Posttraumatic stress disorder among foster care alumni: the role of race, gender, and foster care context.

Little is known about the prevalence of posttraumatic stress disorder (PTSD) in adult alumni of foster care and its demographic and contextual correlates. This is one of the first studies to report on racial/ethnic and gender differences and the influence of foster care experiences (i.e., revictimization during foster care, placement change rate, and placement in kinship care) on past year PTSD. Findings revealed significant gender disparities and a moderating influence of kinship care on the relationship between gender and PTSD, as well as increased risk associated with a history of emotional

Factors associated with cancer-related fatigue in advanced hepatocellular carcinoma patients during the peri-intervention period: a path analysis model.

Cancer-related fatigue (CRF) is a significant concern in oncology, yet limited research has focused on its associated factors and pathways in hepatocellular carcinoma (HCC). This study aimed to identify the factors associated with CRF in advanced HCC patients and to explore the interrelationships among these factors using pathway analysis. This cross-sectional study enrolled participants from a tertiary cancer hospital. Data were collected through self-administered questionnaires assessing demographics, disease characteristics, positive emotions, physical activity (PA), and CRF. Multiple linea

Patterns of health care use that may identify young children who are at risk for maltreatment.

OBJECTIVES: Early identification of children who are at risk for maltreatment continues to pose a challenge to the medical community. The objective of this study was to determine whether children who are at risk for maltreatment have characteristic patterns of health care use before their diagnosis of abuse or neglect that distinguish them from other children. METHODS: We performed a case-control study among Medicaid-enrolled children to compare patterns of health service among maltreated children in the year before a first report for abuse or neglect that led to an immediate placement into f

Consideration of opioid agonist treatment in a pregnant adolescent: A case report and literature review.

Background: Opioid use greatly increases the risk of overdose death, as well as contracting human immunodeficiency virus (HIV) and hepatitis. Opioid agonist treatment is recommended for pregnant women who are dependent on opioids. However, there is a dearth of studies on the use of opioid agonist treatment in pregnant teenagers. Case: Ms. A, a 15 year-old G1PO in foster care, presented to our tertiary women's hospital requesting opioid agonist treatment for use of pill opioids. She reported nasal inhalation of 5-6 opioid tablets daily, with recent attempts to self-taper using nonprescribed bup

Exploring challenges and opportunities in paediatric bereavement care: a qualitative study from a tertiary care hospital in Pakistan.

BACKGROUND: In Pakistan, most children with life-threatening illnesses present at advanced stages and succumb to death, leaving families emotionally traumatised. Parental bereavement leads to long-term psychosocial and financial hardships for families, thus highlighting the need for bereavement care as a crucial part of paediatric palliative care. This study explores key challenges around lack of bereavement care in the healthcare infrastructure in Pakistan and identifies opportunities to establish this programme for grieving families. METHODOLOGY: This study presents a secondary qualitative a

Exploring Nursing Care Coordination Through Swanson's Theory of Caring: Perspectives of Families of Children and Youth With Special Health Care Needs.

PURPOSE OF STUDY: Caring for children and youth with special health care needs can be overwhelming for families. Higher levels of need often require more extensive and coordinated support. The burden of caring for children with complex health care needs can result in negative effects for parents and caregivers over time, thus increasing the risk of poor physical, emotional, and social well-being. Nursing care coordination is an evidence-based intervention that provides families with guidance and support by delivering safe and effective care. The quality of coordination, however, can profoundly

Composition of the gut microbiota transcends genetic determinants of malaria infection severity and influences pregnancy outcome.

BACKGROUND: Malaria infection in pregnancy is a major cause of maternal and foetal morbidity and mortality worldwide. Mouse models for gestational malaria allow for the exploration of the mechanisms linking maternal malaria infection and poor pregnancy outcomes in a tractable model system. The composition of the gut microbiota has been shown to influence susceptibility to malaria infection in inbred virgin mice. In this study, we explore the ability of the gut microbiota to modulate malaria infection severity in pregnant outbred Swiss Webster mice. METHODS: In Swiss Webster mice, the composit

Long-term cardiometabolic effects of early institutionalization and foster care: Evidence from the Bucharest Early Intervention Project.

Early psychological deprivation is associated with increased risk for cardiometabolic disorders, and rapid catch-up growth following improved care environments may further influence this risk. Using data from the Bucharest Early Intervention Project - a randomized longitudinal study of children institutionalized in infancy assigned to high-quality foster care, continued institutional care, or raised by biological families - we examined cardiometabolic outcomes in early adulthood. Participants were 81 women and 58 men (mean age 22.6 years), predominantly Romanian. We assessed associations betwe

The effects of early experience on face recognition: an event-related potential study of institutionalized children in Romania.

Data are reported from 3 groups of children residing in Bucharest, Romania. Face recognition in currently institutionalized, previously institutionalized, and never-institutionalized children was assessed at 3 time points: preintervention (n = 121), 30 months of age (n = 99), and 42 months of age (n = 77). Children watched photographs of caregiver and stranger faces while event-related potentials were recorded. Results demonstrate that institutionalized children show pervasive cortical hypoarousal in response to faces and that foster care is somewhat effective in remediating this deficit by 42

Fetal alcohol spectrum disorder in Israel: increased prevalence in an at-risk population.

BACKGROUND: Maternal exposure to alcohol during pregnancy can lead to a wide range of clinical manifestations in their offspring, termed fetal alcohol spectrum disorder (FASD). In Israel, relatively few cases of FASD have been diagnosed and the prevalence has not been systematically evaluated. OBJECTIVES: To determine the number of children with FASD or at risk for FASD in a select population of high risk patients seen at a clinic evaluating foster and adopted children. METHODS: Israeli children under 2 years old who were candidates for domestic adoption or in foster care were prospectively

Home visits during pregnancy and after birth for women with an alcohol or drug problem.

BACKGROUND: One potential method of improving outcome for pregnant or postpartum women with a drug or alcohol problem is with home visits. OBJECTIVES: To determine the effects of home visits during pregnancy and/or after birth for pregnant women with a drug or alcohol problem. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Trials Register (30 April 2004), CENTRAL (The Cochrane Library, Issue 2, 2004), MEDLINE (1966 to April 2004), EMBASE (1980 to week 16, 2004), CINAHL (1982 to April 2004), PsycINFO (1974 to April 2004), citations from previous reviews and trials, and con

A best-worst scaling experiment to identify patient-centered claims-based outcomes for evaluation of pediatric antipsychotic monitoring programs.

OBJECTIVE: This article employs a best-worst scaling (BWS) experiment to identify the claims-based outcomes that matter most to patients and other relevant parties when evaluating pediatric antipsychotic monitoring programs in the United States. DATA SOURCES: Patients and relevant parties, with pediatric antipsychotic oversight and treatment experience, completed a BWS experiment, including policymakers (n = 31), foster care alumni (n = 28), caseworkers (n = 23), prescribing clinicians (n = 32), and caregivers (n = 18). STUDY DESIGN: Respondents received surveys with a scenario on antipsycho

Ethanol ingestion in two infants under 2 months old: a previously unreported cause of ALTE.

The differential diagnosis for the infant presenting with an apparent life-threatening event (ALTE) is broad. Toxic ingestions are a relatively uncommon cause of an ALTE, although several over-the-counter, prescription, and illicit drugs have been implicated. We present 2 cases of ethanol intoxication in infants as a previously unreported cause of an ALTE. Additionally, serial ethanol levels for these patients offer novel insight into the pharmacokinetics of ethanol metabolism in infants. Ethanol ingestion may be an underrecognized cause of an ALTE and should be considered if the history or ph

Predictors of infant foster care in cases of maternal psychiatric disorders.

PURPOSE: Our aim was to investigate the factors associated with mother-child separation at discharge, after joint hospitalization in psychiatric mother-baby units (MBUs) in France and Belgium. Because parents with postpartum psychiatric disorders are at risk of disturbed parent-infant interactions, their infants have an increased risk of an unstable early foundation. They may be particularly vulnerable to environmental stress and have a higher risk of developing some psychiatric disorders in adulthood. METHODS: This prospective longitudinal study of 1,018 women with postpartum psychiatric dis
