Sex-Related Differences in the Developmental Morphology of the Atlas: A Computed Tomography Study.

STUDY DESIGN: A retrospective study. OBJECTIVE: To elucidate sex-related differences in the age at synchondroses closure, the normative size of the atlas, and the ossification patterns of the atlas in Japanese children. SUMMARY OF BACKGROUND DATA: The atlas develops from three ossification centers during childhood. The anterior and posterior synchondroses, which are separate ossification centers, mimic fracture lines on computed tomography (CT). Sex-related differences of age dependent morphological changes of the atlas in a large sample size have not been reported. METHODS: This study anal

New Technique for C1 Double-Door Laminoplasty Using Allograft Spacers and Titanium Miniplate Screw Fixation: Technical Report.

Although conventional C1 laminectomy is the gold standard for decompression at the atlas, it provides little space for the bone graft to fuse. The fusion area can be extended cranially up to the occipital bone, but it requires sacrificing the function of the craniocervical junction. To date, no reports have focused on surgical techniques for successful decompression and fusion without disruption of the posterior C1 arch while providing enough room for the bone graft to fuse. This study introduces a new technique for C1-C2 fusion and C1 double-door laminoplasty in patients with C1-C2 instabilit

Syndromic versus nonsyndromic atlantoaxial dislocation: do clinico-radiological differences have a bearing on management?

BACKGROUND: This prospective study attempts to study the clinico-radiological differences between patients with syndromic AAD (SAAD), non-syndromic AAD (NSAAD), and AAD with Klippel-Feil anomaly (AADKFA) that may impact management. METHODS: In 46 patients with AAD [SAAD (including Morquio, Down, Larson and Marshall syndrome and achondroplasia; n = 6); NSAAD(n = 20); and, AADKFS (n = 20)], myelopathy was graded as mild (n = 17, 37 %), moderate (15, 32.5 %) or severe (14, 30.5 %) based on Japanese Orthopaedic Association Score modified for Indian patients (mJOAS). Basilar invagination (BI), bas

[Traumatic atlas fracture in a child following fall on head].

An 8-year-old girl presented at the Accident & Emergency Department with torticollis and neck pain after falling on her head from a climbing frame. A computed tomography scan revealed a Gehweiler type 1 avulsion fracture combined with widening of an anatomically variant single anterior synchondrosis. The child was successfully treated with halter traction, painkillers and bed rest. Physicians should look out for upper cervical spine injury in children who sustained a cranial axial load injury. Differential diagnosis between pain-reactive muscular torticollis and atlantoaxial rotatory subluxati

Craniovertebral junction neoplasms in the pediatric population.

INTRODUCTION: The incidence of tumors at the craniovertebral junction in the pediatric population is low. Because of the variable pathology and the rarity of these tumors, ideal therapies are only now being defined. MATERIALS AND METHODS: Thirty-eight children with tumors affecting the craniocervical junction were encountered between 1991 and 2006. These comprised neoplasms of osseous origin and neural extramedullary tumors. RESULTS AND DISCUSSION: Chordomas of the clivus and foramen magnum were seen in eight, fibrous dysplasia in four, aneurysmal bone cysts in four, eosinophilic granuloma a

Odontoid Lateral Mass Interval (OLMI) asymmetry and rotary subluxation: a retrospective study in cervical spine injury.

The aim of the study was to evaluate the significance of odontoid lateral mass interval (OLMI) asymmetry on open-mouth view, in neck injury patients who have otherwise normal cervical spine x-rays. Thirteen neck injury patients were reviewed. Average age was 23.5 years (range, 16-30 years) and average follow-up was 15 weeks (range, 6-39 weeks). All the patients, referred from the casualty department, had computed tomography (CT) scans because of the OLMI asymmetry to rule out rotary subluxation. Clinically none of the patients had torticollis. CT scans were reported normal except for OLMI asym

CT of the main ligaments of the cervico-occipital hinge.

The results of a CT-anatomical correlative study of the main ligaments of the cervico-occipital hinge are reported. CT criteria of normal ligamentous structure of this region are presented in axial, coronal and sagittal views with a special attention to the transverse ligament of the Atlas. Examples of pathological transverse ligaments are illustrated and emphasis is laid upon the better statement CT allows in such cases.

Euthanasia: Manual versus Mechanical Cervical Dislocation for Broilers.

The aim was to assess the onset of brain stem death for two euthanasia methods-manual cervical dislocation (CD) versus the Koechner Euthanizing Device (KED). Over three days broilers of 36 (n = 60), 42 (n = 80), or 43 days old (n = 60) were euthanized. On days 2 and 3, a treatment was added in which the bird's head was extended at a ~90° angle after application of the KED (KED+). On those days, gap size was recorded between the skull and atlas vertebra by 1-cm increments. The onset of brain death was assessed by recording the nictitating membrane reflex, gasping reflex and musculoskeletal move

Intraoperative dynamic angiography to detect resolution of Bow Hunter's syndrome: Technical case report.

BACKGROUND: Bow Hunter's syndrome is a rare form of vertebrobasilar insufficiency that may be successfully treated by surgical intervention. Use of intraoperative dynamic transcranial Doppler ultrasound for surgical treatment of vertebrobasilar insufficiency has been described in literature. However, this technique was inconsistent and unreliable in some patients. We present a case of a patient with Bow Hunter's syndrome treated surgically and emphasize the valuable addition of intraoperative dynamic angiography to determine resolution of vertebral artery compromise. CASE DESCRIPTION: The pat

Halo-vest treatment of unstable traumatic cervical spine injuries.

Eighty-three patients with unstable cervical spine injuries were treated with halo-vest stabilization in a prospective consecutive series during a 10 year period. At the follow-up 2-7 years after the trauma, six patients had died and eight patients had been surgically stabilized. Sixty-seven of the remaining 69 patients (97%) were subjected to the follow-up performed with validated protocols. All patients but three were reexamined clinically and radiographically. Flexion-extension motion and sidebending of the neck was measured radiographically. Rotation was measured with the aid of a compass

[Transoral approach for the treatment of atlanto-axial dislocation with the Os odontoideum and obstruction of the bilateral vertebral arteries (author's transl)].

The two approaches are well-known in the surgical treatment of atlanto-axial dislocation, the one is the anterior and the other is the posterior approach. The indication of these methods is still controversial. This report concerns with one case of atlanto-axial dislocation with the separate odontoid and bilateral obstruction of both vertebral arteries, which was successfully treated by transoral decompression and fusion between the bodies of the atlas and axis. The reasons why we chose this method were: (1) the spinal cord decompression was necessary because of the presence of compression of

CPPD crystal deposition disease of the cervical spine: a common cause of acute neck pain encountered in the neurology department.

BACKGROUND: Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is one of the most common forms of crystal-associated arthropathy in the elderly. However, CPPD deposition on the cervical spine is less well known, and only a limited number of cases have been reported to date. Here, we report our recent clinical experience with CPPD crystal deposition disease of the cervical spine and describe the clinical features of this disease. METHODS: Fourteen patients with clinically diagnosed CPPD crystal deposition disease of the cervical spine at our department during the period from Jan

Evaluation of cervical posture of children in skeletal class I, II, and III.

Previous studies on the relationship between morphological structure of the face and cervical posture have predominantly focused on vertical dimensions of the face. The aim of this study was to investigate whether there are significant differences in cervical posture in subjects with a different sagittal morphology of the face, i.e., a different skeletal class. One hundred twenty (120) children (60 males and 60 females, average age 9.5 yrs., SD+/-0.5) were admitted for orthodontic treatment. Selection criteria was: European ethnic origin, date of birth, considerable skeletal growth potential r

TP53 mutants and non-HPV16/18 genotypes are poor prognostic factors for concurrent chemoradiotherapy in locally advanced cervical cancer.

Targeted sequencing for somatic mutations across the hotspots of 50 cancer-related genes was performed using biopsy specimens to investigate whether clinicopathological factors and genomic alterations correlated with prognosis in locally advanced cervical cancer. Seventy patients diagnosed with International Federation of Obstetrics and Gynecology (FIGO) stage III to IVA cervical cancer underwent radiotherapy or concurrent chemoradiotherapy at the National Cancer Center Hospital between January 2008 and December 2017. Mutations were detected in 47 of 70 [67% of cases; frequency of genetic alte

Posterior osteosynthesis with monoaxial lateral mass screw-rod system for unstable C1 burst fractures.

BACKGROUND CONTEXT: Surgical treatment for unstable atlas fractures has evolved in recent decades from C1-C2 or C0-C2 fusion to motion-preservation techniques of open reduction and internal fixation (ORIF). However, regardless of a transoral or a posterior approach, the reduction is still not satisfactory. PURPOSE: The article describes and evaluates a new technique for treating unstable atlas fractures by using a monoaxial screw-rod system. STUDY DESIGN: This is a retrospective study. PATIENT SAMPLE: The sample includes adult patients with unstable C1 fractures treated with a posterior mon

Positive Outcomes Following Cervical Acceleration-Deceleration (CAD) Injury Using Chiropractic BioPhysics® Methods: A Pre-Auto Injury and Post-Auto Injury Case Series.

This series illustrates how rear-end impact motor vehicle collisions (MVCs) alter the cervical spine's alignment and demonstrates therapeutic use of cervical extension traction to improve lordotic alignment and other outcomes. This is a retrospective reporting of 7 adult patients (4 males and 3 females, 28-42 years) treated for cervical hypolordosis. These subjects received Chiropractic BioPhysics® (CBP®) rehabilitation and then were involved in a rear-end MVC. All cases had radiographic assessment that quantified the buckling of the cervical spine, presumably resulting directly from the CAD t
