Cannulated screw breaking in arthroscopic surgery of osteochondritis dissecans of the knee: a case report.

A 14-year-old boy with the diagnosis of osteochondritis dissecans was treated with screw fixation after a 4-year period of unsuccessful conservative care. When an attempt was made to remove the screw 12 weeks after its insertion, the screw broke at its neck releasing several small slivers of metal into the joint. The screw head was removed, but the metallic slivers were not found. The appearance of the screw at the time of partial removal suggests some type of metallurgical failure. Although the shaft of the screw remained in the bone, the lesion healed uneventfully, and the patient resumed hi

Comparison of Staple, Anchor, and Tenodesis Screw for Posterior Tibialis Tendon Fixation: A Biomechanical Analysis.

Various posterior tibialis tendon fixation techniques are described in literature. Suture anchor, staple and tenodesis screws are widely used for posterior tibialis tendon transfer, but their stiffness and the maximal ultimate failure load were not tested before. We aimed to compare the initial ultimate failure load and stiffnesses of suture anchor, staple and tenodesis screws on bovine tendon fixation to bovine metaphyseal bone. Thirty-five fresh bovine ankle joints and hooves were obtained from a local abattoir. Metatarsals bones with long extensor tendons were harvested. Staple group had 15

Three-Dimensional Patient-Specific Guides for Intraoperative Navigation for Cortical Screw Trajectory Pedicle Fixation.

BACKGROUND: Cortical bone trajectory (CBT) technique for pedicle fixation has been proposed and adopted in recent years. This technique involves a mediolateral direction and a caudocephalad path to maximize screw purchase in cortical bone. Various techniques have been proposed to increase the accuracy of screw placement. A novel technique for CBT screw placement using a three-dimensional printed patient-specific drill guide (PSDG) is presented. METHODS: CBT screw fixation combined with posterior lumbar interbody fusion was performed for reduction of an L4-5 spondylolisthesis in a 71-year-old

Is there a mechanical difference between lag screws and double cerclage?

Comminuted fractures of the long bones present problems of mechanical instability. We therefore used a plate and six screws as the main osteosynthesis combined with supplementary lag screws or cerclage wire. These two supplementary methods were compared. As a model, we used 60 plastic tibias with standard midshaft butterfly fractures. These models, depending on the osteosynthesis applied for the fixation of the butterfly onto the main fracture, were split into three groups of 20 bones. Group A1 with two lag screws, group A2 with two lag screws and two double cerclages, and group A3 with two do

Three-Dimensional-Printed Drill Guides for Occipitothoracic Fusion in a Pediatric Patient With Occipitocervical Instability.

BACKGROUND: Pediatric occipitothoracic fusion can be challenging because of small size pedicles and thin occipital bone. Three-dimensional (3D) printing technology can help with accurate screw insertion but has not been described for occipital keel plate positioning so far. OBJECTIVE: To describe the novel use of 3D technology to position occipital keel plates during pediatric occipitothoracic fixation. METHODS: A young boy with segmental spinal dysgenesis presented with asymmetrical pyramidal paresis in all limbs. Developmental abnormities of the cervical spine caused a thinned spinal cord,

Incomplete insertion of pedicle screws in a standard construct reduces the fatigue life: A biomechanical analysis.

Pedicle screws are commonly used for posterior stabilization of the spine. When used in deformed or degenerated segments, the pedicle screws are often not fully inserted into the bone, but instead the threaded portion is exposed by 1 or 2 threads to accommodate rod placement and ensure alignment between the tulip of the screw and the rod. However, broken pedicle screws have been reported with the use of this method. The aim of this study was to determine how the fatigue life of the screw is affected by not fully inserting the screw into the bone. Spinal constructs were evaluated in accordance

Reducing femoral peri-implant fracture risk through optimized plate length and screw configuration - a biomechanical study.

BACKGROUND: Locked plating of femur fractures is associated with secondary peri-implant fractures which may be a result of stress concentrations at the proximal plate end region. The aim of this study was to investigate whether the strength of healed femoral bone-locking-compression-plate constructs can be increased by modifying the screw configurations and plate length to minimize the risks of peri-implant femur fractures. METHODS: The detached shaft of a variable angle condylar locking compression plate (VA-LCP Condylar Plate; Johnson & Johnson MedTech) was fixed to the proximal two-third o

A transparotid transcutaneous approach for internal rigid fixation in condylar fractures.

Closed versus open reduction in condyle fractures is a dilemma that may torment the plastic surgeon. Although at present it is accepted that there are fractures that must be open reduced as when the middle cranial fossa or temporal fossa are involucrated, foreign body are in the joint capsule, lateral extracapsular deviation of condylar deviation, and open fractures. Risdon or retromandibular approaches are used for the treatment of fractures in the condyle neck and superior third of the lower ramus. When both approaches are used the correct placement of screws is very difficult for the followi

Non-union of the scaphoid: Russe graft vs Herbert screw.

The results of treatment in 50 consecutive patients with established non-union of the scaphoid are presented. All patients were treated under the care of a single consultant, for the same indications. Russe bone graft is compared with wedge bone graft plus Herbert screw fixation, in terms of union and function. Overall, a higher success rate was obtained by wedge graft plus Herbert screw, and a significantly better range of movement. However, Russe bone graft appears equally satisfactory for fibrous non-union.

The Effect of Intra-carpal Kirschner Wire Augmentation in Screw Fixation of Scaphoid - A Retrospective Cohort Study.

INTRODUCTION: Scaphoid fractures are most often treated with a single headless compression screw. However, intercarpal Kirschner wire (K-wire) might be added to improve stability and fracture outcomes. This study will determine if there is a difference in treatment outcome (union rate and time to union) between scaphoid fracture fixations using a single headless compression screw with and without augmentation using a intracarpal intramedullary K-wire. MATERIAL AND METHODS: We conducted a retrospective review of patients who underwent surgery for isolated scaphoid fractures over a 15 years per

Population-based design of mandibular plates based on bone quality and morphology.

In this paper we present a new population-based implant design methodology, which advances the state-of-the-art approaches by combining shape and bone quality information into the design strategy. The method enhances the mechanical stability of the fixation and reduces the intra-operative in-plane bending which might impede the functionality of the locking mechanism. The method is presented for the case of mandibular locking fixation plates, where the mandibular angle and the bone quality at screw locations are taken into account. Using computational anatomy techniques, the method automaticall

Does three-dimensional computer simulation improve results of scaphoid nonunion surgery?

The purpose of this study was to clarify the effectiveness of the three-dimensional computer simulations in scaphoid nonunion surgery. Seven consecutive clinical patients with scaphoid nonunion at the middle third comprised the study group. Surface models of the scaphoid were constructed on the computer using computed tomography data of the bilateral wrists in neutral position. The distal and proximal fragments of the nonunion model were matched to the mirror image of the contralateral scaphoid model. The rotation of the distal fragment relative to the proximal fragment was calculated, and red

Quantification of metallic artifact on CT associated with titanium pedicle screws.

BACKGROUND: In dogs undergoing vertebral column stabilization, post-operative computed tomography (CT) evaluates implant placement. The impact on the interpretation of metallic artifact associated with titanium implants in dogs remains to be established. Our objective was to quantify metallic artifact on CT associated with titanium pedicle screws. METHODS: The study design included an in vitro model and a retrospective review of 11 dogs with vertebral column stabilization. Twenty four titanium pedicle screws (6 each: 2.0 mm, 2.7 mm, 3.5 mm, and 4.5 mm) were inserted into a 20% ballistic gel,

Computerized analysis of resorbable polymer plates and screws for the rigid fixation of mandibular angle fractures.

PURPOSE: This computer-based study used finite element analysis (FEA) to assess whether rigid fixation by resorbable polymer plates and screws can provide the required stiffness and strength for a typical mandibular angle fracture. MATERIALS AND METHODS: Two separate 3-dimensional FEA models of the mandible were generated using 8-noded hexahedral elements. The jaw segments in 1 model were fixed with titanium plates and screws as those in common use today. The jaw segments in the other model were fixed with resorbable polymer plates and screws as used in a developmental product currently in tr
