Evidence of Peripheral Vestibular Impairment Among Adults With Chronic Moderate-Severe Traumatic Brain Injury.

PURPOSE: Traumatic brain injury (TBI) is a leading cause of death and disability among adults in the United States. There is evidence to suggest the peripheral vestibular system is vulnerable to damage in individuals with TBI. However, there are limited prospective studies that describe the type and frequency of vestibular impairment in individuals with chronic moderate-severe TBI (> 6 months postinjury). METHOD: Cervical and ocular vestibular evoked myogenic potentials (VEMPs) and video head impulse test (vHIT) were used to assess the function of otolith organ and horizontal semicircular can

Association of air-conducted sound oVEMP findings with cVEMP and caloric test findings in patients with unilateral peripheral vestibular disorders.

CONCLUSION: This study showed that the ocular vestibular evoked myogenic potential (oVEMP) in response to air-conducted sound (ACS) reflects functions of different parts of the vestibular labyrinth from cervical VEMP (cVEMP). OBJECTIVE: To determine whether the origin of the vestibular end organs of the oVEMP in response to ACS (500 Hz tone bursts) is the same as that of cVEMP. METHODS: Twenty patients definitively diagnosed with unilateral Meniere's disease (MD), 6 patients with unilateral vestibular neuritis (VN), and 7 healthy subjects were enrolled. In these subjects, the oVEMP and cVEMP

Novel heterozygous mutations in the otogelin-like (OTOGL) gene in a child with bilateral mild nonsyndromic sensorineural hearing loss.

Hearing loss is a common disease, of which genetic factors are the main cause. The incidence of mild or moderate postlingual deafness in children is not high, and the impact on life and learning is not as severe as that of prelingual deafness. This leads to insufficient attention to the disorder in the clinic. To date, only a few disease-causing genes have been reported. This report describe a case of novel heterozygous mutations in OTOGL that causes nonsyndromic mild sensorineural hearing loss. Basic information, imaging examinations, audiological examination, and vestibular function tests of…

High-fat diet attenuates vestibular and auditory dysfunction in aged mice via macrophage-mediated anti-inflammatory mechanisms.

OBJECTIVE: The purpose of this research was to explore how a high-fat diet (HFD) influences vestibular impairment and hearing deterioration associated with aging, to lay the groundwork for comprehending their underlying physiological processes, and to create innovative treatment strategies. METHODS: Clinical data analysis revealed correlations between lipid levels and age-related decline of auditory and vestibular functions. Older C57BL/6 J mice were divided into two groups: one receiving a HFD (60 % fat) and a control group, for a duration of three months. The auditory brainstem response (AB

Quantitative video-oculography to help diagnose stroke in acute vertigo and dizziness: toward an ECG for the eyes.

BACKGROUND AND PURPOSE: Strokes can be distinguished from benign peripheral causes of acute vestibular syndrome using bedside oculomotor tests (head impulse test, nystagmus, test-of-skew). Using head impulse test, nystagmus, test-of-skew is more sensitive and less costly than early magnetic resonance imaging for stroke diagnosis in acute vestibular syndrome but requires expertise not routinely available in emergency departments. We sought to begin standardizing the head impulse test, nystagmus, test-of-skew diagnostic approach for eventual emergency department use through the novel application

Translabyrinthine vestibular neurectomy and simultaneous cochlear implant for Ménière's disease.

BACKGROUND: Surgical management of Ménière's disease (MD) is recommended in case of medical and intratympanic treatment failures. Translabyrinthine vestibular nerve section has been considered the gold standard for denervation procedures in order to control vertigo attacks, although at the cost of sacrificing residual hearing. To the best of our knowledge, no work has been published with regard to a group of patients submitted to translabyrinthine vestibular neurectomy and simultaneous cochlear implant for MD. The aim of the present study was to assess the effectiveness of translabyrinthine ve

Evaluating Postural Control Functions With a Focus on Vestibular Sensation in Thalamic Astasia: A Case Report.

Thalamic astasia (TA) is a condition characterized by difficulty maintaining an upright posture due to a unilateral lesion in the thalamus. Motor paralysis, sensory disturbances, and ataxia are typically absent or mild. While vestibular dysfunction has been implicated in the onset of TA, a comprehensive evaluation strategy that considers vestibular function has yet to be established. In this report, we present a case of TA following a left thalamic infarction where the patient exhibited no abnormalities in their subjective vertical visual (SVV) function, which is often used as an indicator of

The dissociation between pathological caloric testing and a normal video head impulse test helps differentiate between Menière's disease, vestibular migraine, and other vestibular disorders: a confirmatory study in a large cohort of 2,101 patients.

Vestibular migraine (VM) and Menière's disease (MD) are characterized by episodes of vertigo of similar duration. It is well known that differentiation between both diseases is not always possible based only on the patient history, physical examination, and audiological testing. In addition, the quantification of the vestibular function can also be helpful since, among patients with MD, there is often a dissociation between a normal/pseudo-normal video head impulse test (vHIT) and reduced caloric testing. The goal of this confirmatory study was to determine the sensitivity, specificity, and po

Is galvanic VEMP a prediction of the nerve origin and damage in patients of vestibular schwannoma.

BACKGROUND: Recent studies proved that certain proportions of vestibular schwannoma (VS) originated other than vestibular nerve of the eighth cranial nerve. AIMS/OBJECTIVES: Unlike air-conducted sounds (ACS) and bone-conducted vibration (BCV), galvanic vestibular stimulation (GVS) evokes vestibular evoked myogenic potentials (VEMPs) from the vestibular nerve. MATERIALS AND METHODS: Case-control study was conducted in unilateral VS patients pre-operatively. Healthy ears were controls. Patients examined ACS, BCV and GVS ocular VEMP (oVEMP) and cervical VEMP (cVEMP), caloric test, video head im

Calcitonin gene-related peptide in the audiovestibular system.

OBJECTIVE: This study aimed to summarize and analyze the current literature related to calcitonin gene-related peptide (CGRP), the vestibular system, and vestibular migraine. BACKGROUND: CGRP is a neuropeptide that has been implicated in the pathophysiology of migraine. Vestibular migraine (VM) is a subtype of migraine that causes recurring episodes of vestibular symptoms like vertigo and is often associated with migrainous symptoms. Although the pathophysiology of VM is not completely understood, CGRP expression in the central and peripheral vestibular systems has suggested that it may also

Dizziness in Parkinson's disease patients is associated with vestibular function.

Dizziness is common in Parkinson's disease (PD) patients. It is known that orthostatic hypotension (OH) is the main cause of such dizziness, but even without OH, quite a few PD patients complain of dizziness in the clinic. It can be regarded as non-specific because most of these patients have no neurological abnormalities. We hypothesized that this type of dizziness would be associated with vestibular function, although included patients did not have clinically confirmed vestibulopathy. We studied 84 patients without OH among 121 PD patients. Their clinical features and function were compared

CANVAS an update: clinical presentation, investigation and management.

BACKGROUND: Cerebellar Ataxia with Neuropathy and bilateral Vestibular Areflexia Syndrome (CANVAS) is a multi-system ataxia which results in cerebellar ataxia, a bilateral vestibulopathy and a somatosensory deficit. This sensory deficit has recently been shown to be a neuronopathy, with marked dorsal root ganglia neuronal loss. The characteristic oculomotor clinical sign is an abnormal visually enhanced vestibulo-ocular reflex. OBJECTIVE: To outline the expanding understanding of the pathology in this condition, as well as diagnostic and management issues encountered in clinical practice. ME

Idiopathic latent vestibulopathy: a clinical entity as a cause of chronic postural instability.

The objective of this study is to describe a new clinical entity of idiopathic latent vestibulopathy (ILV), in which patients have unilateral or bilateral vestibulopathy combined with unsteadiness but without episodic vertigo, auditory disturbance, or a medical history suggesting the presence of vestibulopathy. A retrospective study of 1,233 consecutive new outpatients was conducted. Two-legged stance tasks were performed by 11 patients identified as having ILV in four conditions: eyes open with and without foam rubber, and eyes closed with and without foam rubber. We examined six parameters:

Relative diagnostic value of ocular vestibular evoked potentials and the subjective visual vertical during tilt and eccentric rotation.

OBJECTIVE: We compared vibration-induced ocular vestibular evoked myogenic potentials (OVEMPs) with the visual vertical during whole-body roll tilt and eccentric rotation in healthy subjects and patients with unilateral vestibular loss, to determine which test was most sensitive in discriminating impaired utricle function. METHODS: OVEMPs and the visual vertical were measured in 11 patients and 11 healthy subjects. Visual vertical was measured during roll tilts between -9.6° and 9.6°, and during rotation at 400°/s with the head upright and the vertical rotation axis located between ±3.5 cm fr

Gaze stabilization and gait performance in vestibular dysfunction.

BACKGROUND: The gaze stability test (GST) quantifies the ability of a person to recognize a target projected on a personal computer monitor during active head movement. PURPOSE: The purpose of this study was to determine if there was a relationship between clinical measures of walking performance and the GST in patients with vestibular disorders and in healthy subjects. We hypothesized that impairment of the ability to keep objects in focus during active head movement would be correlated with walking performance. SUBJECTS: Twenty older asymptomatic adults acted as controls and 12 patients wi

Benign recurrent vertigo--true or artificial diagnosis?

The etiology of many diseases involving vertigo is still unknown although the same etiologic factors have been suggested for several diseases. In most cases the diseases are diagnosed on the basis of exclusion-other possible causes being ruled out before the diagnosis can be confirmed. Benign recurrent vertigo (BRV) has been defined as spells of vertigo characteristic of Menieres disease without auditory or clinical neurologic symptoms and signs. The etiology of this condition is also unknown. BRV has been linked to migraine and viral diseases. In a prospective study we collected the clinical
