Closely similar to the mineral composition of bone, di- and tricalcium phosphates would seem of interest as oral calcium supplement; unfortunately, they are commonly regarded as non-absorbable because of their insolubility. Seven elderly patients received a constant regimen of 1500 mg of calcium a day, supplied either by diet alone, or by a hypocalcemic diet (Ca = 500 mg) supplemented with di- or tricalcium phosphate (Ca = 1000 mg). Calcium balance remained positive under the calcium phosphate treatment. Inexpensive, stable and well-accepted by patients, these salts, especially tricalcium phos…

In hypoparathyroidism the absence of parathyroid hormone leads to a reduction in the absorption of calcium by renal tubular cells. In spite of treatment with vitamin D, hypercalciuria persists and normocalcaemia can only be maintained by providing the kidney with a large load of calcium. Thiazide diuretics enhance tubular calcium reabsorption and it has been suggested that they can be used as an alternative to vitamin D. Bendrofluazide in a dose of 10 mg daily was given to 9 patients with severe hypoparathyroidism in addition to their usual treatment with calcium and vitamin D. Following the i…

The renal handling of Ca in response to Na intake was evaluated in 12 patients with hypercalciuria and active kidney stone disease. There was no depression of urinary Ca excretion in response to the hypocalciuric effect of metolazone. Patients were hospitalized and their Ca and Na excretions measured while on a 190-mEq Na, 800-mg Ca, 1200-mg PO4- diet. These measurements were then repeated after Na intake decreased to 35 mEq/day while other variables, including diuretic dose and Ca intake, were unchanged. Two distinct responses were elicited by patients after Na restriction. In group I or the

Calcium channel blockers have a selective action on the cardiovascular system. They reduce the energy requirement of the heart, reduce vascular smooth muscle tone, and increase systemic blood flow. Vasodilatation occurs in both the systemic and the pulmonary systems to an extent proportional to the baseline level of vascular resistance, and results in reduction of blood pressure when it is elevated. Thus, these blockers are useful in patients with high blood pressure. Clinical experience of calcium channel blockers in hypertension is so far confined almost exclusively to verapamil and nifedipi…

Calcium metabolism has been investigated in patients with essential hypertension and normal renal function to evaluate the renal calcium handling and the reported increase in renal calcium loss. In 55 hypertensive and 55 sex- and age-matched healthy normotensive subjects creatinine clearance, serum total and ionized calcium, plasma parathyroid hormone and 24 h urinary excretion of calcium, sodium and cAMP were measured. In a subgroup of 20 hypertensive patients and 20 controls the fasting calcium excretion rate was also measured. Both 24 h and fasting calcium excretion rates were higher in the…

The synergistic effect of serum on the drug combination of salicylhydroxamic acid plus glycerol, which is active against Trypanosoma brucei, is due to diffusible calcium ions. The synergistic activity can be removed by dialysis of the serum or by addition of calcium chelating agents. A buffer containing calcium can mimic the synergistic activity of serum. This finding may have important implications in the clinical management of African trypanosomiasis in humans. Calcium also has a synergistic effect on melarsoprol, the only drug available for treating sleeping sickness patients with central n…

Duodenal and ileal calcium transport was studied by 45Ca uptake and estimation of unidirectional fluxes in vitro in kidney-intact and 5/6-nephrectomized rats. After normal-calcium diet, calcium transport was impaired by uremia in the duodenum but not in the ileum. However, 5/6-nephrectomized rats on low-calcium diet showed, in concert with impaired growth, a reduced calcium transport both in duodenum and ileum. Comparing data after normal- and low-calcium diets, the ileal adaptation to low-calcium diet was intact in mild renal failure but abolished in severe uremia (urea less than 100 mg/dl). …

The role of calcium in gastrin release was investigated using rat antral organ culture. During the initial 4-h culture interval, in the absence of calcium in the culture medium, gastrin release was not different from that observed with 0.5, 1, and 2 mM calcium. However, after 6 h of culture, gastrin release with 0.5, 1, and 2 mM calcium was significantly greater than that with antral explants cultured in calcium-free media. In the presence of 1 mM calcium in the culture medium gastrin release was stimulated by dibutyryl cyclic AMP, whereas with calcium-free culture medium dibutyryl cyclic AMP …

Serious controversy pervades the scientific study of radio-frequency (RF) radiation and its biological effects. The issues range broadly from international differences in safe exposure standards to questions pertaining to the neurological symptoms purportedly induced by electromagnetic radiation. In a more specialized vein, there is great concern in the discipline about the influence of different sources of radiation on the activity of calcium in the brain. A principal and very realistic reason for this concern stems from the pivotal importance of calcium ions in the normal functioning of the …

Forty-nine patients with gout, many with hypertension and/or renal calculi, were given hydrochlorothiazide, furosemide, or ticrynafen. Diuresis and increased clearances of sodium (Na), potassium (K), chloride (Cl), and calcium (Ca) occurred after a single dose of hydrochlorothiazide, 100 mg, or furosemide, 40 mg, orally. There was very slight change in urate and phosphorus clearances. With prolonged use of hydrochloride or furosemide, diuresis and increased electrolyte excretion disappeared. Urate and Ca excretion fell with hydrochlorothiazide. With long-term use of furosemide, urate excretion…

The disposition of 45Ca was studied in equine parathyroid gland slices. This preparation was shown to be viable by its ability to carry out protein synthesis, by the responsiveness of its respiration to effectors of oxidative phosphorylation, and by its calcium-inhibitable PTH secretion. 45Ca accumulates in the slices as the extracellular calcium is raised through the range in which physiological serum calcium concentration lies. The tissue slices behave as if they have no calcium pump: (a) depriving the cells of energy by treatment with antimycin A does not admit more calcium to the tissue, (…

In a woman with bilateral recurrent calcium nephrolithiasis and hypercalciuria, hypercalcemia developed and she underwent parathyroid surgery, which led to excision of a histologically-confirmed adenoma. The patient became normocalcemic but remained hypercalciuric despite reduction of dietary calcium intake. Several calculi recurred in both kidneys. Four to six years after parathyroidectomy, hypercalcemia recurred and the patient underwent a new surgical exploration; a parathyroid gland with diffuse adenomatous aspects and another gland with pure hyperplasia were excised. Once again, after sur…

I describe 110 patients in whom diseased and destructed bone was restored to normal healthy bone. The first ten patients were healed by using bismuth-iodoform, and the following 100 patients were treated using the hemihydrate of calcium sulfate (CaSO4.2H3O). Both alloplasts were replaced in all patients by normal healthy cancellous bone within two to four months. It is a safe procedure in which bone regeneration is stimulated to a maximum rate. During June 1974, bismuth-iodoform was used as an allograft. On July 9, 1974, the first three operations in the history of otorhinolaryngology, to my k…

Decreases in cerebrospinal fluid (CSF) calcium accompany mood elevation and motor activation in depressed patients undergoing treatment with ECT, lithium, and total sleep deprivation. Similarly, decreases in CSF calcium occur during acute psychotic agitation or mania. On the other hand, periodic recurrences of such agitated states are accompanied at their onset by transient increases in serum calcium and phosphorus. Several observations suggest that such serum ion shifts may trigger the more enduring and opposite shifts in CSF calcium and, in turn, the manic behavior. Progressive restriction o…

Weanling male rats were fed for 24 days on a meat diet. Some received supplements of CaCO3, but others did not. For the final 14 days of the experiment some of the rats were given massive doses of retinol as retinyl acetate, but others were undosed. Some of the rats not given retinol had their food intakes paired with those given retinol. Skeletal examinations were made by radiography, and postmortem by measurement of the femur ash contents. The meat diet regularly reduced the femur ash to about half its normal level, but the CaCO3 supplements prevented any reduction. Skeletal fractures occurr

To determine the site of mineralization in calcifying tendinitis, we studied the ultrastructure of tendon fragments that were obtained during surgical interventions on ten patients with that condition. We found that calcification was occurring extracellularly and essentially in areas where the tendon had undergone cartilaginous transformation. Calcific crystals were present in matrix-vesicle-like structures that were seen either singly among collagen fibers or in aggregates. When they were in aggregates, the calcifying vesicles tended to fuse with each other. Thus, the process of mineralizatio
