High-dose hydroxocobalamin administered after H2S exposure counteracts sulfide-poisoning-induced cardiac depression in sheep.

CONTEXT: Severe H2S poisoning leads to death by rapid respiratory and cardiac arrest, the latter can occur within seconds or minutes in severe forms of intoxication. OBJECTIVES: To determine the time course and the nature of H2S-induced cardiac arrest and the effects of high-dose hydroxocobalamin administered after the end of sulfide exposure. MATERIALS AND METHODS: NaHS was infused in 16 sedated mechanically ventilated sheep to reach concentrations of H2S in the blood, which was previously found to lead to cardiac arrest within minutes following the cessation of H2S exposure. High-dose hydr

Management of aluminium phosphide poisoning.

This manuscript presents a clinical case of a patient poisoned by aluminium phosphide in its hydrolysed form, phosphine gas, following a suicide attempt, and its therapeutic management in the emergency room. The cardiovascular, metabolic and gasometric changes described in the pathophysiology of this poisoning were documented. There is no specific antidote, and treatment involves careful supportive care and reduction of the bioavailability of the phosphine in the poisoned patient. In this case, the patient was treated with vegetable oil, mannitol, bicarbonate and activated charcoal during the

Fentanyl-Antibody Interaction as a Novel Strategy against Opiates and Opioids Abuse.

While naloxone remains the antidote for opioid overdoses, more efficient tools are required to effectively combat this growing crisis. Vaccines and antibodies targeting substances of abuse appear to be a novel and promising approach to tackling the fentanyl and opioid epidemic. After an initial in-depth rundown on the pharmacodynamics of the substances involved from a structural and mechanistic standpoint, and a brief overview of pharmacological approaches used in clinical settings for managing overdoses and opioid addiction, this Perspective will be mainly focused on these innovative strategi

Native mercury, hydrargyrism and the birth of occupational toxicology in Colombia.

The "La Esperanza" native mercury mine in Aranzazu, (Caldas, Colombia) was active from 1948 until 1975. Before the final closure of the mine, the company began using dimercaprol (BAL, British Anti-Lewisite) and penicillamine for the treatment of hydrargyrism among workers. Mercury poisoning among miners was frequent due to precarious working conditions, inadequate technology, difficult terrain, and the high toxicity of native mercury within the mine. The costs associated with the treatment was one of the causes of the closure of the mine. The early use of these chelators corresponds to the fir

Occupational methanol toxicity: a case report study.

This case report describes a case of methanol occupational toxicity in a 44 year-old male ship worker in Jeddah, Saudi Arabia, on 27 August 2024. The patient suddenly lost sight in both eyes and had an unusual acid-base disturbance that was marked by respiratory alkalosis (pH 7.607). This is different from the acidosis that is usually seen in methanol poisoning. Despite aggressive treatment with FOMEPIZOLE, the methanol antidote, and four hemodialysis sessions, the patient's visual prognosis remained poor. This case highlights the potentially devastating consequences of inhalational and contac

Case report: Successful multimodal assessment and management of chemothorax.

Dislocation or wrong placement of central venous catheters into the pleural cavity is rare, but if undiagnosed, may cause major, sometimes life-threatening, complications (pneumothorax, hemothorax, infection, and migration) and accidental pleural effusion due to intravenous injection of fluids containing drugs (i.e. chemotherapy, antibiotics, parenteral nutrition, other). We report a rare case of pleural effusion consisting of chemotherapy (chemothorax) directly injected into the pleural cavity due to the wrong placement of a central venous catheter (Porth-A-Cath) in a woman with breast cancer

Mental retardation in Down syndrome: Two ways to treat.

Mental retardation is a progressive condition in Down syndrome: intelligence starts to decline linearly within the first year. This phenomenon could be related to the overproduction of a toxic compound, hydrogen sulfide. Indeed, a gene located on chromosome 21 controls the production of cystathionine-β-synthase, an enzyme involved in hydrogen sulfide production in the central nervous system. It has recently been demonstrated that excess cystathionine-β-synthase levels are needed and sufficient to induce cognitive phenotypes in mouse models of Down syndrome. Thus, two therapeutic options might

Purification and identification of 4-allylbenzene-1,2-diol: an antilisterial and biofilm preventing compound from the leaves of Piper betle L. var Pachaikodi.

Antibiotic-resistant food-borne Listeriosis has been rising with up to 30% mortality threat in humans since several decades. Hence, discovering antilisterial from the extracts of ethnomedicinal plants may be of value as a novel antidote. In our preceding study, we reported that ethanolic extract of Piper betle L. var Pachaikodi leaves exhibited antibacterial activity towards Listeria monocytogenes MTCC 657. Consequently in the present study, the bioactive molecule responsible for anti-Listeria activity was purified and identified as 4-allylbenzene-1,2-diol. This identified bioactive compound m

Molecular biology of botulinum neurotoxin serotype A: a cosmetic perspective.

Cosmetic use of botulinum neurotoxin serotype A (BoNT/A) involves low doses of toxin administered for facial wrinkles and hyperhidrosis. The structural and functional properties of BoNT/A can affect the degree and duration of effect. Actively using the injected muscle is favorable as it exposes more receptors to BoNT/A. Divided doses of BoNT/A at an interval of more than 3 days may be longer lasting than single dose by blocking nascent neuronal sprouts. Antibodies are unlikely to be effective in BoNT/A neutralization because of the large area of receptor interaction. Several commonly used drug

Treatment of intoxication with GV compound in laboratory rats.

Changes of some physiological functions in rats were studied following intoxication with new type of highly toxic organophosphate GV [2-dimethylaminoethyl-(dimethylamido)-fluorophosphate] in doses of 2x and 4x LD50 i.m. The intoxication has begun with increasing motoric activity followed by increased salivation, rumination and bristling. Tachypnoe and fasciculations followed by convulsions and death were observed later. Therapy of intoxication with GV compound (in dose of 2x LD50) in rats demonstrated the best antidotal effect of combination of benactyzine, atropine and HI-6. In GV intoxicatio

Shellfish and fish poisoning related to the toxic dinoflagellates.

At least four different species of the toxic dinoflagellates cause shellfish and fish poisoning in the United States: Ptychodiscus brevis, neurotoxic shellfish poisoning; Protogonyaulax catenella and P tamarensis, paralytic shellfish poisoning; and Gambierdiscus toxicus, ciguatera fish poisoning. These three disorders have similar clinical manifestations, primarily neurologic and alimentary. A complete history is essential; confirmation, while dependent on specific laboratory analysis, is usually based on a history of ingestion of fish or shellfish previously associated with these types of poi

Dissociation of increased sulfation from sulfate replenishment and hepatoprotection in acetaminophen-poisoned mice by N-acetylcysteine stereoisomers.

N-Acetylcysteine stereoisomers were compared for their ability to alter the sulfation and hepatotoxicity of acetaminophen. The clinically used L-isomer increased urinary excretion of inorganic sulfate 2-3 fold and prevented liver injury, but failed to increase acetaminophen sulfation in mice. Conversely, the nonphysiologic D-isomer failed to increase urinary excretion of inorganic sulfate or prevent hepatotoxicity, but increased acetaminophen sulfation appreciably (by 39%). The basis of the incongruence between changes in the availability of inorganic sulfate and the sulfation of acetaminophen

Cyanide Medical Countermeasure Development: Assessing the Efficacy of Intramuscular Sodium Tetrathionate for the Treatment of Acute, Severe Cyanide Toxicity in Swine (Sus scrofa).

INTRODUCTION: Given its availability and lethality, cyanide has potential for weaponization and thus has the attention of several governmental agencies. In large scale exposure scenarios, an effective countermeasure that can be administered quickly and in low volume intramuscularly may prove valuable because IV medications may have limited practical applications in these situations. Sodium tetrathionate, a potential cyanide antidote, is a compound that provides sulfur to rhodanese, the enzyme that detoxifies cyanide endogenously. Additionally, sodium tetrathionate has been reported to directly

Fomepizole as an adjunct in acetylcysteine treated acetaminophen overdose patients: a case series.

INTRODUCTION: Acetaminophen (N-acetyl-para-aminophenol or APAP) is the leading cause of acute liver failure worldwide. Standard therapy for APAP overdose is with IV N-acetylcysteine (NAC). However, overdose patients treated with NAC can still incur hepatotoxicity in some circumstances. Fomepizole has proven safety in methanol and ethylene glycol poisoning and is a potent CYP2E1 and c-Jun-N-terminal Kinase (JNK) inhibitor that is effective even in the metabolic phase. METHODS: We present a prospective case series of 14 consecutive, high-risk patients who had elevated APAP levels after overdose

Efficacy of oral administration of sodium thiosulfate in a large, swine model of oral cyanide toxicity.

INTRODUCTION: Cyanide is a deadly poison, particularly with oral exposure where larger doses can occur before symptoms develop. Prior studies and multiple governmentagencies highlight oral cyanide as an agent with the potential for use in a terrorist attack. Currently, there are no FDA approved antidotes specific to oralcyanide. An oral countermeasure that can neutralize and prevent absorption of cyanide from the GI tract after oral exposure is needed. Our objective was toevaluate the efficacy of oral sodium thiosulfate on survival and clinical outcomes in a large, swine model of severe cyanid…

Cost impact of hydroxocobalamin in the treatment of patients with known or suspected cyanide poisoning due to smoke inhalation from closed-space fires.

BACKGROUND & OBJECTIVES: Cyanide poisoning can occur due to exposure to smoke in closed-space fires. With no point of care cyanide test at the scene of a fire, first responders and clinicians base decisions to treat with cyanide antidote on patient history, clinical signs, and other indirect data points that have not been proven to correspond with actual systemic levels of cyanide. The aim of this exploratory study was to determine the economic implications of treating patients with known or suspected cyanide poisoning due to smoke inhalation with hydroxocobalamin. METHODS: A decision analysi
