PURPOSE: Surgical treatments for proximal humerus metastasis include modular prosthesis, intramedullary nailing, plate fixation, and hemiarthroplasty. However, it is controversial which surgical treatment could offer less complications and implant failure. The purpose of our study was to report the risk of complications, reoperations and revisions in patients with proximal humerus metastasis treated with modular prosthesis, intramedullary nailing, plate fixation, or hemiarthroplasty.
METHODS: We performed research in the PubMed and Scopus libraries, obtaining 2247 studies. We analyzed studies

A significant amount of all paediatric fractures are forearm fractures involving the radius, ulnar shaft, or both. As surgical stabilisation lowers the likelihood of re-displacement, surgical intervention is currently recommended over conservative treatment of such fractures involving significant displacement and angulation. Open reduction and plating can better anatomically repair the majority of fractures. Bracing is necessary for the first six to eight weeks after nailing since nailing does not give a rigid fixation. External bracing is generally not necessary for plating. In our facility,

INTRODUCTION: Intramedullary nailing is an acceptable treatment option for femoral shaft fracture in young patients but not extensively studied in the elderly with osteoporotic fractures. Plate fixation for osteoporotic femoral shaft fractures have a high rate of complications and delayed healing time, and the most acceptable treatment is intramedullary nailing. This study evaluated the healing time and incidence of complications in osteoporotic femoral shaft fractures after intramedullary nailing.
PATIENTS &METHODS: This was a retrospective study that included 16 patients above 60 years old

INTRODUCTION: The treatment options for nonunions of infraisthmal femoral shaft fractures after internal fixation are controversial. Methods such as exchanging an existing nail with a nail of a larger size, dynamization, removal of the nail followed by plating, and bone grafting have all been reported. Among those options, exchange nailing seems to be the most popular choice. In this study, a Poller screw, or an additional interlocking screw, was used in conjunction with exchange intramedullary nailing. The purpose of this study was to evaluate the effectiveness of treating femoral shaft nonun

INTRODUCTION: Humeral shaft nonunion after intramedullary nailing is a rare but serious complication. Treatment options include implant removal, open plating, exchange nailing and external fixation. The objective of this retrospective study was to determine whether augmentation plating without nail removal is feasible for treating a humeral shaft nonunion.
MATERIALS AND METHODS: Between 2002 and 2014, 37 patients (mean age 51, range 20-84 years) with aseptic humeral shaft nonunions prior to intramedullary nailing were treated with augmentation plating. The initial fractures had been fixed wit

OBJECTIVES: Intramedullary nails with special lag screw designs may provide improved mechanical performance and alleviate clinical problems. We hypothesize that the proximal design of trochanteric nails affects mechanical performance.
METHODS: Ten pairs of human cadaveric femora were implanted with 2 different short intramedullary nails without (Gamma3) and with an interlocking lag screw (Intertan). An unstable, multifragmentary, pertrochanteric fracture was created. Bones were tested in a cyclic testing protocol with increasing loads until failure simulating 1 leg stance. Stiffness, failure

The induced membrane technique (Masquelet technique) combined with the Ilizarov technique may provide a novel strategy for managing infected nonunion with angular deformity and limb shortening within a relatively short treatment period. We report a 45-year-old male with a Gustilo-Anderson type IIIA open tibial fracture complicated by fracture-related infection, 33° valgus deformity, and 50-mm limb shortening. Treatment was performed in three stages: radical debridement with rapid soft-tissue lengthening at 3 mm/day using an Ilizarov external fixator; definitive fixation with an intramedullary

The aim of this study was to evaluate the rate of open reduction and complications of elastic stable intramedullary nailing (ESIN) in treating unstable diaphyseal forearm fractures in children. We performed a retrospective review of a consecutive series of 102 paediatric patients with a mean age of 9 years (range: 7-14 years) who underwent ESIN of unstable closed forearm fractures at three different centres. Closed reduction of one or both bones was achieved in 68 (67%) patients and open reduction was required in 34 (33%) patients. The rate of open reduction in single-bone fractures (52.2%) wa

Peri-prosthetic joint infection (PJI) is a severe complication that can arise following joint replacement surgery. PJI treatment is extremely complex due to the formation of bacterial biofilms and the emergence of antimicrobial resistance (AMR) to antibiotic classes commonly used in the treatment of PJI. This critical development highlights the urgent need for novel prophylactic strategies that do not rely on conventional antibiotic agents to prevent bacterial adherence and subsequent biofilm formation on implant surfaces. This study evaluated the contact-killing properties of a supramolecular

BACKGROUND: Exchange nailing has been the favored method to treat aseptic nonunions of the femoral shaft. The recommended amount of over-reaming is at least 2 mm. The purpose of this study was to determine whether the effectiveness of nonunion treatment is greatly reduced with less than 2 mm of over-reaming.
METHODS: Aseptic nonunions of the femoral shaft were treated by exchange nailing with 1 mm (n = 37) or at least 2 mm (n = 44) of over-reaming. Union rates, union periods, and complications were compared. Indications for exchange nailing were aseptic nonunions of the femoral shaft with an

We report the case of a patient with recurrent periprosthetic infections after total knee arthroplasty associated with bone destruction and massive bone loss that was successfully treated with a 2-stage procedure-resection arthroplasty with insertion of an antibiotic-impregnated cement rod-spacer and systemic antibiotics and then a resection arthrodesis of the knee with the use of an intercalary allograft fixed with a long intramedullary nail. This technique is a viable option for the treatment of recurrent periprosthetic infections after total knee arthroplasty associated with bone destructio

INTRODUCTION: Intramedullary (IM) nailing is the classical treatment for diaphyseal fractures of the tibia. Stabilizing fractures of the distal quarter is recognized as being delicate. We report a continuous, multicenter prospective study of distal tibia-fibula fractures treated with anterograde intramedullary nailing.
HYPOTHESIS: The working hypothesis was to identify the problems encountered with IM nailing alone of distal leg fractures.
PATIENTS AND METHODS: From May 2007 to November 2008, 51 fractures in 51 patients (19 females and 32 males; mean age, 46.2 years [range, 17-93 years]) wer

BACKGROUND: Intramedullary nail breakage is an uncommon complication in patients with femoral intertrochanteric fractures treated with proximal femoral nail antirotation. Salvage surgery for nail breakages associated with delayed union or nonunion is challenging, particularly when breakage occurs within an acceptable reduction range, complicating implant selection. This study evaluated outcomes in patients with proximal femoral nail antirotation breakage, acceptable reduction, and fixation treated with the long proximal femoral nail antirotation change with distal screw dynamization and latera

BACKGROUND: Two types of implants used for the surgical fixation of extracapsular hip fractures are cephalocondylic intramedullary nails, which are inserted into the femoral canal proximally to distally across the fracture, and extramedullary implants (e.g. the sliding hip screw).
OBJECTIVES: To compare cephalocondylic intramedullary nails with extramedullary implants for extracapsular hip fractures in adults.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (June 2007), the Cochrane Central Register of Controlled Trials (The Cochrane Library

INTRODUCTION: Open tibial fractures, usually caused by high-energy trauma, are among the most common long-bone fractures. Clinically, the treatment of open tibial fractures remains a major therapeutic problem for surgeons because of the poor soft-tissue coverage and blood supply in the tibia, with resultant difficulties arising from infection and poor bone healing. In this study, we will assess the results of intramedullary flexible nailing and external fixators in the management of fractures shaft tibia (G.A. type II, IIIA, and IIIB).
MATERIALS AND METHODS: Twenty-six patients who presented
