[Urinary tract infections in diabetes mellitus].

Urinary tract infections are more frequent in diabetic patients than in non-diabetics, or take a more severe course. The difference is more pronounced in women both in symptomatic infections and asymptomatic bacteriuria. The spectrum of pathogens is similar to that of non-diabetic patients. In case of cystitis, the treatment strategy the same that is recommended in non-diabetic patients (due to lack of clinical trials) but because of the more frequent renal involvement a highly effective antibiotic therapy is suggested. In pyelonephritis, parenteral therapy and close observation are recommende

'Telephone treatment' of uncomplicated acute cystitis.

Acute cystitis in women is typically uncomplicated and amenable to empiric antimicrobial therapy. We advocate a simple approach, in which women with uncomplicated cases are treated over the telephone. Such a program has been in place at the Cleveland Clinic since 1992, and has yielded good results.

[Hemorrhagic cystitis in pregnancy: report of a case and analysis of its treatment].

Hemorrhagic cystitis is generally a benign self-limited disorder, however there are some severe cases which are associated to a significant blood loss. The etiology may be either bacterial, viral or chemical in origin; though the cause is not identified in most of the cases. Immunocompromised patients or patients who have undergone chemotherapy or radiation constitute the highest risk group. There are only a few articles about hemorrhagic cystitis in pregnancy, that is the reason why the therapy for this disorder is not uniform. Hemorrhagic cystitis in pregnant women frequently is associated w

Recognition and prevention of healthcare-associated urinary tract infections in the intensive care unit.

Urinary tract infection is the most common healthcare-associated infection in the intensive care unit and predominantly occurs in patients with indwelling urinary catheters. The predominant microorganisms causing catheter-associated urinary tract infection (CAUTI) in the intensive care unit are enteric Gram-negative bacilli, enterococci, Candida species, and Pseudomonas aeruginosa. Multidrug resistance is a significant problem in urinary pathogens. Duration of catheterization is the most important risk factor for development of CAUTI. Diagnosis, particularly in the intensive care unit setting,

Relationship between ciprofloxacin resistance and extended-spectrum beta-lactamase production in Escherichia coli and Klebsiella pneumoniae strains.

Resistance to fluoroquinolones has increased markedly since their introduction. Mechanisms of resistance to any antibiotic class might play a role in resistance to an unrelated antibiotic class. This study evaluated the relationship between extended-spectrum beta-lactamase (ESBL) production and ciprofloxacin resistance in Escherichia coli and Klebsiella pneumoniae strains isolated as causative agents of urinary tract infection. ESBL-producing strains were significantly more frequent among ciprofloxacin-resistant E. coli strains than among ciprofloxacin-susceptible E. coli strains (p = 0.015),

Intravesical Gentamicin for Prevention of Recurrent Urinary Tract Infections in Patients With Neurogenic Lower Urinary Tract Dysfunction Performing Intermittent Catheterization: A Systematic Review.

INTRODUCTION: Patients with neurogenic lower urinary tract dysfunction (NLUTD) performing clean intermittent catheterization (CIC) are at increased risk of recurrent urinary tract infections (rUTIs). Oral antimicrobial prophylaxis has limited efficacy and fosters resistance. Intravesical gentamicin (IVG) has emerged as an alternative, but evidence remains limited and heterogeneous. This systematic review evaluated the efficacy and safety of IVG for rUTI prevention in adults with NLUTD and summarized reported instillation protocols and additional clinically relevant outcomes. METHODS: A system

Pathogen distribution and antibiotic sensitivity analysis in urosepsis associated with urinary stones: A retrospective observational study.

Urosepsis is the most serious type of urinary tract infection, and its incidence is gradually increasing. The pathogenic flora that emerge in urosepsis associated with urinary stones are complex, virulent, and resistant to antibiotics, and empirical antibiotics are not always effective in treating them. This study aims to analyze the distribution of pathogenic bacteria, as well as the sensitivity of antibiotics to these bacteria, in patients with urosepsis associated urinary stones. The case data of patients treated at our single-center hospital between July 2015 and February 2025 were retrosp

The management of urinary catheters: compliance of practice in Danish hospitals, nursing homes and home care to national guidelines.

The compliance of practice to national guidelines on urethral catheter care has been examined. Questionnaires on the practices used with patients under their care were sent to 1350 nursing staff. Replies were received from 1153 individuals, 692 from hospitals, 345 from nursing homes and 116 from home care. While national guidelines stress the importance of maintaining a closed urine drainage system, the results revealed that 25.4% of respondents opened the drainage system to collect samples of urine for analysis, 57.9% to perform bladder washouts and 76% to change urine bags. 26% of respondent

Common bacterial urinary tract infections in women.

Unfortunately, there is no general consensus as to how long patients with bacteriuria or urinary tract infections should be monitored and certainly there is no agreement on how long recurrent episodes should be treated beyond ten days to two weeks. The most important points to remember are: 1. Culture the urine both at the time of therapy and during follow-up. The patient should be examined periodically for the presence of bacteruria. If bacteria cannot be eradicated, at least the physician is aware of the organism most likely causing the patient's symptoms. 2. Do not subject the patient with

Differential antimicrobial responses in polymicrobial triple-species biofilms associated with persistent urinary tract infections.

Polymicrobial urinary tract infections (UTIs) pose significant treatment challenges due to interspecies interactions within biofilms. To address this, we developed a static, triple-species biofilm model comprising Escherichia coli, Enterococcus faecalis, and Candida albicans to simulate recurrent UTI communities. Biofilms were treated with ampicillin, ciprofloxacin, fluconazole, ethylenediaminetetraacetic acid (EDTA), and N-acetylcysteine (NAC), and evaluated for biomass (crystal violet assay) and viability (CFU/mL). Ampicillin and ciprofloxacin reduced bacterial viability by > 90%, but had li

Comparison of local povidone-iodine antisepsis with parenteral antibacterial prophylaxis for prevention of infective complications of TURP: a prospective randomized controlled study.

OBJECTIVE: To determine whether antisepsis with povidone-iodine solution applied at the external urethral meatus confers protection against infective complications following transurethral resection of the prostate (TURP) and compare that with antibacterial prophylaxis. METHODS: A total of 167 patients with sterile urine undergoing TURP for benign prostatic hyperplasia (BPH) were prospectively randomized into three groups. Group A, had gauze soaked in saline applied at the urethral meatus (control group). In group B, the gauze was soaked in povidone-iodine instead of saline and group C had a s

Improving Antibiotic Prescribing for Pediatric Urinary Tract Infections in Outpatient Settings.

OBJECTIVES: To determine if a multicomponent intervention was associated with increased use of first-line antibiotics (cephalexin or sulfamethoxazole and trimethoprim) among children with uncomplicated urinary tract infections (UTIs) in outpatient settings. METHODS: The study was conducted at Kaiser Permanente Colorado, a large health care organization with ∼127 000 members <18 years of age. After conducting a gap analysis, an intervention was developed to target key drivers of antibiotic prescribing for pediatric UTIs. Intervention activities included development of new local clinical guidel
