently adopted in some countries. However, reflex cytology's sensitivity is variable, and a suitable triage approach for hrHPV+ remains controversial. Here, we compared the performance of three triage tools in hrHPV+ women. METHODS: Three triage tools-cytology, HPV16/18 genotyping, and DNA methylation biomarker PAX1m-were analyzed for their clinical performance in hrHPV+ women. In addition, women without cervical cancer at enrollment were…

Deep-learning algorithms for the interpretation of chest radiographs to aid in the triage of COVID-19 patients: A multicenter retrospective study.

The recent medical applications of deep-learning (DL) algorithms have demonstrated their clinical efficacy in improving speed and accuracy of image interpretation. If the DL algorithm achieves a performance equivalent to that achieved by physicians in chest radiography (CR) diagnoses with Coronavirus disease 2019 (COVID-19) pneumonia, the automatic interpretation of the CR with DL algorithms can significantly reduce the burden on clinicians and radiologists in sudden surges of suspected COVID-19 patients. The aim of this study was to evaluate the efficacy of the DL algorithm for detecting COVI
