Mannitol-enhanced delivery of stem cells and their growth factors across the blood-brain barrier.

Ischemic brain injury in adults and neonates is a significant clinical problem with limited therapeutic interventions. Currently, clinicians have only tPA available for stroke treatment and hypothermia for cerebral palsy. Owing to the lack of treatment options, there is a need for novel treatments such as stem cell therapy. Various stem cells including cells from embryo, fetus, perinatal, and adult tissues have proved effective in preclinical and small clinical trials. However, a limiting factor in the success of these treatments is the delivery of the cells and their by-products (neurotrophic

From pluripotent stem cells to multifunctional cordocytic phenotypes in the human brain: an ultrastructural study.

Light microscopy and transmission electron microscopy were used to investigate surgical cases in a variety of pathological conditions (thromboses, tumors, cerebrovascular malformations, Moyamoya disease) to identify and characterize different phenotypes belonging to a new interstitial cell recently described ultrastructurally in the brain and here named "cordocyte." Also, this work is an attempt to identify and characterize precursor/stem cells for cordocytic lineage in the perivascular areas, within perivascular nerves and pia mater (now considered a cordocytic-vascular tissue). Unexpected re

[Distribution of human enterovirus 71 in brainstem of infants with brain stem encephalitis and infection mechanism].

OBJECTIVE: To explore the mechanism that how human enterovirus 71 (EV71) invades the brainstem and how intercellular adhesion molecules-1 (ICAM-1) participates by analyzing the expression and distribution of human EV71, and ICAM-1 in brainstem of infants with brain stem encephalitis. METHODS: Twenty-two brainstem of infants with brain stem encephalitis were collected as the experimental group and 10 brainstems of fatal congenital heart disease were selected as the control group. The sections with perivascular cuffings were selected to observe EV71-VP1 expression by immunohistochemistry method…

Strategies for regenerating striatal neurons in the adult brain by using endogenous neural stem cells.

Currently, there is no effective treatment for the marked neuronal loss caused by neurodegenerative diseases, such as Huntington's disease (HD) or ischemic stroke. However, recent studies have shown that new neurons are continuously generated by endogenous neural stem cells in the subventricular zone (SVZ) of the adult mammalian brain, including the human brain. Because some of these new neurons migrate to the injured striatum and differentiate into mature neurons, such new neurons may be able to replace degenerated neurons and improve or repair neurological deficits. To establish a neuroregen

Dural arteriovenous fistula on the brain stem and upper cervical spinal cord - a case report -.

A 53-year-old man abruptly developed headache and unconsciousness. Brain computed tomography (CT) and CT angiography showed subarachnoid hemorrhage, intraventricular hemorrhage, and multiple tortuous vascular structures on the brain stem and upper cervical spinal cord. Four-vessel angiography displayed intradural ventral arteriovenous fistula, supplied by the left vertebral and occipital arteries. Drainage was via both sigmoid sinus and cervical venous plexus. He had been treated with transarterial coil embolization of the left vertebral artery. Subsequently, he suffered from left hemiplegia a…

Neural induction with neurogenin1 increases the therapeutic effects of mesenchymal stem cells in the ischemic brain.

Mesenchymal stem cells (MSCs) have been shown to ameliorate a variety of neurological dysfunctions. This effect is believed to be mediated by their paracrine functions, since these cells rarely differentiate into neuronal cells. It is of clinical interest whether neural induction of MSCs is beneficial for the replacement therapy of neurological diseases. Here we report that expression of Neurogenin1 (Ngn1), a proneural gene that directs neuronal differentiation of progenitor cells during development, is sufficient to convert the mesodermal cell fate of MSCs into a neuronal one. Ngn1-expressing

Early defects of GABAergic synapses in the brain stem of a MeCP2 mouse model of Rett syndrome.

Rett syndrome is a neurodevelopmental disorder caused by mutations in the transcriptional repressor methyl-CpG-binding protein 2 (MeCP2) and … e that in the MeCP2 -/y mice specific synaptic molecules and signaling pathways are impaired in the brain stem during early postnatal development. These observations mandate the search for more refined diagnostic tools and may provide a rationale for the timing of future therapeutic interventions in Rett patients.

Pediatric brain stem lesions: introduction of a scoring system for clinical evaluation and their treatment analysis.

INTRODUCTION: Brainstem lesions in pediatric age group include mainly gliomas. As these are not usually amenable to complete surgical resection, multimodality treatments are used. To assess treatment responses, Karnofsky's Performance Score (KPS) has been the gold standard; however, in pediatric patients, comprehensive neurological examination is more valuable. OBJECTIVE: Our purpose was to formulate neurological examination based scoring system and compare its effectiveness with KPS. MATERIALS AND METHODS: A retrospective analysis of 37 pediatric patients with brainstem lesions was performe

Combined high cervical spine and brain stem injuries: a complex and devastating injury in children.

BACKGROUND: In young children, high cervical spine injuries (HCSI) can result in inaugural reversible, cardiac arrest or apnea. We noted in children sustaining such injuries an unusual incidence of associated brain stem injuries and defined a special pattern of combined lesions. METHODS: Children with HSCI surviving inaugural cardiac arrest/apnea were selected for a retrospective analysis of a trauma data bank. Epidemiologic, clinical, and radiological characteristics, and outcome were reviewed and compared with those of the rest of the trauma population with severe neurologic injuries (defin…

Recurrent haemorrhage from a malignant brain stem glioma: a review of the mechanisms of tumoural haemorrhage.

The authors report a 23-year-old man who was diagnosed with a brain stem malignant glioma following his first episode of generalised seizure. This was subsequently complicated by three separate documented episodes of intracranial haemorrhage. The literature is reviewed and the pathophysiological mechanisms of tumoural haemorrhage are discussed. It is speculated that significant intratumoural arteriovenous shunting may lead to recurrent haemorrhages.

Hypothermia and recovery from respiratory arrest in a neonatal rat in vitro brain stem preparation.

This study was designed to examine the possibility that respiratory arrest during hypothermia occurs at the level of premotor or motor neurons rather than at the level of the central rhythm generator itself. Specifically, we sought to determine the consequences of hypothermic cooling until respiratory arrest, and subsequent rewarming, on neurons in the pre-Bötzinger Complex, as an indication of the output of the entire rhythmogenic network; and from cervical spinal (phrenic) ventral roots, as an indication of motor neuron output, in an in vitro neonatal rat brain stem-spinal cord preparation. …

Perinatal maturation of the auditory brain stem response: changes in path length and conduction velocity.

OBJECTIVE: The goal of this study was to correlate developmental data on brain stem auditory path length with data on auditory brain stem response (ABR) conduction time. This was done to estimate changing axonal conduction velocity during the perinatal period. DESIGN: Pathway length was determined by three-dimensional reconstruction of postmortem fetal and infant brain stems in an AutoCAD system. Brain stem conduction time was obtained from previous ABR studies of premature, term, and post-term infants. The process of correlation of path length and conduction time was based on a model of ABR …

Abnormal brain stem auditory evoked potentials in a girl with the central alveolar hypoventilation syndrome.

Brainstem auditory evoked potentials were recorded in a 3-year-old girl with the central alveolar hypoventilation syndrome (Ondine's syndrome). Abnormal findings were seen at the level of the mid to upper brain stem (wave III), which was not reproducibly recordable on the left side. This electrophysiologic abnormality is consistent with a previous finding in a patient with sleep apnea.

Organization of vestibular inputs to nucleus tractus solitarius and adjacent structures in cat brain stem.

The vestibular system is involved in maintaining stable blood pressure and respiration during changes in posture and is essential for eliciting motion sickness-related vomiting. Because the nucleus tractus solitarius (NTS) participates in the regulation of sympathetic and inspiratory outflow and the triggering of emesis, we tested the hypothesis that this region receives vestibular inputs in cats. In one set of experiments, microinjections of the tracer Phaseolus vulgaris leucoagglutinin into the medial and inferior vestibular nuclei labeled projections to the middle and lateral regions of the

[A case of acute pandysautonomia and diffuse brain stem impairment associated with EB virus infection].

A 37-year-old woman with complaints of headache and nausea presented with temporary disturbance of consciousness, bulbar palsy and ataxic speech following flu-like symptoms. After the recovery of her consciousness, she developed orthostatic syncope and numbness all over the body. When she was admitted to our hospital two months later, she showed emaciation, diminished sweat production and butterfly-patch-like pigmentation. Neurologic examinations were remarkable for anisocoric pupils that sluggishly reacted to light, impaired left facial movements, bulbar palsy, numbness of the whole body, tot

Angiotensinogen mRNA and pressor reactions to angiotensin in brain stem areas of spontaneously hypertensive rats.

The role of angiotensin (ANG II) at the tissue level, particularly in the brain, remains imperfectly defined. We measured angiotensinogen (A degrees) mRNA in the brain stems, sensory and sympathetic ganglia, and blood vessels of Wistar-Kyoto (WKY) and stroke-prone spontaneously hypertensive rats (SHR-SP) by quantitative, liquid hybridization. We micro-injected ANG II and glutamate into the brain stems of these rats to gain insight into the functional significance of our findings. A. mRNA was found in the dorsolateral, dorsomedial, and ventrolateral pons, as well as in the dorsolateral, dorsome…

Physiological localization of the facial colliculus during direct surgery on an intrinsic brain stem lesion.

We designed a method for localizing facial colliculus intraoperatively by means of weak stimulation of the 4th ventricular floor and recording the electromyographic response of the facial muscle. This method is applicable clinically to prevent 6th and 7th cranial nerve complications during direct surgery on an intrinsic lesion of the brain stem, in which the normal anatomy of the rhomboid fossa is often distorted.
