INTRODUCTION: Non-Steroidal AntiInflammatory Drugs (NSAIDs) are widely used drugs for the management of pain. Oral administration of NSAIDs has various adverse events like gastrointestinal alterations - nausea, dyspepsia and gastrointestinal bleeding, hepatotoxicity and blood dyscrasias. As orally given NSAIDs are associated with systemic side effects, it is preferred to alleviate pain and inflammation using topical medication. This study described a rare case of an 83-year-old man diagnosed with drug-induced skin lesions of bilateral lower extremities after continuous self-administration with clobetasol propionate ointment for eczema. On admission, his skin was paper-thin, peeling, dry, bruising, and swollen on both lower extremities. Topical clobetasol propionate was immediately discontinued. During a 2-week follow-up period, his symptoms had resolved significantly.

Apraclonidine eye drop is an alpha adrenergic agonist derived topical clonidine, used for the treatment of intraocular pressure elevation. We report what is to our knowledge the first case of systemic toxicity of apraclonidine resulted from repeated local administration. Clinical manifestation of toxicity was similar to oral clonidine overdose. Toxicities of ocular drugs should always be considered when a patient presents with new systemic problems.

The tapering or termination of prolonged strong topical and/or systemic corticosteroid application for extensive generalized eczema has adverse effects on the body and thus presents a very difficult situation. The present case is that of a 68-year-old man with erythroderma following eczema, whose steroid withdrawal was successfully treated with intermittent ACTH and low dose systemic cyclosporin administration over a period of one year.

In a study of 52 consecutive out-patients the effects of diazepam and dixyrazine were compared with each other and with placebo as premedication for elective gastroscopy. All patients also received topical lidocaine and intravenous administration of propantheline bromide immediately before the examination. No difference was found between diazepam and dixyrazine, but they were both significantly superior to placebo in relieving anxiety, retching, and eructations.
