Trends in blood transfusion among hospitalized children with sickle cell disease.

BACKGROUND: Blood transfusions represent a major therapeutic option in acute management of sickle cell disease (SCD). Few data exist documenting trends in transfusion among children with SCD, particularly during hospitalization. PROCEDURE: This was an analysis of cross-sectional data of hospital discharges within the Kid's Inpatient Database (years 1997, 2000, 2003, 2006, 2009). Hospitalizations for children (0-18 years) with a primary or secondary SCD-related diagnosis were examined. The primary outcome was blood transfusion. Trends in transfusion were assessed using weighted multivariate lo

Does fever phobia cross borders? The case of Japan.

BACKGROUND: Undue parental fear of fever in children was termed "fever phobia" by Schmitt following a survey in the USA in 1980. In 2000, Crocetti et al. conducted the same survey and concluded that fever phobia existed even 20 years later. In this study, we explore differences in fever phobia between these two US populations and a Japanese sample, and determine whether parents of a single child or those whose child was previously hospitalized or had a febrile seizure report greater anxiety about fever. METHODS: A questionnaire was distributed to parents of children who visited a pediatric ou

[Glucocorticosteroids in the treatment of subglottic laryngitis in children].

Subglottic laryngitis is one of the acute children's diseases, directly caused by a violently growing edema of the subglottic area. Its symptoms generally appear very suddenly, when children seem to be in perfect health, at night, several hours after falling asleep. Their symptoms included barking cough, clear voice, stridor, inspiratory dyspnoea with participation of auxiliary respiratory muscles, excitation and anxiety of a child, changes in skin coloration. The movement of the wings of the nostrils is intensified. In especially severe cases, agitation, cyanosis, pallor of skin, obnubilation

[Child abuse by intoxication].

Child abuse by toxic substances is not easy to identify due to both the lack of signs of physical violence and the high incidence of accidental poisonings. Twelve cases of documented abuse, out of 6175 poisonings in children aged 0-13 years, were registered during 1975-1990 at Genova, Torino and Trieste Children Hospitals. In other 139 children the suspicion of abuse couldn't be confirmed. An early suspicion of abuse, is needed to prevent abuse and help the child and his family.

A model for acute psychiatric inpatient treatment of latency-age children.

Beginning with the premise that acute inpatient treatment of severely disturbed latency-age children offers enormous advantages over other types of interventions, the author describes a model for such treatment. The model consists of three phases: initial, working-through, and termination. It has a psychoanalytic basis and takes into account changing factors, such as the child's symptoms and the attitudes of both the child and his parents throughout different stages of the hospitalization. A case history illustrates the various phases and the treatment interventions used.

Do no more harm: the psychological stress of the medical examination for alleged child sexual abuse.

AIM: Currently, there is some controversy that the medical examination following allegations of child sexual abuse may further traumatise the child. Access for children to appropriate care may be hindered if decisions about referral are influenced by personal beliefs, rather than by recognition of the potential health and psychological benefits of the assessment. We aimed to study the expectations and emotional responses of children and their parents to the medical examination. METHODS: We conducted a prospective quantitative and qualitative study at the Children's Hospital at Westmead. Parti

The child who has severe neurologic impairment.

The formulation of a diagnosis in a child who has neurologic impairment should not (except in extreme circumstances) be made on the basis of a single finding. For example, the presence of delayed milestones, primitive or exaggerated reflexes, abnormal muscle tone, abnormal posture, abnormal neurologic examination, and temporal course should all be used to make the diagnosis of cerebral palsy. Furthermore, because each condition has a spectrum of disabilities from mild to severe involvement, determination of the child's abilities and impairments should be part of the diagnostic process. Because

Heliox: a new treatment for life-threatening asthma.

Rates of asthma are increasing in adults and children. In some pediatric institutions, asthma is the most common admitting diagnosis. At University of Chicago Children's Hospital (UCCH), at the University of Chicago Hospitals, the use of helium-oxygen mixtures (heliox) has been explored for treatment of children in status asthmaticus. Helium has a low density, and therefore has an increased flow rate which results in less airway resistance, decreased work of breathing, minimized airway collapse, and less hyperinflation. Little research on heliox and asthma has been done with children. A small

A screening protocol for child abuse at out-of-hours primary care locations: a descriptive study.

BACKGROUND: Child abuse is often unrecognized at out-of-hours primary care (OOH-PC) services. The aim of our study was to evaluate the clinical outcome of the screening instrument SPUTOVAMO-R2 for child abuse (checklist), followed by a structured approach (reporting code), at OOH-PC services. The reporting code with five steps should ensure consistent action in case of a suspicion. METHODS: All children attending one of the five participating OOH-PC services in the region of Utrecht, the Netherlands, in a year time, were included. The checklist is an obligatory field in the electronic patient
