Polyorchidism is a rare congenital anomaly. A review of literature has yielded about 100 cases. We describe a rarer case of polyorchidism consisting of 3 testes on left hemiscrotum and one normal testis in right hemiscrotum. Diagnosis and management are discussed.

PURPOSE: While vasectomy reversal is a highly successful procedure 10% to 30% of reversals may fail. Despite the general consensus that an epididymal obstruction may occur following a vasectomy and that some men should undergo vasoepididymostomy (VE) rather than vasovasostomy (VV), the practice of many urologists in our region has been to offer only VV for vasectomy reversal. We present one of the very few deliveries occurring following intracytoplasmic sperm injection of thawed immotile testicular spermatozoa from a testicular biopsy of a man with bilateral congenital absence of the vas deferens. A first attempt in the 33 year old woman with fresh testicular biopsy extracted spermatozoa was unsuccessful. The supernumerary spermatozoa were cryopreserved for later use. A case of bilateral nonpalpable testes is described. Ultrasonography showed no testis. Laparoscopy revealed no intraabdominal testis, vas or vessels. Human chorionic gonadotrophine test suggested the presence of testicular tissue. Computed tomography showed a testis present in each inguinal canal. Inguinal exploration confirmed the finding. This case stresses the importance of careful interpretation of the laparoscopic findings together with other diagnostic methods in localizing testicular tissue in patients with nonpalpable testes.

Injuries to the spermatic cords, sustained during childhood inguinal hernioplasty, caused unilateral testicular atrophy, multiple bilateral excurrent duct obstruction and aspermia. Simultaneous unilateral vasal reconstruction and vaso-epididymostomy resulted in satisfactory semen parameters. It is expected that vasal integrity will be maintained after multiple reconstructive procedures at different locations if further injury to the vascular supply of the excurrent duct is avoided.

Melatonin (aMT) implantation during testicular inactive phase (January-March) brought an early initiation of testicular development in Perdicula asiatica. Patients with retained mullerian ducts who have a predominantly male phenotype must undergo removal of the mullerian structure in addition to preservation of male structures. Often the vas conjoins with the vaginal wall. Whereas formerly we divided the vas from the mullerian structures, we now attempt to preserve the vas to maintain as much testicular function as possible by creation of a neoseminal vesicle. This procedure can be done separately or can accompany hypospadias reconstruction.

After vasectomy reversal by vasovasostomy or vasoepididymostomy motile sperm appear commonly in the semen even when only nonmotile sperm are present in the intraoperative vasal or epididymal fluid. We studied patients with bilateral congenitally absent vasa deferentia to see if relief of obstruction by implantation of an alloplastic spermatocele also benefits sperm motility in such patients. Artificial spermatoceles were implanted into three patients with congenital absence of the vas. In each case, the testicular biopsy demonstrated normal spermatogenesis, and the dilated epididymal tubule was packed with spermatozoa. The ciliated epididymal mucosa appeared normal despite the tubular dilatation. Our experiences with 343 vaso-epididymal anastomosis operations carried out for the relief of obstructive azoospermia in the past 20 years were reported. The etiologic factors in obstructive azoospermia were discussed. Smallpox was the single most common factor responsible for the obstructive lesion in our series.
