73 consecutive aphakic retinal detachments were operated on by microsurgery combined with intra-operative biomicroscopic control of the ocular fundus. Retinal reattachment was achieved in 60 cases (82%), 59 eyes had only one operation. The visual, refractive, and surgical results with 190 consecutive secondary implantations of an anterior chamber intraocular lens were studied. Best-corrected postoperative visual acuity was either better than or within 1 Snellen line of best-corrected preoperative vision in 87% of the cases. Thirty-three keratophakias were performed at Holy Cross Hospital in Mission Hills, California between August 1979 and October 1982. Fifteen consecutive eyes in 14 patients (six men and eight women, ranging in age from 43 to 75 years) underwent anterior vitrectomy for chronic cystoid macular edema documented by fluorescein angiography. A contact lens made from silicone rubber has been fitted successfully for extended wear in children. The lens has been used primarily with aphakic children, but it has also been tolerated by children with anisometropia, high myopia, and keratoconus. The lens can be handled like a hard contact lens and can be fitted by a variety of methods. The eye shows excellent tolerance of the lens and complications or failures to wear the lens have been due primarily to discomfort. After about a year of wear the lens surface can no longer be cleaned.

In 650 consecutive Binkhorst lens implants, secondary membrane surgery was required more than eight times as frequently after an extracapsular than after an intracapsular cataract extraction. The visual acuity results after discussion were uniformly good. Transpars plana vitrectomy was reserved for more severe cases. The pupils were cleared, but the visual acuity results were mixed. Ninety eyes undergoing surgery for the removal of traumatic cataracts were evaluated. In the total group, the visual results fell generally into the 2 categories of satisfactory or poor. Most commonly, the latter patients sustained extensive preoperative trauma or were visually immature at the time of the injury. Forty-eight eyes, or 54% of this series, achieved an acuity of 20/80 or better, with 38, or 42%, achieving a corrected acuity of 20/20 to 20/30. There were 20 eyes in the patients 6 years of age and under, and a visual result of 20/60 or better was obtained in 7 of those eyes.

PURPOSE: To report outcomes of secondary intraocular lens (IOL) implantation in the Infant Aphakia Treatment Study (IATS).
SETTING: Multicenter clinical practice.
DESIGN: Secondary analysis of patients enrolled in a randomized clinical trial.
METHODS: Details regarding all secondary IOL surgeries conducted in children enrolled in the IATS were compiled. Visual outcomes, refractive outcomes, and adverse events at the age of 10½ years were evaluated. OBJECTIVE: To evaluate the impact on visual acuity of reducing or abandoning patching therapy during the first 6 years of life following early unilateral cataract surgery.
