Pathological changes in appendectomy specimens including the role of parasites: A retrospective study of 2400 cases of acute appendicitis.

BACKGROUND: Parasitic infestation of the appendix is uncommon. Parasitic infections can cause inflammation in the appendix and can imitate acute appendicitis clinically. Enterobius vermicularis and Taenia species are among the most common parasitic agents causing acute appendicitis. OBJECTIVE: We have studied the ratio of parasites causing acute appendicitis in appendectomy specimens and whether there is any relationship between the type of the parasite, histopathological type of acute appendicitis, age of the patient, and gender of the patient in the appendectomy cases. MATERIALS AND METHOD…

Appendectomy does not decrease the risk of future colectomy in UC: results from a large cohort and meta-analysis.

OBJECTIVES: Early appendectomy is inversely associated with the development of UC. However, the impact of appendectomy on the clinical course of UC is controversial, generally favouring a milder disease course. We aim to describe the effect appendectomy has on the disease course of UC with focus on the timing of appendectomy in relation to UC diagnosis. DESIGN: Using the National Institute of Diabetes and Digestive and Kidney Diseases Inflammatory Bowel Disease Genetics Consortium database of patients with UC, the risk of colectomy was compared between patients who did and did not undergo app…

Increased complications after appendectomy in patients with cerebral palsy: are special needs patients at risk for disparities in outcomes?

BACKGROUND: Cerebral palsy (CP) is a nonprogressive neurologic disorder. Anecdotal evidence suggests there are worse outcomes in this population after common operative procedures like appendectomy. This study aims to classify whether there are relevant disparities in postoperative outcomes in CP versus non-CP patients after open or laparoscopic appendectomy. METHODS: Hospital discharge data from the 2003-2009 weighted Nationwide Inpatient Sample were used. Unadjusted and adjusted multiple logistic regression were used to assess postoperative complications, as well as inpatient mortality, aver…

Unexpected findings on laparoscopy for suspected acute appendicitis: a pro for laparoscopic appendectomy as the standard procedure for acute appendicitis.

PURPOSE: Evaluation of the feasibility, cost-effectiveness, time of surgery, morbidities, and other/additional findings during laparoscopy for suspected appendicitis. METHODS: Prospective evaluation of 148 laparoscopies for suspected acute appendicitis. RESULTS: Laparoscopic appendectomy was safe and cost-effective. No appendiceal stump leaks or wound infections occurred. Of the patients, 4.7% developed intra-abdominal abscesses. Mean time of all procedures was 47 min: 42 min for simple appendectomies (n = 126), 67 min for perforated appendicitis (n = 15), and 75 min for converted procedures…

[Obstruction of the small intestine caused by adhesions after appendectomy. Argument for repeated surgery as proposed by Mac Burney].

After appendectomy, mechanical obstruction of the small intestine is often due to local bands. This type of obstruction is classically treated by median laparotomy. We present ten cases of such obstruction treated via the initial gridiron incision, whose advantages are discussed.

Severe acute respiratory distress syndrome following laparoscopic appendectomy in a previously healthy middle-aged man: A case report.

This case report describes an exceptionally rare occurrence of severe acute respiratory distress syndrome following laparoscopic appendectomy in a previously healthy 57-year-old male. The patient presented with acute appendicitis and underwent an uncomplicated surgical procedure. Approximately 4 h postoperatively, he developed rapid respiratory deterioration with severe hypoxemia, fulfilling the Berlin criteria for severe acute respiratory distress syndrome (partial pressure of arterial oxygen/fraction of inspired oxygen ratio, 99.2). Other causes, including aspiration, pulmonary embolism, and…

Consumption of Oral Nicotine Products in Relation to Postanesthesia Care Unit Recovery After Laparoscopic Appendectomy. A Prospective Study.

BACKGROUND: Oral nicotine product (ONP) consumption is constantly increasing while combustible cigar smoking is becoming rare. Possible effects of ONP consumption for early recovery after general anesthesia are poorly recorded. METHODS: Prospective single-center study with 151 participants undergoing laparoscopic appendectomy. Incidence of ONP consumption was recorded and consumers were compared to non-consumers intraoperatively and during postanesthesia care unit (PACU) stay. RESULTS: Of 151 patients 32 (21%, 95% CI, 15%-28%) reported regular ONP consumption and it was associated with eleva…

Selective non-operative management of post-appendectomy abscesses in children: a comprehensive systematic review and pairwise meta-analysis.

Post-appendectomy intra-abdominal abscess remains a common complication following pediatric appendicitis. Although drainage has traditionally been considered standard treatment, increasing evidence suggests selected patients may be successfully managed with antibiotics alone. A systematic review and meta-analysis were conducted in accordance with PRISMA guidelines and registered in PROSPERO (CRD420251075191). PubMed, Embase, and the Cochrane Central Register of Controlled Trials were searched through June 20, 2025. Eligible studies included pediatric patients (0-18 years) with post-appendectom…

Transversus abdominis plane block versus port site anesthetic in adult laparoscopic appendectomy: a systematic review and meta-analysis.

BACKGROUND: Despite advances in minimally invasive surgery, pain after appendectomy remains common. The transversus abdominis plane (TAP) block has been proposed as an alternative to port site local anesthetic. We aimed to synthesize randomized controlled trials (RCTs) comparing the efficacy and safety of 1) TAP block versus port site local anesthetic, and 2) ultrasound to laparoscope-guided TAP blocks following adult minimally invasive appendectomy. METHODS: Relevant databases were searched for RCTs until December 2024. Two authors independently identified trials, extracted data, assessed ri…

Incidental Visceral Peritoneal and Hepatic Sarcoidosis During Routine Laparoscopic Appendectomy: A Case Report and Implications for Laparoscopic Entry.

A 50-year-old male with a history of pulmonary sarcoidosis presented to the DC Veterans Affairs Medical Center with acute appendicitis. Laparoscopic view of the liver and peritoneum revealed studding lesions with biopsy confirmation of non-necrotizing granulomas consistent with intra-abdominal sarcoidosis. Operative and postoperative hospital courses were uncomplicated. In our case, avoidance of peritoneal sarcoid disease segments during laparoscopic entry into the abdomen may have helped reduce the chances of postoperative complications such as surgical site infection, wound dehiscence, or de

Comparison of three different methods for stump closure in laparoscopic appendectomy: Endoloop, Hem-o-lok clip, and endostapler.

BACKGROUND: Acute appendicitis is a common surgical emergency that causes acute abdominal pain and affects approximately 7-8% of the population during their lifetime. The closure of the appendix stump during laparoscopic appendectomy is one of the most critical steps of the surgery to prevent life-threatening complications such as postoperative fistula, peritonitis, and sepsis. The mate-rial chosen for appendix stump closure must be effective, safe, and economical. However, there is still no consensus on the optimal method for stump closure. In this study, we aimed to compare the advantages an…
