OBJECTIVE: The purpose of this study was to determine if the prevalence of neonatal and maternal infectious morbidity in patients with preterm premature rupture of membranes (PROM) who received ampicillin prophylaxis for presemptive group B streptococcal colonization is increased compared to those who received no prophylaxis.
METHODS: The charts of all patients with preterm PROM who delivered between January 1988 and December 1993 were retrospectively reviewed. The routine use of ampicillin prophylaxis was initiated in January 1991. Patients with singleton gestations were included in the anal

BACKGROUND: The optimal duration of antibiotic prophylaxis in cirrhotic patients with variceal bleeding remains debated. Current guidelines (European Association for the Study of the Liver, American Association for the Study of Liver Disease, and Baveno VII) recommend 5-7 days of antibiotics, largely based on pre-2010 studies. While ceftriaxone reduces mortality, rebleeding, and infections, prolonged use may increase antimicrobial resistance, hospital length of stay, and costs. We evaluated whether shorter or on-demand antibiotic strategies are comparable to standard prophylaxis.
AIM: This sy…

BACKGROUND: While perioperative antibiotic prophylaxis has been widely implemented to reduce periprosthetic joint infection (PJI) rates, the efficacy of an extended oral antibiotic prophylaxis (EOAP) regimen postoperatively remains equivocal among arthroplasty surgeons. This study investigated the association of EOAP with PJI rates after primary total knee arthroplasty (TKA) using a large database and propensity-matched cohorts.
METHODS: A retrospective analysis using a national database stratified primary TKA patients into two cohorts: those who had and did not have a qualifying oral antibio…

BACKGROUND: Endoscopic lung volume reduction (ELVR) using endobronchial valves is an established treatment for advanced COPD and emphysema. To reduce procedure-related complications such as pneumonia or exacerbations, peri-interventional antibiotic prophylaxis is commonly used; however, its clinical benefit remains uncertain. We aimed to evaluate the effect of different peri-interventional antibiotic strategies in a German COPD cohort.
METHODS: Comparative analyses were performed using data from 900 patients enrolled in the multicentre, observational German Lung Emphysema Registry (LE-Registr…

Background Preoperative antibiotic prophylaxis plays a vital role in preventing surgical site infections (SSIs) and supporting antimicrobial use. Although clear international guidelines exist, gaps in antibiotic selection, dosing, and timing persist in daily surgical practice. This closed-loop audit was conducted to evaluate current adherence to international prophylaxis guidelines in general surgery and assess the impact of targeted quality improvement interventions. Methods A retrospective clinical audit was performed at the General Surgery Department of Care Medical Hospital-Alrawabi, Saudi…

BACKGROUND AND AIMS: Although beneficial in reducing the risk of bacterial infections in patients with advanced decompensated cirrhosis after upper gastrointestinal (GI) bleed, the utility of prophylactic antibiotics in those with Child-Pugh A cirrhosis is not known. We studied if prophylactic antibiotics can be withheld in this cohort.
METHODS: This was a single-centre, open-label randomised-controlled-trial with non-inferiority design. Patients of Child-Pugh A cirrhosis with upper-GI bleed and hemodynamic stability were randomised to receive either no prophylactic antibiotics (test-group) o

CONTEXT/OBJECTIVE: Despite urinary tract infections (UTIs) being a common problem in patients with spinal cord injuries (SCIs), and a well-known complication of invasive urologic procedures, little consensus exists regarding the standard of care for peri-procedural antibiotic use for SCI patients undergoing urodynamics studies (UDS). Our research seeks to evaluate local antibiotic prophylaxis pattern in SCI patients undergoing UDS, assess incidence of post-procedural UTI, describe local antibiotic resistance trends, and provide antibiotic stewardship considerations to guide future practice.

Meconium-stained amniotic fluid (MSAF) presents a complex medical scenario with significant implications for maternal and neonatal health. This case report explores the intricacies surrounding MSAF, focusing on its diagnosis, treatment, and the associated meconium aspiration syndrome (MAS). The report emphasizes the critical role of antibiotic prophylaxis in lower segment cesarean sections (LSCS) in balancing infection prevention in the mother with neonatal considerations. Additionally, it highlights personalized pain management and post-operative care regimens, contributing to a comprehensive…

OBJECTIVES: Worldwide, a number of studies have been conducted to assess the prevalence of adherence to secondary antibiotic prophylaxis and to identify the associated factors (reasons) for poor adherence among patients with rheumatic heart disease or acute rheumatic fever (RHD/ARF). However, results were highly inconsistent with a prevalence ranging from 10% to 93%; and the reported reasons or associated factors have not been systematically reviewed. Therefore, this study aimed to assess the prevalence of adherence to secondary antibiotic prophylaxis among patients with RHD/ARF; and to review…

BACKGROUND: An increase in infections after transrectal prostate biopsy (PB), related to an increasing number of patients with ciprofloxacin-resistant rectal flora, necessitates the exploration of alternatives for the traditionally used empirical prophylaxis of ciprofloxacin. We compared infectious complication rates after transrectal PB using empirical ciprofloxacin prophylaxis versus culture-based prophylaxis.
METHODS: In this nonblinded, randomized trial, between 4 April 2018 and 30 July 2021, we enrolled 1538 patients from 11 Dutch hospitals undergoing transrectal PB. After rectal swab co

BACKGROUND: Endoscopic endonasal skull base surgery (EESBS) is performed through a non-sterile corridor. Intracranial infection rates in a pediatric population have not been extensively studied and the exact effect of antibiotic prophylaxis on rates of infection after EESBS in pediatric patients remains unclear.
OBJECTIVE: The purpose of our study is to investigate risk factors for postoperative intracranial infection during EESBS in a pediatric population and help elucidate the role of antibiotic prophylaxis.
METHODS: We conducted a retrospective chart review of 99 consecutive patients unde…

BACKGROUND: Bacterial infection is a major cause of mortality in patients with cirrhosis. Spontaneous bacterial peritonitis (SBP) is a serious and common infection in patients with cirrhosis and ascites. Secondary prophylactic antibiotic therapy has been shown to improve outcomes after an episode of SBP but primary prophylaxis to prevent the first episode of SBP remains contentious. The aim of this trial is to assess whether primary antibiotic prophylaxis with co-trimoxazole improves overall survival compared to placebo in adults with cirrhosis and ascites.
METHODS: The ASEPTIC trial is a mul…

BACKGROUND: Disturbances in the early establishment of the intestinal microbiota may produce important implications for the infant's health and for the risk of disease later on. Different perinatal conditions may be affecting the development of the gut microbiota. Some of them, such as delivery mode or feeding habits, have been extensively assessed whereas others remain to be studied, being critical to identify their impact on the microbiota and, if any, to minimize it. Antibiotics are among the drugs most frequently used in early life, the use of intrapartum antimicrobial prophylaxis (IAP), p

This commentary evaluates the systematic review assessing intracameral antibiotic prophylaxis in cataract surgery complicated by posterior capsular rupture. While pooled findings suggest reduced endophthalmitis risk, reliance on observational studies, heterogeneity in antibiotic agents, and limited patient-level data restrict causal interpretation. Prospective studies with standardized protocols are needed to guide optimal prophylactic strategies.

Group B Streptococcus (GBS) colonisation affects around 13% of pregnant women and increases the risk of peripartum infection in newborns, preterm delivery and premature rupture of membranes. GBS infections are a major cause of neonatal sepsis. Early onset GBS (EOGBS) infections in the first seven days of life are particularly dangerous for newborns. Prevention is achieved by intrapartum antibiotic prophylaxis following a positive GBS screening during pregnancy. Screening strategies in pregnant women include universal GBS screening using a rectovaginal swab, assessment of maternal risk factors …
