Primary mediastinal histiocytic sarcoma presenting as pleural effusion.

Histiocytic sarcoma (HS) is a rare malignant neoplasm showing morphological and immunophenotypic features of mature tissue histiocytes. As HS may mimic non-Hodgkin lymphoma (NHL) pathologically, before the era of immunohistochemistry, many previously reported cases were misdiagnosed NHL. Up to date, there are only a few reports delineating the cytological features in fine-needle aspiration or bronchoalveolar lavage, but not in the effusion fluid yet. Herein, we report the case of a 61-year-old male with a mediastinal tumor presenting with malignant pleural effusion. The effusion cytology showe

Global attention facilitates the planning, but not execution of goal-directed reaches.

In daily life, humans interact with multiple objects in complex environments. A large body of literature demonstrates that target selection is biased toward recently attended features, such that reaches are faster and trajectory curvature is reduced when target features (i.e., color) are repeated (priming of pop-out). In the real world, however, objects are comprised of several features-some of which may be more suitable for action than others. When fetching a mug from the cupboard, for example, attention not only has to be allocated to the object, but also the handle. To date, no study has in

Early frontalis flap surgery as first option to correct congenital ptosis with poor levator function.

UNLABELLED: In congenital blepharoptosis the upper eyelid cannot be lifted normally because of congenital impairment in the levator function. The descended eyelid margin partially or completely obstructs of the visual axis with the consequent risk of amblyopia. Frontalis suspension is the surgery of choice for ptosis with poor levator function creating a linkage between the frontalis muscle and the tarsus; the frontalis muscle is used to elevate the eyelid. Direct transplantation of frontalis muscle to the upper eyelid has been widely described. We report our experience using frontalis flap in

Novel treatment for massive lower extremity avulsion injuries in children: slow, but effective with good cosmesis.

BACKGROUND: Extended avulsion injuries are associated with significant loss of skin and subcutaneous fat, leaving the reconstructive surgeon with the challenge of substituting all tissues lost in the best possible way. We wanted to test whether the combined use of a Vacuum Assisted Closure system (VAC) and Integra Dermal Regeneration Template (IDRT) matched the required treatment profile encompassing initial control of infection, remodeling of body contours, and reconstruction of near normal skin. MATERIALS AND METHODS: 4 children with massive lower extremity avulsion injuries were treated wi

Differences between current and historical breast cancer axillary lymph node irradiation based on arm position: implications for radiation oncologists.

PURPOSE: To identify differences in regional node irradiation using historical treatment planning techniques between 2 arm positions. MATERIALS AND METHODS: Sixteen breast cancer patients were scanned using a wide-bore computed tomography (CT) scanner. The patients were scanned in 2 arm positions: historical position (HP), in which the ipsilateral arm is at 90 degrees to the body axis; and standard-bore position (CT-P), in which the arms are above the head. The locations of the axillary lymph nodes were compared between the 2 positions. The dose distribution to the axillary lymph nodes was co

The Rapid Evolution of Continuous Blood Pressure Measurement: Future Considerations.

BACKGROUND: There exists an accelerating pace of availability and expanding variety of new blood pressure (BP) measurement technologies worldwide that seek to measure BP in entirely different ways than through the traditional "BP Cuff" commonly used for office and home measurements. A growing number of these devices are viewed by many clinicians and patients as potentially complementary sources of data when combined with office and home readings. However, wearable devices are intentionally designed to provide a more patient-centered experience and may have an important future potential to repl

Rethinking Breast "Non-Reconstructive" Reconstruction: Expanding the Spectrum with Aesthetic Flat Closure and Goldilocks Reconstruction.

BACKGROUND: Aesthetic flat closure (AFC) and Goldilocks reconstruction (GL) are increasingly utilized for patients seeking prosthesis-free breast reconstruction after mastectomy. Both techniques leverage the inferior adipodermal flap (IAF) to achieve contour restoration, yet remain underrepresented in reconstructive algorithms. This study evaluates outcomes of AFC and GL performed using a standardized IAF-based approach. METHODS: A retrospective review of patients undergoing mastectomy with AFC or GL reconstruction using the IAF from April 2021 to August 2024 at a single institution was perfo

Rapidly Polymerizing Click Hydrogel Provides Localized Delivery of rhBMP2 to Promote Bone Formation.

Delivery of bioactive agents to achieve tissue regeneration at targeted sites with minimal side effects requires the use of biodegradable carriers and sustained release of the therapeutic at appropriate concentrations. We developed a copper-free polyethylene glycol-based click hydrogel to deliver bone morphogenetic protein-2 (BMP2), a potent regulator for bone regeneration that is currently delivered to orthotopic sites using an absorbable collagen sponge, leading to a burst release of BMP2, potentially causing ectopic bone formation. In contrast, the hydrogel is delivered as a liquid, conform

Organ-level instance segmentation enables continuous time-space-spectrum analysis of pre-clinical abdominal photoacoustic tomography images.

Photoacoustic tomography (PAT), as a novel biomedical imaging technique, is able to capture temporal, spatial and spectral tomographic information from organisms. Organ-level multi-parametric analysis of continuous PAT images are of interest since it enables the quantification of organ specific morphological and functional parameters in small animals. Accurate organ delineation is imperative for organ-level image analysis, yet the low contrast and blurred organ boundaries in PAT images pose challenge for their precise segmentation. Fortunately, shared structural information among continuous im

Are realistic details important for learning with visualizations or can depth cues provide sufficient guidance?

The optimal choice of the level of realism in instructional visualizations is a difficult task. Previous studies suggest that realism can overwhelm learners, but a growing body of research demonstrates that realistic details can enhance learning. In the first experiment (n = 107), it was assessed whether learning using realistic visualizations can be distracting and therefore particularly benefits from pre-training. Participants learned the anatomy of the parotid gland using labeled visualizations. While pre-training did not have an effect, a more realistic visualization enhanced learning comp
