The clinical and autopsy findings in a patient with both systemic sclerosis (scleroderma) and giant cell arteritis are presented. Giant cell arteritis and systemic sclerosis are autoimmune diseases affecting the elderly that may be associated with similar clinical symptoms of facial pain and arthralgias. Their coexistence is virtually unknown in the medical literature but is of great clinical importance. Systemic sclerosis is not treated with the steroid dosage necessary to prevent the morbid results of giant cell arteritis. An elevated sedimentation rate and/or an abnormal temporal artery exa…

PMR and GCA are diagnosed relatively frequently in the white geriatric population. Patients may suffer from both conditions or from either alone. PMR is considered a diagnosis of exclusion, with the most characteristic symptoms being profound morning stiffness affecting the hip and shoulder girdles. The diagnosis of PMR can be buttressed by the findings of an elevated ESR, anemia, and mildly elevated liver enzymes. PMR can be controlled with less than or equal to 20 mg/d of prednisone, whereas higher doses of glucocorticoids are required for GCA. Classic GCA is characterized by headache, visua

In the authors' experience, 180 aorto-bifemoral by-pass grafts represent 42.4% of all reconstructive operations for aorto-iliac atheromatous disease performed between 1st January 1972 and 1st July 1983. 37% of the patients presented a threatened amputation and 38.8% had a walking distance of less than 100 metres. The immediate post-operative mortality (1st month) was 2.2% (4 patients). The 5 year survival, expressed according to the actuarial method, was 89% and the patency was 91%. The long-term functional deterioration was due to a progressive alteration of the distal vascular bed and to com

A 58-year-old woman had a clinical history compatible with polymyalgia rheumatica but with an unexplained interstitial lung disease. Evaluation, including biopsy specimens of temporal artery, lung, and gastrocnemius muscle, was consistent with giant cell arteritis. This case identifies giant cell arteritis as a cause of interstitial lung disease.

Of 248 patients with giant cell arteritis, 34 had evidence that the disease affected the aorta or its major branches. Symptoms suggestive of large artery involvement were intermittent claudication of an extremity, paresthesias, and Raynaud's phenomenon. Physical findings included absent or decreased large artery pulses and bruits over large arteries. Four patients presented with decreased upper extremity pulses as the initial manifestation of their arteritis. Nine other patients under treatment for temporal arteritis or polymyalgia rheumatica first developed evidence of large artery involvemen…

Eosinophilic granulomatosis with polyangiitis (EGPA, formerly Churg-Strauss syndrome) is a rare systemic vasculitis affecting small- and med … case of EGPA involving infiltrated erythema on the extremities, with different stages of cutaneous arteritis characterized by eosinophilic arteritis and granulomatous arteritis in the same affected artery. We present this as a unique diagnostic clue for EGPA.

BACKGROUND: Leflunomide (LEF) shows promising effect in Takayasu arteritis (TAK), but evidence from randomized controlled trials is lacking. This study aims to investigate the efficacy and safety of LEF versus placebo combined with prednisone for the treatment of TAK.
METHODS: This is a multicenter, randomized, double-blind, placebo-controlled trial at six sites across China, conducting from December 22, 2016, to November 4, 2022. A total of 116 eligible patients were recruited and randomized 1:1 to receive LEF (20 mg/d, p.o.) or matched placebo for 24 weeks, with all patients having initial …

Ninety patients with giant cell arteritis (GCA) were followed 3-10 years after the diagnosis. The mean observation time was 63 months. Thirteen patients died. Corticosteroids were administered to all but one patient; 35 were still on treatment after a mean observation period of 59 months. In 38 patients, 94 flare-ups of the disease were recorded during corticosteroid treatment, most of them occurring during the first year of treatment and when a low dose of prednisolone had been given. Thirty-three relapses, 76% within 3 months, occurred in 28 patients after withdrawal of treatment. One patien…

OBJECTIVES: The aim is to access the real-world clinical management of physicians who treat Takayasu arteritis (TAK) and giant cell arteritis (GCA) after the publication of the Japanese Circulation Society (JCS) 2017 Guidelines for the Management of Vasculitis Syndrome.
METHODS: This descriptive, cross-sectional study utilized self-administered electronic questionnaires, which were answered in February 2022 by physicians treating TAK or GCA and registered with Macromill Inc.
RESULTS: The 329 survey respondents were enrolled. The 2017 JCS Guidelines were the most commonly referenced informati…

Temporal arteritis (TA) is a large-vessel vasculitis mostly seen in older patients. Amyloid A (AA) amyloidosis secondary to a chronic inflammation induces multiple organ dysfunctions, including a dysfunction of the gastrointestinal tract. Herein, we present a case of TA complicated by AA amyloidosis that was resistant to oral and intravenous steroids. An 80-year-old man with a history of new-onset headache, jaw claudication, and distended temporal arteries was referred to our department. On admission, the patient presented with tenderness and a subcutaneous temporal nodule in both temple arter…
