Sacroiliac joint tuberculosis revisited - A clinico-radiological review.

Osteoarticular Tuberculosis (TB) of the Sacroiliac (SI) joint is an uncommon site affected by Mycobacterium Tuberculosis infection. The SI joint is involved in approximately 5-10% of all cases of TB. Diagnosis of SI joint TB can be delayed in early stages due to its varied and hidden presentation and probability of being confused with other spinal diseases. Delay in diagnosis can lead to chronic pain, joint destruction, and a natural progression to symptomatic bony ankylosis. A focused clinical examination, complementary imaging, microbiological and histopathological confirmation of Mycobacter

Intramedullary tuberculoma of the Spinal Cord, Clinical features & imaging: Possibility of early diagnosis with imaging?

Involvement of vertebral column is common in tuberculosis(TB) but intramedullary tuberculomas are rare. Spinal intramedullary tuberculoma are extremely rare, seen in only 2 of 100,000 cases of TB and 2 of 1000 cases of CNS TB. Intramedullary tuberculomas normally respond well to conventional antituberculous medications, requiring surgery only occasionally. MRI is optimal diagnostic modality in intramedullary tuberculoma as it can show specific findings. This report describes a case of Intramedullary Tuberculoma of the Spinal Cord with clinical features and specific MRI findings with review of

Missing red flags in back pain--tuberculosis of the spine: an important differential for back pain in non-endemic countries.

Chronic back pain is an extremely common complaint. All primary care physicians will be on the lookout for the 'red flags' that suggest serious pathology. The diagnosis of spinal infection with tuberculosis (TB) is uncommon and often not considered, especially in areas where the rate is very low such as the south west of England. We describe a patient presenting to the emergency department with severe pain, immobility and with a sensory deficit level. Unfortunately, given the favourable results for early medical treatment for spinal TB, this patient presented late and had a very poor outcome.

[Medical treatment of osteoarticular tuberculosis].

The authors report 45 personal cases observed over the last ten years, from a series of 520 cases collected during a multi-centre enquiry of the French Society of Rheumatology and a review of the literature; they discuss the present tendencies in the treatment of bone and joint tuberculosis. Antibiotic and chemotherapy uses drugs which are more and more effective, easier and easier to manipulate, ensuring in almost all cases a lasting cure with frequent conservation of joint function, shortening the natural course of the disease, and preventing or curing its complications. The role of surgery

Intradural extramedullary tuberculoma of the thoracic spine - A rare case report of paraparesis.

BACKGROUND: Intradural extramedullary tuberculoma of the spinal cord (IETSC) is an extremely rare form of spinal tuberculosis (TB) that is believed to be due to a host's immune reaction against the Mycobacterium protein derivatives. CASE DESCRIPTION: A 25-year-old male with human immunodeficiency virus, hepatitis C virus, and disseminated TB on antitubercular therapy for the past 8 months, presented with paraplegia of 2 months duration. When the MRI spine revealed multiple peripheral rim enhancing intradural extramedullary lesions from T6 to T8 and dorsally from T10 to T11, the patient was di

[Neurosurgical management of paraplegia complicating Pott's disease: a six-case study].

Tuberculosis is a rare disease in economically developed countries. Paraplegia is one of its severe complications, occurring in cases of spondylodiscitis with compressive epiduritis and/or pathological fracture of vertebral bodies with subacute kyphosis. Six patients aged 15-75 years were treated in our department from May 2005 to April 2006 by the same operator (L.N.) for paraplegia complicating Pott's disease. Patients' neurologic function at admission and their outcomes three and 12 months after surgery were graded using the Frankel classification. MRI showed a single lesion in five cases a

Multiple sclerosis showing elevation of adenosine deaminase levels in the cerebrospinal fluid.

An 80-year-old man developed dysarthria, quadriplegia, sensory disturbance and ataxia in all limbs. Brain and spinal magnetic resonance imaging (MRI) revealed multiple enhanced lesions. Cerebrospinal fluid (CSF) levels of adenosine deaminase (ADA) remarkably elevated. Tuberculosis DNA was not detected, and tuberculosis was not cultured either in the CSF. Brain biopsy revealed the inflammatory demyelinating lesions. With the diagnosis of multiple sclerosis, corticosteroid therapy resulted in rapid improvement of his symptoms and MRI abnormalities. CSF levels of ADA also decreased. Multiple scle

Retropharyngeal abscess coinfected with Staphylococcus aureus and Mycobacterium tuberculosis after rhinoviral infection in a 1-month-old infant.

A retropharyngeal abscess is a rare disease entity in young infants but can develop after nasopharyngeal viral infection. Group B Streptococcus and Staphylococcus aureus are the most common pathogens in young infants, however, Mycobacterium tuberculosis is very rare. We report the case of retropharyngeal abscess and coinfection with S. aureus and M. tuberculosis in a very young infant presenting with respiratory symptoms due to upper airway obstruction. Usually tuberculous retropharyngeal abscesses are caused by the direct invasion of the bacteria from the spine via anterior longitudinal ligam

Possible correlation between Mycobacterium abscessus subsp. massiliense spondylodiscitis and spinal anesthesia: Diagnostic and therapeutic challenges.

Spinal infections caused by Mycobacterium abscessus are rare and difficult to treat. We report the first case of M. abscessus subsp. massiliense spondylodiscitis temporally associated with spinal anesthesia. A 57-year-old man developed back pain after undergoing spinal anesthesia for prostate surgery. Six months later, decompressive laminectomy was performed; results for bacterial and fungal culture and molecular testing for Mycobacterium tuberculosis were negative. Two years later, a mycobacterial culture of material from a paravertebral abscess revealed the presence of M. abscessus, later id

Extradural Dumbbell-Shaped Tuberculoma Masquerading as Nerve Sheath Tumor: Case Report and Review of Literature.

BACKGROUND: Spinal tuberculosis has been listed as a rare cause of neuroforaminal widening with only 2 previous reports in the literature. Here, we report the third case of an extradural tuberculoma extending through and expanding the neural foramen closely masquerading as a nerve sheath tumor including, to the best of our knowledge, the first description of magnetic resonance imaging, operative, and histopathology findings. CASE DESCRIPTION: A 65-year-old Nigerian man presented with signs and symptoms of worsening thoracic myeloradiculopathy for the past month. Imaging found an extradural du

[Comparative evaluation of the immunoenzyme system used in the serodiagnosis of tuberculosis and non-tuberculous diseases].

A method for detection of antituberculous antibodies by enzyme immunoassay system developed by the authors, was tested on 317 blood serum samples taken from patients with tuberculosis and non-tuberculous diseases of the lungs, vertebral column, kidneys and genitalia. Depending on the process site, the detection rate of specific antibodies and their specificity made up 57-77% and 85-95%, respectively, which practically agrees with the data produced as a result of a simultaneously conduction of three routine serologic reactions.
