Regeneration of the periodontium using enamel matrix derivative in combination with an injectable bone cement.

OBJECTIVES: Enamel matrix derivative (EMD) has proven to enhance periodontal regeneration; however, its effect is mainly restricted to the soft periodontal tissues. Therefore, to stimulate not only the soft tissues, but also the hard tissues, in this study EMD is combined with an injectable calcium phosphate cement (CaP; bone graft material). The aim was to evaluate histologically the healing of a macroporous CaP in combination with EMD. MATERIALS AND METHODS: Intrabony, three-wall periodontal defects (2 × 2 × 1.7 mm) were created mesial of the first upper molar in 15 rats (30 defects). Defec

An uncommon cause of cemented unicompartmental knee arthroplasty failure: fracture of metallic components.

PURPOSE: Despite good overall clinical results, unicompartmental knee replacements (UKR) are not without their problems and failures have been reported. The most common causes of UKR failure are component loosening, poor patient selection, poor surgical technique, polyethylene wear and progression of arthritis in other compartments. The purpose of this study is to present a series of atraumatic fractures of metallic components in a UKR treated in a single orthopaedic centre. METHOD: Since 1999, 121 failed unicompartmental knee arthroplasties have been referred to our centre. In six of these,

Magnitude of cement-device interfacial stresses with and without tibial stemming: impact of BMI.

Patients expect their total knee arthroplasty to relieve pain and to be long lasting. With patients becoming more active, weighing more, and living longer, this expectation becomes increasingly more difficult to fulfill. Patients who are obese and active put greater loads on their implants and may have a greater risk of failure. Although much attention has been paid to decreasing polyethylene wear, a major cause of implant failure, very little research focus has been directed to elucidate other measures to reduce failure, such as the efficacy of prophylactic stemming of the tibial tray. This s

An ex vivo exothermal and mechanical evaluation of two-solution bone cements in vertebroplasty.

BACKGROUND CONTEXT: Previous ex vivo studies showed that the properties of commercial cements modified for use in vertebroplasty are not optimal and are associated with several drawbacks, including high exothermic reaction, low cement viscosity and consequent extravasation, and unpredictable wait time after cement preparation. Additionally, strength and stiffness restoration are controversial varying with the cement type, volume injected, and technique used. PURPOSE: To investigate maximum polymerization temperatures and mechanical performance of novel two-solution bone cement (TSBC) modified

Two-stage total infected knee arthroplasty treatment with articulating cement spacer.

INTRODUCTION: The treatment of infected total knee arthroplasty (TKA) is controversial and various. Two-stage prolonged reimplantation and 6-week systemic antibiotics use have been a gold standard of treatment in recent years. PATIENTS: Seventeen knees of 17 patients, who underwent primary TKA and subsequently developed infections, were implanted articulating antibiotic-loaded cement spacer through two-stage reimplantation. In the postoperative period, parenteral antibiotic treatment targeting the specific microorganism detected in each patient was started and continued with oral administrati

Using bone cement for surgical treatment of bilateral osseous temporomandibular joint ankylosis.

Temporomandibular joint (TMJ) ankylosis with complete immobility of lower jaw represents a maximal functional impairment and causes malocclusion and impaired function of mastication. Main goal of surgical treatment of patients with TMJ ankylosis is regaining of organ function and good cosmetic results. Authors modified the implant of articular head by bone cement for surgical treatment of TMJ ankylosis (five cases). Mandibular arch osteotomy on both sides was performed under nasotracheal anesthesia. 1.5-2 cm bone conglomerate was removed and articular fossa was formed in remaining part. Titan

Calcium phosphate cement as a "barrier-graft" for the treatment of human periodontal intraosseous defects.

BACKGROUND: Calcium phosphate cements (CPC) are apparently good candidates for periodontal treatment by virtue of their biocompatibility, mouldability and osteoconductivity. However, the clinical efficacy in this regard has not been established. This study is aimed at the evaluation of the efficacy of a formulation of CPC in healing human periodontal intraosseous defects in comparison with hydroxyapatite ceramic granules. MATERIALS AND METHODS: In this clinical study, 60 patients with periodontal defects were divided into 2 test groups and 1 control group. The defect sites in the test groups

Characterization and cytocompatibility of nano-hydroxyapatite/chitosan bone cement with the addition of calcium salts.

In the present study, a calcium carbonate and calcium oxide loaded nano-hydroxyapatite/chitosan bone cement was developed and its in vitro cell responses were investigated. The physico-chemical properties were represented in terms of setting time, compressive strength, and wettability. The results show that the setting time is about 15 min, the compressive strength is up to 17.80 +/- 0.45 MPa throughout the immersion period, and the static contact angle of the cement against water is 33.5 degrees +/- 2.7 degrees, indicating that the cement has good setting time, compressive strength, and wetta

Photocatalytic cementitious materials: influence of the microstructure of cement paste on photocatalytic pollution degradation.

Incorporation of nanophotocatalysts into cementitious materials is an important development in the field of photocatalytic pollution mitigation. In this study, the photocatalytic nitrogen oxides (NO(x)) conversion by titanium dioxide (TiO(2)) blended cement pastes was used as a standard process to evaluate the internal factors that may influence the depollution performance. The chemical composition and microstructure of the TiO(2) modified cement pastes were characterized and analyzed. The active photocatalytic sites related to the surface area of TiO(2) are the key factor in determining the p

Clinical and biological assessment of cemented titanium femoral stems: an 11-year experience.

This study prospectively assessed the outcome of 134 cemented titanium stems and serum ion levels. The stems were polished (0.1 microm Ra) with circular cross section. At the end point, only one stem revision was performed for aseptic loosening, and two were planned due to subsidence greater than 5 mm. Non-progressive radiolucencies in zones 1 and 7 were observed in 16 hips at the cement-bone interface without osteolysis. Median serum titanium concentrations were below the detection limit (30 nmol/l) except in patients with failed stems. The overall stem survival rate was 97.7% at nine years,

Dual mobility cemented cups have low dislocation rates in THA revisions.

UNLABELLED: THA revisions using standard cups are at risk of dislocation (5.1% to 14.4% incidence), especially in patients over 70 years of age. Constrained tripolar cups have reduced this risk (6% incidence) but are associated with substantial loosening rates (9%). The nonconstrained dual mobility cup was designed to improve prosthetic stability (polyethylene head >or= 40 mm diameter) without increasing loosening rates by reducing wear and limiting impingement (rotation range of 108 degrees). We implanted 88 cemented dual mobility cups for THA revisions in 82 patients at high risk of dislocat

Radiographic and pathological stages of the changes at the bone-cement interface: an in-vivo experimental study.

INTRODUCTION: Chemical and physical effects of cementation cause radiographic and histological changes at bone-cement interface. These changes can be of interest in the assessment of the residual lesions and subsequent recurrences after local resection and cementation of local aggressive tumours. AIM: The aim of the study was to evaluate the evolution and determine the stages of the changes that occur at the bone-cement interface after cementation of cavitary lesions. MATERIAL AND METHODS: We operated on 16 hind legs of 8 sheep (Ovies Aries) under general anaesthesia (Xylasin HCl, Ketamin HC…

Serious metallosis of a metal head due to fragmented ceramic screws in a cemented THA.

We reported a rare revision case necessitated by massive metallosis of a metal head due to a fragmented ceramic screw which had been extraarticularily used in the primary THA here and reviewed previous literature about metallosis of metal heads associated with ceramic materials. The ceramic screw had been used for the fixation of a bone graft and reattachment of the greater trochanter in the primary THA. Scanning electron microscope analysis revealed large alumina ceramic particles embedded on the bearing surface of the polyethylene. It has previously been pointed out that metallosis of an inn

The long-term results of press-fit cemented stems in total knee prostheses.

We evaluated the long-term fixation of 64 press-fit cemented stems of constrained total knee prostheses in 32 young patients with primary malignant bone tumours. Initial stable fixation, especially in rotation, was achieved by precise fit of the stem into the reamed endosteum, before cementation. Complementary fixation, especially in migration and rotation, was obtained by pressurised antibiotic-loaded cement. The mean age at operation was 33 years (13 to 61). No patient was lost to follow-up; 13 patients died and the 19 survivors were examined at a mean follow-up of 12.5 years (4 to 21). Stan…

Modified PMMA cements for a hydrolysis resistant metal-polymer interface in orthopaedic applications.

Amongst the many factors influencing the long-term stability of cemented hip prostheses, the interface between the implant and bone cement is considered to be one of the most susceptible to failure. Osteolysis and loosening of the implant can occur by the interaction of mechanically and/or hydrolytically induced bond failure of the metal-cement interface. In this work, an improvement of the hydrolysis resistance of the titanium-bone cement interface was obtained by cement modification with a bifunctional coupling agent combined with a tribochemical TiO2-modification of the metal surface. Effect of cement volume and placement on mechanical-property restoration resulting from vertebroplasty.

BACKGROUND AND PURPOSE: Concern exists that vertebral bodies augmented with cement placed laterally may be at risk of collapse on the nonaugmented side. The purpose of the current study was to determine if placing the cement laterally rather than centrally resulted in risk of collapse on the nonaugmented side. METHODS: Vertebral bodies (L2-L5) were harvested from eight osteoporotic female cadaver spines. Simulated vertebral-body compression fractures were created and stabilized by injecting 3.5 or 7.0 mL of cement centrally or laterally. Vertebral bodies were recrushed to measure their augmen
