AIM: To examine how task source and disease severity influence cognitive workload and decision-making performance among intensive care unit nurses during concurrent multitasking. Understanding how different clinical information sources influence nurses' decision-making may provide insights for supporting decision-making in complex critical care environments.
DESIGN: A laboratory-based 2 × 2 mixed-design experimental study.
METHODS: Thirty ICU nurses participated in a concurrent task prioritization task in which task source (machine-induced vs. human-induced) and disease severity (high vs. lo

More information is needed on how to recruit and retain rural health professionals, especially during times of crisis. In this article, we explore the experiences of rural health professionals during the COVID-19 pandemic. We conducted in-depth, semi-structured interviews with physicians, advanced practice providers, nurses, and allied health professionals in the state of Minnesota from 2023 to 2024 (n = 17) and used iterative thematic analysis to identify key themes. Two key themes emerged from our analysis as challenges during the COVID-19 pandemic: staffing shortages and the politicization

Abdominal pain is a common chief complaint in Emergency departments (EDs) nationwide which presents a wide array of potential etiologies including acute coronary syndrome ( Daniels et al., 2020 ). High-sensitivity troponin I (hs-TnI), a biomarker traditionally linked to cardiac injury, can also be elevated in noncardiac conditions, such as acute cholecystitis. This case study underscores the advantages of hs-TnI over conventional troponin tests, specifically among patients with abnormal ECGs. This case also highlights the hs-TnI superior sensitivity in detecting or excluding subtle myocardial

Nephrology nurse practitioners (NNPs) play a critical role in providing kidney supportive care for patients with chronic kidney disease (CKD) and kidney failure. Their responsibilities focus on enhancing patient outcomes and improving quality of life. NNPs are instrumental in conducting comprehensive assessments to manage the complex medical, psychological, and social aspects of kidney disease. NNPs coordinate multidisciplinary care teams to ensure holistic treatment plans that address both symptoms and psychosocial support. This includes managing pain, anxiety, depression, and other distressi

Evaluation and management of older adults within emergency care settings is often complex and has the potential for avoidable complications. In an effort to prevent harm to the patient, treatment strategies need to be multifactorial. For geriatric patients presenting with rectal prolapse, unique management strategies, including the use of granulated sugar and gentle pressure, may assist in the reduction of the prolapsed tissue. Additional interventions that are important to incorporate into the older adult's plan of care include gentle approaches to positioning, involvement of family/caregiver

More than 5 million central lines are placed in the United States each year. Advanced practice providers place central lines and must understand the importance of ultrasound guidance technology. The use of anatomic landmarks to place central lines has been employed in the past and in some instances is still used. This method may make accessing the target vessel difficult in the patient with anomalous anatomy or in the obese patient. These characteristics decrease successful placement and increase complications. Different organizations have agreed that the use of ultrasound during central venou

Patients with severe traumatic brain injury or large intracranial space-occupying lesions (spontaneous cerebral hemorrhage, infarction, or tumor) commonly present to the neurocritical care unit with an altered mental status. Many experience progressive stupor and coma from mass effects and transtentorial brain herniation compromising the ascending arousal (reticular activating) system. Yet, little progress has been made in the practicality of bedside, noninvasive, real-time, automated, neurophysiological brainstem, or cerebral hemispheric monitoring. In this critical review, we discuss the asc

PURPOSE: To evaluate the role of counseling for patients undergoing screening for type 2 herpes simplex virus (HSV-2). Using pre- and post-screening questionnaires, we evaluated correlations between demographic factors, measures of self-efficacy and follow-through with the screening test, and willingness to take antiviral medication and share results with sexual partners.
DATA SOURCES: Subjects (N= 115) were recruited as part of usual care at a private, urban family practice. A total of 111 subjects completed pre-screening questionnaires, and 38 completed post-screening questionnaires.

BACKGROUND: Carotid ultrasound is traditionally confined to specialist settings because examination quality and diagnostic reliability are highly dependent on the operator's technical skill and experience. Recent advances in AI-assisted ultrasound may enable task shifting to primary care staff and support the integration of personalized visual risk communication into routine preventive care. Visualization of subclinical atherosclerosis has been shown to strengthen cardiovascular risk communication and support preventive engagement. However, limited evidence exists on how such technology can be

OBJECTIVE: To compare outcomes between patients admitted to different clinicians within skilled nursing facilities for post-acute care, leveraging the plausibly random distribution of patients to admitting clinicians in the case of clinicians who specialize in nursing facility care (SNFists). We also compare patient outcomes between SNFists who are physicians versus advanced practice providers (APPs).
STUDY SETTING AND DESIGN: We used multi-level modeling to estimate within-SNF variation in the characteristics and outcomes of patients admitted to different SNFists and linear regression to com

BACKGROUND: Adolescent transition programs are patient education programs. They are geared towards enabling adolescents with chronic or long-term illnesses to become active partners in their health care and manage their own health. Although there is agreement about their importance, there is not an agreement on content or how they should be delivered. The study reported here was part of the first steps of an action research project.
AIM: Our aim was to explore how health professionals understand the program at our hospital, and their opinions of its implementation. This would advance our know

Overtreatment of diabetes mellitus and hypertension resulting in iatrogenic hypoglycemia and hypotension are especially common in vulnerable older adults and can result in excess morbidity, mortality, and health care costs. In this article, we review the prevalence of these conditions and the incidence of their complications. We recommend that physicians who provide primary and specialty care, as well as advanced practice practitioners (nurse practitioners and physician associates) who write prescriptions and orders for these treatments be incentivized to avoid these harms through carefully de

The increasing prevalence of chronic respiratory diseases and the longer survival of patients with these disorders have lead to a rise in the number of individuals in a terminal condition with no chances of responding to additional medication. Nevertheless, this epidemiological fact has not been accompanied by a parallel development of palliative care programmes, and this prevents the patients from receiving optimal medical care to ensure their well-being and to avoid further unnecessary admissions to hospital. Our local experience in the Lucus Augusti University Hospital reveals that 7% of th

Described more than 150 years ago by Thomas Addison, adrenal gland dysfunction, while treatable, remains a clinically significant and potentially fatal disease. Vague and non-specific symptomatology can delay diagnosis of adrenal insufficiency and lead to adrenal crisis. Affected individuals may delay self-management due to knowledge deficits or lack of required therapies. Advanced practice nurses must remain vigilant for signs and symptoms of adrenal insufficiency and prevention of crisis. Education of patients and their caregivers/family members must emphasize early intervention with regards

Alterations in hemostasis and coagulation are common problems during critical illness. For that reason, it is essential that advanced practice and critical care nurses have an understanding of the medications used to treat these potentially deadly disorders. The antithrombotic drugs, including anticoagulants, antiplatelet agents, and fibrinolytics, are among the most frequently used drug therapies in the United States. These agents prevent and treat the thrombotic diseases, a leading cause of morbidity and mortality. Anticoagulants, the indirect and direct thrombin inhibitors as well as the vi

OBJECTIVES AND BACKGROUND: Older adults with multimorbidity and frailty frequently require palliative care, yet prescribing practices often remain focused on primary or secondary disease prevention rather than symptom relief. Polypharmacy and potentially inappropriate medications (PIMs) may undermine quality of life and alter the benefit-to-harm ratio in this population. This study evaluated the prevalence of polypharmacy, pharmacological prescribing patterns and PIMs in hospitalized older adults requiring palliative care.
STUDY DESIGN: This nested study of the Italian multicenter point-preva
