Diagnosis of pseudogout and septic arthritis.

A patient with septic arthritis of the wrists was noted to have concurrent calcium pyrophosphate crystals in synovial fluid aspirated from both joints. These were present in unusually large numbers and were associated with destructive changes of the joints. It seemed likely that the complete dissolution of calcified cartilage accounted for the liberation of such numerous crystals. Previous reports of pseudogout associated with septic arthritis have noted the presence of abundant calcium pyrophosphate crystals in synovial fluid. It is suggested that this feature should always raise the possibil

Osteoarticular sporotrichosis in a dog.

Osteoarticular sporotrichosis was diagnosed in a dog referred for evaluation of hindlimb lameness. There was radiographic evidence of osteopenia of the fourth tarsal and proximal aspects of the metatarsal bones. The diagnosis was based on histologic findings and results of physical examination, radiography, fungal culturing, and serologic tests. The dog was treated successfully with ketoconazole for 3 1/2 months.

Septic polyarthritis in a hemophiliac.

A first report in the English literature of septic polyarthritis in a hemophiliac is reported. The patient was a 40-year-old man with classic hemophilia and an inhibitor who developed S. aureus bacteremia and multiple septic joints. The previous 4 cases with single septic joints are discussed with regard to therapy and relation to underlying joint disease and presence of antibodies to Factor VIII. Awareness that infection can occur in hemophiliacs should prompt aspiration and bacteriologic study of suspicious hemarthrotic joints.

Septic Arthritis and osteomyelitis caused by an organism of the genus Rhodococcus.

We describe a previously healthy, immunologically normal young girl who presented painless swelling of fingers, a toe, and one knee. Roentgenograms were consistent with osteomyelitis of the phalanges and knee effusion. Rice bodies (corpora oryzoidea) were identified in viscous fluid obtained from the knee during arthroscopy. Culture of this fluid grew an organism initially believed to be a member of the genus Nocardia but which was later presumptively identified as a member of the genus Rhodococcus. The patient was successfully treated with a combination of erythromycin and amoxicillin for a t

A clinical study of Osteomyelitis and septic arthritis.

Sixty-nine cases of Osteomyelitis, and 27 of septic arthritis have been presented. 69/96 (71%) were children below 12. The sites frequently involved were tibia, femur and humerus. Septic arthritis commonly involved the knee. The commonest organism was Penicillin - resistant Staphylococcus aureus. Though we sought but failed to establish that overt clinical malnutrition was aetiologically important, since 54% were well-fed children, the virulence of the sepsis in which an entire bone shaft may die is disturbing but unexplained. Whereas arthrotomy plus antibiotics gave uniformly good results for

Spectrum of gonococcal arthritis: evidence for sequential stages and clinical subgroups.

In a series of 84 patients with gonococcal (GC) arthritis, 41 were documented as follows: 17 had positive blood culture or typical skin rash (Hematogenous Group); four had positive joint culture plus hematogenous manifestations (Transition Group); and 20 had positive joint culture without positive blood culture or skin rash (Joint Localization Group). The hematogenous group, especially patients with proved gonococcemia, presented significantly earlier after onset of symptoms than patients with joint localization alone, suggesting sequential stages of disease. Patients with verified bacteremia
