Detail Coverage and Evidence Integration: Evaluating ChatGPT 5.0 and DeepSeek R1 in Guideline-Based Crohn's Disease Applications.

BACKGROUND: Crohn's disease (CD) requires accurate, guideline-based diagnosis and management, yet the growing complexity of therapeutic options and international recommendations creates challenges for timely clinical decision-making. Large language models (LLMs) have emerged as potential tools for clinical decision support, but their performance in guideline-based gastroenterology remains insufficiently characterized. METHODS: We conducted a structured comparative evaluation of ChatGPT 5.0 and DeepSeek R1 using 47 standardized clinical questions derived from the 2023 Chinese guidelines for th…

Large language models for clinical decision support in gastroenterology and hepatology.

Clinical decision making in gastroenterology and hepatology has become increasingly complex and challenging for physicians. This growing complexity can be addressed by computational tools that support clinical decisions. Although numerous clinical decision support systems (CDSS) have emerged, they have faced difficulties with real-world performance and generalizability, resulting in limited clinical adoption. Generative artificial intelligence (AI), particularly large language models (LLMs), are introducing new possibilities for CDSS by offering more flexible and adaptable support that better

Predictors of Surgical Decision-Making After Neoadjuvant Chemotherapy for Breast Cancer.

BACKGROUND: Following neoadjuvant chemotherapy (NAC), a subset of breast cancer patients becomes candidates for breast conserving surgery (BCS) after tumor downstaging. In the literature, there is little analysis of patient choice after NAC. The goal of this study is to investigate clinical and demographic predictors of patients' surgical choices after NAC to better inform shared decision-making. PATIENTS AND METHODS: A retrospective chart review was conducted on breast cancer patients at our institution who became BCS candidates after receiving NAC from January 1, 2010 to September 30, 2020.

Intricacies of human-AI interaction in dynamic decision-making for precision oncology.

AI decision support systems can assist clinicians in planning adaptive treatment strategies that can dynamically react to individuals' cancer progression for effective personalized care. However, AI's imperfections can lead to suboptimal therapeutics if clinicians over or under rely on AI. To investigate such collaborative decision-making process, we conducted a Human-AI interaction study on response-adaptive radiotherapy for non-small cell lung cancer and hepatocellular carcinoma. We investigated two levels of collaborative behavior: model-agnostic and model-specific; and found that Human-AI

Decision-making criteria in polygenic embryo screening: A survey of reproductive medicine physicians.

PURPOSE: Polygenic embryo screening (PES) examines embryos for their genetic likelihood of developing complex conditions and traits. The commercialization of PES places reproductive endocrinologists and infertility specialists (REIs) as the primary decision-makers in its use, despite professional guidelines cautioning that PES is not ready for clinical use. METHODS: This survey examines how 152 US REIs perceive and approach potential clinical decision-making scenarios regarding the use of PES. RESULTS: REIs were more likely to offer PES for medical conditions (up to 62% for cancer) than for …

De-escalating locoregional radiation therapy in breast cancer: balancing promises and prudence.

INTRODUCTION: The integration of primary systemic therapy (PST) into breast cancer management increasingly shapes locoregional treatment. Rising pathological complete response rates and improved survival have fueled interest in radiation therapy (RT) de-escalation to limit late toxicity while preserving oncological control. Yet the risk of undertreatment and its delayed consequences remains a major concern. AREAS COVERED: This article appraises evidence supporting response-adapted RT de-escalation after PST. We review key prospective and retrospective studies, including RAPCHEM and NSABP/B-51

Using Tree-Based Reinforcement Learning Methods to Support Personalized Decision-Making in Hand Treatment.

Personalized treatment enhances healthcare by tailoring optimal decisions to each patient based on their specific characteristics and treatment history. Reinforcement learning (RL) methods are powerful tools for estimating optimal, data-driven, dynamic treatment decision rules. This article presents a tutorial on Tree-based RL and Multi-Objective Tree-based RL for advancing the estimation of optimal dynamic treatment regimes. Data from the Silicone Arthroplasty in Rheumatoid Arthritis study demonstrate their application in optimizing joint arthroplasty decisions. These methods support personal

Are Clinical Decision Support Systems Compatible with Patient-Centred Care?

Few, if any, of the Clinical Decision Support Systems developed and reported within the informatics literature incorporate patient preferences in the formal and quantitatively analytic way adopted for evidence. Preferences are assumed to be 'taken into account' by the clinician in the associated clinical encounter. Many CDSS produce management recommendations on the basis of embedded algorithms or expert rules. These are often focused on a single criterion, and the preference trade-offs involved have no empirical basis outside an expert panel. After illustrating these points with the Osteoporo

Agentic AI for Prostate Cancer: A Vision for Multimodal Clinical Intelligence.

While artificial intelligence (AI) is rapidly advancing in prostate cancer diagnostics and therapy planning, its impact on everyday clinical decision-making remains marginal. To fill this gap, "Agentic AI systems" are under development: working at an orchestration layer integrating the different AI agents, they are proposed as a powerful tool to synthesize the different clinical information, supporting clinicians' decision-making more effectively.

A CDE-based data structure for radiotherapeutic decision-making in breast cancer.

BACKGROUND: The growing complexity of oncology and radiation therapy demands structured and precise data management strategies. The National Institutes of Health (NIH) have introduced Common Data Elements (CDEs) as a uniform approach to facilitate consistent data collection. However, there is currently a lack of a comprehensive set of CDEs for describing situations for and within radiation oncology. Specifically for breast cancer, where radiotherapeutic decision-making is complex and based on multiple diverse criteria, there is a clear need for more standardized data. Aim of this study was to

Conflict and catastrophe-related severe burn injuries: A challenging setting for antimicrobial decision-making.

Severe burns are a major component of conflict-related injuries and can result in high rates of mortality. Conflict and disaster-related severe burn injuries present unique challenges in logistic, diagnostic and treatment options, while wider conflict is associated with driving local antimicrobial resistance. We present a targeted review of available literature over the last 10 years on the use of systemic antimicrobial antibiotics in this setting and, given limited available data, provide an expert consensus discussion. While international guidelines do not tend to recommend routine use of pr

Piloting a new method to estimate action thresholds in medicine through intuitive weighing.

OBJECTIVES: In clinical decision-making, physicians take actions such as prescribing treatment only when the probability of disease is sufficiently high. The lowest probability at which the action will be considered, is the action threshold. Such thresholds play an important role whenever decisions have to be taken under uncertainty. However, while several methods to estimate action thresholds exist, few methods give satisfactory results or have been adopted in clinical practice. We piloted the adapted nominal group technique (aNGT), a new prescriptive method based on a formal consensus techni…

Laying the foundation for Real-world evidence studies: a case study from Newfoundland and Labrador.

The Janssen and Newfoundland and Labrador Health Innovation Partnership (JANL-HIP) was established to carry out Real-World Evidence (RWE) projects to generate evidence about disease pathways, healthcare delivery, the effects of clinical interventions. Doing so will support and influence clinical decision-making in Newfoundland and Labrador (NL). This case study describes the foundational elements necessary for a real-world evidence generation project in NL and may provide learning for the effective execution of real-world studies in other jurisdictions. It uses an ongoing project in psoriatic …

Wait-and-See Approach or Gluten-Free Diet Administration-The Rational Management of Potential Coeliac Disease.

Potential celiac disease (PCD) is a heterogeneous disease; only some patients develop full celiac disease (CD), characterised by advanced atrophic changes in the small intestine. Few accurate prognostic factors exist for the progression of PCD; therefore, therapeutic decisions should be made on an individual basis in each case. Patients with clinical gastroenterological or parenteral symptoms often benefit from a gluten-free diet, and those left on a diet containing gluten should receive clinical, serological and histopathological supervision.

Strategies in Surgical Decompression for Thyroid Eye Disease.

Surgical management of thyroid eye disease- (TED-) associated morbidity has been plagued by the complex interplay of different operative techniques. Orbital decompression is the well-recognized procedure for disfiguring exophthalmos and dysthyroid optic neuropathy (DON). There are numerous published techniques described for the removal of the orbital bone, fat, or a combination. The diverse studies are noncomparative as they include different indications, stages of disease, and methods of evaluation. Thus, it is difficult to conclude the most efficient decompression technique. To obtain effect
