Over-the-counter products for oral ulcerations.

Over-the-counter products can be useful and effective in alleviating the pain from ulcerations of the oral mucosa. This article reviews over-the-counter medications that are available to treat ulcerative lesions. Among the categories included are covering agents, local anesthetics, and mouthrinses.

Study of anaesthetic agents for their ability to elicit porphyrin biosynthesis in chick embryo liver.

Effects of some anaesthetic drugs on the activity of delta-aminolevulinate synthetase and on the formation of porphyrins and cytochrome P-450 were studied in 18-day-old chick embryo livers in ovo. The drugs were either tested alone or with a small dose of 1,4-dihydro-3,5-dicarbethoxycollidine, which reproduces in the embryo liver a partial block in the heme biosynthesis pathway similar to that found in cells of human patients with porphyrias. Two series of local anaesthetics were tested: procaine and its derivatives (proxymetacaine, oxybuprocaine, butacaine and tetracaine) had no (or very slig

Prevalence of drug allergy in an out-patient population.

The prevalence of drug allergy in an out-patient population has been studied, 448 cases were diagnosed mainly to NSAIDS and beta-lactams and local anesthetics, sulfa drugs, quinolones and others. Clinical history and skin tests are sufficient to diagnose most cases of drug allergy.

[Allergies to dental materials and dental pharmacologic agents].

Allergic reactions to dental materials and remedies cause stomatitis in the patient and a contact dermatitis in the dental personnel. Topical contact (acrylic resin denture materials, heat accumulation, plaques on the denture) and endogenic (symptom of internal and psychiatric illnesses) factors are the cause for denture sore mouth. In metallic alloys corrosions and electric currents can effect an irritation to the oral mucosa. The application of amalgam fillings can possibly result in a mercurialism. Dental remedies used for treatment of wounds and dental roots are potent sensitizers. Surface

Cauda equina syndrome after continuous spinal anesthesia.

Four cases of cauda equina syndrome occurring after continuous spinal anesthesia are reported. In all four cases, there was evidence of a focal sensory block and, to achieve adequate analgesia, a dose of local anesthetic was given that was greater than that usually administered with a single-injection technique. We postulate that the combination of maldistribution and a relatively high dose of local anesthetic resulted in neurotoxic injury. Suggestions that may reduce the potential for neurotoxicity are discussed. Use of a lower concentration and a "ceiling" or maximum dose of local anesthetic

Localized complications from local anesthesia.

The inherent safety of local anesthetics allows practitioners to use them frequently with the confidence that adverse events are rare. Nevertheless, complications can occur. The aim of this article is to briefly review the localized adverse events that may result from local anesthetic administration. Descriptions of each complication will be followed by suggestions for prevention and management. In spite of a dentist's conscientious practice, many of these complications cannot always be prevented.

Limited dilute lidocaine anesthesia: A useful technique with many practical applications.

BACKGROUND: Limited dilute lidocaine infiltration facilitates a comfortable procedure and a rapid recovery process following a novel intervention for reduction of cellulite. Infiltration of dilute lidocaine has many other practical applications in dermatologic surgery. OBJECTIVES: This article describes a safe, effective technique for local infiltration of limited volume dilute lidocaine anesthesia in a cellulite reduction procedure. METHODS: The limited dilute lidocaine technique was utilized in studies of a novel device designed to reduce the appearance of cellulite by focal release of fib

Syringe-delivered tumescent anesthesia made easier.

A simple method for the infiltration of tumescent anesthesia is presented. An assembly is made using an infusion set, a three-way tap, and two unidirectional valves. The assembly and use of this system are straightforward and easy. The addition of unidirectional valves prevents the risk of reverse injection or aspiration of fluid from the patient.

Local anaesthesia of the skin.

Local anaesthesia of intact skin is of growing importance given the increased use of minor surgical procedures on an out-patient basis. Specific pain receptors (nociceptors) are responsible for sensing cutaneous pain. Mechanisms of pain perception and the role of local anaesthetics in reversibly blocking nociception are considered. Effective routes of administration are dermal infiltration or, more recently, anaesthesia by topical application of specifically formulated preparations which promote percutaneous absorption of the drug (percutaneous local anaesthesia). Although many other topical a

[Neonatal mepivacaine poisoning following episiotomy].

Two cases of mepivacaine-intoxication in newborn infants following local anesthesia for maternal episiotomy are presented. The accidental direct injection of local anesthetics into the fetal scalp produce acute drug poisoning in the newborn. Seizures, bradycardia, apnea, hypotonia, loss of pupillary light reflexes are the major manifestations among babies severely poisoned. The early diagnosis and therapy of this syndrome permit a more favorable prognosis without risks of perinatal death or neurologic sequelae.

Prolonged Pulseless Electrical Activity Cardiac Arrest After Intranasal Injection of Lidocaine With Epinephrine: A Case Report.

Local anesthetic toxicity is a rare but serious complication of local anesthetic administration. Although lidocaine has a safety profile superior to other amide local anesthetics, we report a case of cardiac arrest after intranasal injection of lidocaine. The case involves a 22-year-old healthy woman who experienced pulseless electrical activity shortly after a submucosal injection of 2.2 mg/kg of lidocaine with epinephrine. Resuscitative efforts were unsuccessful until a bolus of intralipid was given. This case emphasizes that even a "low" dose of a less lipophilic drug has the potential for

[Alterations in local cerebral glucose utilization during various anesthesia--the effect of urethane and a review].

Anesthetic is thought to depress the energy metabolism or oxygen consumption of the brain as a whole. It has been clear, however, that anesthetic agent may either decrease or increase local metabolic rate of the brain since [14C] 2-deoxyglucose method made one possible to measure local cerebral glucose utilization. In this paper, the authors report the effect of urethane anesthesia on the local cerebral glucose utilization. We measured local cerebral glucose utilization by means of [14C] 2-deoxyglucose method in 87 brain structures of albino rats (about 300 g) under urethane anesthesia, 1 g/kg

Articaine and neurotoxicity - a review.

The biochemical composition of articaine differs from other amide anaesthetics. The lipophilic part of articaine consists of a thiophene ring, whereas other amide anaesthetics contain a benzene ring. When used correctly, local anaesthetics are remarkably safe. However, all local anaesthetics are potentially neurotoxic. In rare cases a prolonged abnormal perception/sensation may be present after the expected duration of action (paraesthesia). In several countries retrospective studies have been conducted that examined the incidence of persistent paraesthesia after the use of local anaesthetics.
