PURPOSE OF REVIEW: Benign pleural effusions (BPEs) are more common than malignant effusions and their treatment involves treating the underlying disease process. However, in cases refractory to medical management, pleural interventions might be necessary. Use of indwelling pleural catheters (IPCs) has been gaining popularity for patients with refractory effusion. In this review, we will focus on reviewing the data assessing safety and efficacy of IPC in patients with BPE related to congestive heart failure (CHF), hepatic hydrothorax, end-stage renal disease (ESRD) and chylothorax.

Thoracoscopy offers numerous diagnostic and therapeutic options in pediatric surgery. This paper reviews some of the principal thoracoscpic procedures performed in children. Thoracoscopy for empyema, pectus excavatum, chylothorax, etc., will be discussed. Thoracoscpy has been postulated to offer similar results as conventional surgery with lower morbidity and reduced hospital stay. These assumptions still await systematic studies , however.

We report a case of abdomino-thoracic oesophageal duplication revealed by prenatal ultrasonography as a liquid mass in the posterior mediastinum, responsible for a major chylothorax. Tow our knowledge, this is the first thoroughly documented case occurring in the prenatal period. The various possible ultrasonographic diagnoses and the possibility of therapeutic drainage of pleural effusions are discussed in the light of anatomy, embryogenesis and ultrasonographic images of the lesion.

The principal types of reflux of chyle are reviewed in relation to the different pathological localizations at which it may occur, these including chylothorax, chylopericardium, chyloperitonitis, chylous ascites, exudative enteropathies, chyluria, genital chyledema and chylometrorrhea and chylocele, etc. A new approach to therapy of these conditions has been provided by microsurgical techniques which constitute presently an effective procedure for many cases.

A chyliform (pseudochylous) effusion is an uncommon disease entity which develops after an exudative effusion is in situ for a long time. It is commonly initiated by a tuberculous pleurisy and it is important to exclude the presence of active tuberculosis. We present a case with the characteristic features. Chylothorax was excluded by demonstrating in the pleural fluid a relatively low level of triglycerides with a cholesterol/triglyceride level of less than one and abundant cholesterol crystals.

Involvement of pleura by sarcoidosis remains a rare manifestation and varies from pleural effusion, pneumothorax, pleural thickening, hydropneumothorax, trapped lung, hemothorax, or chylothorax. Sarcoid pleural effusions presenting as hemorrhagic effusions are even more rare. We report a case of active pulmonary sarcoidosis presenting as hemorrhagic pleural effusion requiring tissue diagnosis to rule out malignancy. The rarity of the presentation prompted us to report this case.

There is no agreement whether prophylactic thoracic duct ligation (TDL), with or without resection, during esophagectomy for patients with cancer is beneficial. The effects of these procedures on postoperative complications and overall survival remain unclear. This systematic review included 16 articles. TDL did not influence short- and long-term outcomes. However, thoracic duct resection increased postoperative chylothorax and overall complications, with no improvement in survival.

We present a fascinating case of major penetrating trauma to the chest which resulted in a successful outcome. Often such trauma is associated with a poor outcome due to injury to the heart, lungs or major blood vessels with subsequent massive blood loss, pnuemothorax or haemo-pneumothorax. Late complications include infection rarely mediastinitis, empyema and occasionally chylothorax from damage to the thoracic duct.

We report a 27-year-old Caucasian woman with Jaffe-Campanacci syndrome. She presented with multiple café-au-lait spots and multiple non-ossifying fibromata of the bones, noted on radiographs and proven by tissue biopsies. Only the left side was involved. She also suffered from moderate scoliosis, stenosis of the aortic isthmus, chylothorax, and a chylopericardium. The etiology is discussed.

Generalized lymphangiomyomatosis with diffuse pulmonary involvement was diagnosed at post-mortem of a 26-year-old woman. The disease occurs only in women, during or after their reproductive phase, with chylous effusion, especially chylothorax, dyspnoea and chronic progression. It is a multitopic condition starting with a focal lesion, which has its origin in the smooth musculature of the lymphatics and probably develops in dependence on hormonal factors.

We describe a trisomy 21 patient with postnatal diagnosis of atrioventricular septal defect with isolated ventricular component who had a complicated post-surgical course following complete repair. Clinical outcomes included moderate-severe residual atrioventricular valve regurgitation needing re-operation, complete heart block leading to pacemaker insertion, seizures secondary to subdural haemorrhages, and chylothorax. We describe the surgical considerations specific to this cardiac morphology.

The use of talc for pleurodesis is a controversial issue. I have used talc in patients with malignant pleural effusion, recurrent pneumothorax, chylothorax and in selected cases of empyema. We present a case of a 58 year-old woman with primary chylopericardium associated with chylothorax. Chylopericardium is a condition in which chylous fluid containing a high concentration of triglycerides accumulates in the pericardial cavity, and it can form for many different reasons. 3D computed tomography with lymphography precisely depicted the specific location of the lymphatic leak in this patient, which was successfully repaired using targeted video assisted thoracic surgery (VATS).
