Fatal embolism of the anterior spinal artery after local cervical analgetic infiltration.

A 66-year-old man received a local infiltration of cortisone and xylocain in the left paravertebral region between the fifth and sixth cervical vertebrae. Respiratory failure occurred 2.5 h later with subsequent successful resuscitation. During the following 2 months of assisted ventilation up to the patients death, tetraplegia with concurrent full consciousness throughout was observed. Magnetic resonance imaging confirmed an ischaemic lesion of the upper anterior cervical myelon. The forensic autopsy revealed pneumonia as cause of death. Neuropathology confirmed anterior infarction at the C2/…

The Klippel-Feil syndrome: a constellation of deformities.

The clinical triad, described by Klippel and Feil in 1912 and consisting in a short neck, a low dorsal hairline and restricted neck mobility, is the result of synostosis of several cervical vertebrae, which may or may not be deformed. In recent years, however, even more attention has been given to associated urological, neurological, cardiopulmonary and locomotor deformities, which significantly increase the morbidity. A multidisciplinary approach and treatment are therefore required. We have studied 18 patients and examined 6 additional records. Of these 24 cases, 18 showed associated deformi…

Cervical spondylotic radiculo-myelopathy in patients with athetoid-dystonic cerebral palsy: clinical evaluation and surgical treatment.

The acute onset of symptoms of severe cervical radiculo-myelopathy in four patients with athetoid-dystonic cerebral palsy is reported. Neurological and radiological examination showed that the spondylotic changes of the cervical spine were responsible for new neurological deficits leading to the patients being bedridden. Dystonic-athetoid neck movements may cause excessive axial neck rotation as well as flexion and extension movements of the spine. These repetitive exaggerated movements may result in early degenerative changes of the vertebrae which may enhance the radiculo-myelopathy. The fou

Craniofacial and extracranial malformations in the Klippel-Feil syndrome.

The Klippel-Feil syndrome is of special interest to those concerned with cleft palate for the following reasons: (1) Cleft palate is a commonly associated finding: (2) malformations of the cervical vertebrae, in the absence of other stigmata of the syndrome, are a common finding in cleft palate; (3) anomalies of the upper cervical column and cranial base can impede velopharyngeal valving; (4) hearing loss is a common finding in the syndrome irrespective of the presence or absence of cleft palate; (5) cervical anomalies may complicate endotracheal intubation or head extension during pharyngeal …

Biomechanical analysis of C4-C6 spine segment considering anisotropy of annulus fibrosus.

Based on computed tomography scanning images of human cervical spine, this paper develops a detailed and anatomically accurate finite element model for C4-C6 motion segment to investigate its biomechanical responses. The model includes vertebrae, intervertebral disc, and various ligaments. The annulus fibrosus is characterized by using a previously developed nonlinear, anisotropic, hyperelastic fiber-reinforced model with fiber-matrix shear interaction considered. Other materials are simplified as linear or nonlinear elastic. The proposed cervical model is first validated by comparing numerica

A new system of classification for spinal injuries.

BACKGROUND CONTEXT: A comprehensive classification for spinal trauma has proved difficult to achieve as attested to by the number of systems in use today. In addition, few systems can be applied to all levels of the spine, and the first two cervical vertebrae are usually treated as altogether separate from the remainder of the spine. Consequently, outcome data and comparison of published data are difficult, at best. PURPOSE: The purpose of this presentation is to introduce a new system for classification of spine trauma applicable to all levels of the spine. STUDY DESIGN/SETTING: A different…

Cervical spine chordoma: Imaging features and diagnostic challenges of a rare C6 localization.

Chordomas are rare malignant tumors arising from remnants of the embryonic notochord. They are slow-growing but locally aggressive. They most frequently occur in the sacrococcygeal and skull base regions, while cervical spine involvement is rare. Their proximity to critical neurovascular structures makes diagnosis and management challenging. We report the case of a 62-year-old man presenting with a progressive left cervical mass, dysphonia, and a foreign body sensation in the lower neck. Clinical examination revealed a firm, immobile cervical swelling with mild displacement of adjacent larynge

[Anterior hyperostosis of the cervical spine: diagnostic studies and surgical therapy].

BACKGROUND: Dysphagia due external compression by anterior hyperostosis of the cervical spine is rare. The diagnosis may be established by conventional X-ray of the spine, esophagogram, and CT. PATIENTS: We operated on three patients with large anterior osteophytes from C3 to C7. In two cases morphologic changes of the cervical spine were the main cause of dysphagia. One patient with progressive hypopharynx cancer had hyperostosis of cervical spine as secondary findings. RESULTS: The patients were asymptomatic, post-operatively. CONCLUSIONS: Cervical osteophytes can be detected in 20-30% of

Surgical treatment for patients with cervical flexion myelopathy.

OBJECT: Cervical flexion myelopathy is a rare condition that mainly affects adolescent boys. In recent years, avoidance of neck flexion has been advocated as the treatment for cervical flexion myelopathy, and treatment with a cervical collar and surgery in which fusion of the cervical spine is performed have been found to be effective. However, previously reported series contained only a limited number of patients. The authors report their experience with treating 10 male patients in whom surgery was performed to correct cervical flexion myelopathy, and they evaluate the patients' surgical out

Congenital lower brachial plexus palsy due to cervical ribs.

Congenital brachial plexus palsy (CBPP) usually occurs secondarily to intrapartum trauma, but this is not always the case. Cervical ribs have previously been reported to increase the risk of CBPP in association with birth trauma. We report the cases of two children (one female, one male) with congenital lower brachial plexus palsy in whom the presence of non-ossified cervical ribs was the only identified risk factor. In the female child magnetic resonance imaging (MRI) of the brain, spinal cord, and brachial plexus revealed no abnormality except for the presence of bilateral cervical ribs at t

Intramedullary hydatid cyst of the cervical spine.

Hydatid disease (Echinococcosis) is a common parasitic infection caused by Echinococcus granulosus mainly in sheep-raising areas of the world. Liver, lungs and brain are the predominantly involved organs. However, 0.5-1% of the hydatid disease involves the spine and in 90% of the cases it is confined to the bone and the epidural space. Although intramedullary involvement is extremely rare, in this report, we present a 55-year-old female patient who was diagnosed with a cervical intramedullary hydatid cyst during magnetic resonance imaging of the cervical vertebrae. Accordingly, we imply that p…

Towards an anthropomorphic MRI phantom mimicking static and dynamic B0 field variations in the human cervical spinal cord.

OBJECTIVE: Spinal cord MRI is heavily affected by static and dynamic B0 field variations. The aim of this study was to develop an anthropomorphic cervical spine phantom that replicates these. MATERIALS AND METHODS: The phantom consisted of a 3D-printed head-to-thorax shell containing 3D-printed vertebrae (C1-T1) in a water-based liquid solution. An external respiration system mimicked breathing-induced field fluctuations by moving metal staples over the phantom. The accuracy of the reproduced fields was evaluated at 7 T by field maps and multi-echo GREs acquired in the phantom and in four sub

Postcranial axial skeletal pathology in Commerson's dolphins Cephalorhynchus c. commersonii from Tierra del Fuego, Argentina.

Commerson's dolphins Cephalorhynchus c. commersonii are the small cetacean most frequently found stranded along the coast of Tierra del Fuego, Argentina, but little is known about their pathologies. We examined the postcranial axial skeleton of 425 Commerson's dolphins collected in the period 1974-2011 for the presence of pathologies. Miscellaneous lesions were detected in 107 (25.2%) of the specimens. Among them, idiopathic hyperostosis was the most frequent pathology (73.8%), followed by spondyloarthropathy (40.2%). Traumata (25.2%), spondylitis (10.3%) and osteoarthritis (11.2%) occurred le

Segmental relationship between somites and vertebral column in zebrafish.

The segmental heritage of all vertebrates is evident in the character of the vertebral column. And yet, the extent to which direct translation of pattern from the somitic mesoderm and de novo cell and tissue interactions pattern the vertebral column remains a fundamental, unresolved issue. The elements of vertebral column pattern under debate include both segmental pattern and anteroposterior regional specificity. Understanding how vertebral segmentation and anteroposterior positional identity are patterned requires understanding vertebral column cellular and developmental biology. In this stu

[Klippel-Feil syndrome with tracheoesophageal fistula, bifid thumb and cerebral angiolipoma.].

The Klippel-Feil syndrome is a congenital malformation of the skull flap involving complex cervical vertebrae and organs, characterized by a classic triad: short neck, limitation of movement of the head due to cervical spine fusion and low hairline in occipital region. It results from an error in the axial skeleton segmentation of the embryo; its incidence is estimated at 1/40,000-42,000 births and predominates in females. The aim of this paper is to describe the clinical picture of a patient with Klippel-Feil syndrome and multiple malformations, including tracheoesophageal fistula, bifid thum…

Intra-operative Iso-C3D navigation for pedicle screw instrumentation of hangman's fracture: a case report.

Pedicle screw instrumentation of the upper cervical spine is rarely performed in trauma surgery because of the risk of damaging neurovascular structures. We report successful treatment of an unstable hangman's fracture with posterior pedicle screw fixation using Iso-C3D fluoroscopy-based computer navigation guidance. Postoperative computed tomographic images confirmed accurate placement of the pedicle screws. The navigation system is useful, especially in an unstable upper cervical spine injury where the likelihood of change in the inter-segmental relationship is maximal before and after posit
