PU is a disease having many etiologies. The psychological constitutional type, low psychological adaptability and vegetative imbalance are the main causes in the pathogenesis of the disease. Chronic and acute stresses lead to vegetative tone disturbance, immunological imbalance, reduced resistance to HP, hypercoagulation and local blood flow impairment, causing hemorrhage and necrosis in the mucous coat of the gastroduodenal area.

Multifactor analysis of 818 cases of acute gastroduodenal ulcer bleedings (clinical and laboratory examination, gastroscopy, morphological studies) was carried out, 97 of these patients had recurrence of bleeding. This is a retrospective study of corneal ulcers caused by Pythium insidiosum in Ramathibodi Hospital from 1988 to 1998. The clinical data was from the medical records of 10 patients, of which 8 had complete information. Mean age of the patients was 49.8 years old and seven were farmers. OBJECTIVE: To determine the pattern of surgically managed peptic ulcer disease.
DESIGN: A retrospective study.

The case of a 16-year-old woman with typical clinical features of acute vulval ulcer, Lipschutz's disease, is reported. The main differential diagnoses are discussed. Isolated vulval ulcer can reveal a recurrent aphthous ulcer, Behçet's disease or Crohn's disease. In contrast, acute vulval ulcer spontaneously heals with no recurrence, that must be taken into account for diagnosis.

Experiments in 5 pigs and 18 mongrel dogs have shown the possibility to close perforating ulcers with plates of biopolymer "Tachocomb". The method is proposed as an alternative to suturing perforations in the zone of pylorus in order to prevent stenosis of the pyloric part of the stomach. The experimental data obtained have confirmed good adhesive and stimulating properties of the fibrin-collagen substance. Successful results of using the method were demonstrated in 2 patients with perforating ulcers.

Of 1441 patients operated on, the giant gastric ulcer was revealed in 16.6%. It is characterized by polymorphism: several ulcers were revealed in 7.6%, coexistent gastric and duodenal ulcer--in 19.8% and, in contradistinction to genuine gastric ulcer (I type--according to Johnson), by high frequency of such complications as hemorrhage (in 44.1% of observations), penetration (in 74.1%), malignancy (in 14.3%).

BACKGROUND: Medical treatment of peptic ulcer is highly successful, and the eradication of Helicobacter pylori (H. pylori) reduces ulcer recurrence. However, the incidence of perforated duodenal ulcer and its associated mortality have not been reduced by modern methods of therapy.
