Intense Imagery Movements (IIM): More to motor stereotypies than meets the eye.

AIM: A subgroup of children who present with motor stereotypies in the context of episodes of intense imagery have recently been described in the literature,(1) termed Intense Imagery Movements (IIM). All children report conscious engagement in acts of imagery or imagination, with stereotyped movements occurring simultaneously with limited conscious awareness. This article reports preliminary cognitive data to inform clinical management and guide future research. METHOD: Intellectual functioning was assessed for ten children with IIM (7 boys, 3 girls; mean age = 10;01, age range = 6;06 to 14;

Phenotypic cognitive impairment in late-onset delusional disorder.

BACKGROUND: Previous use of heterogeneous diagnostic criteria and insensitive cognitive measures has impeded clarification of the extent and type of cognitive impairment specific to late-onset delusional disorder. We examined whether clinical presentations of late-onset delusional disorder are associated with prodromal or established dementia, and whether it might be a discrete clinical syndrome characterized by its own profile of cognitive impairment. METHOD: Nineteen patients with late-onset delusional disorder from a hospital psychiatric service and 20 patients with dementia of the Alzheim

Evaluating large language models for interpreting and applying caries guidelines in clinical decision support.

OBJECTIVE: To systematically evaluate the capability of three large language models (LLMs)—GPT-4o, Grok-3, and DeepSeek—in interpreting and … relevant treatment recommendations. This performance gap may be attributed to GPT-4o’s more robust clinical reasoning architecture and better alignment with guideline logic. For public-facing summaries, GPT-4o excelled in clarity and coherence, while DeepSeek achieved the highest accuracy in preserving source terminology. For professional summaries, all models perf …

Deconstructing Oral Surgical and Orthodontic Considerations in Complex Cases: A Case Report on Two Patients With Multiple Impacted Teeth.

Management of multiple impacted teeth in nonsyndromic adolescents can be challenging, particularly when impactions involve several quadrants. This case report describes two adolescent patients with extensive impactions affecting both permanent and supernumerary teeth, with an emphasis on the clinical reasoning that guided treatment decisions. A quadrant-by-quadrant approach was used to determine when teeth were extracted, surgically exposed, or retained based on eruption potential, root development, and surgical access. Although the anatomical complexity of the cases was similar, treatment str…

Tricuspid Regurgitation: A Review of Current Interventional Management.

Tricuspid regurgitation is a common yet clinically complex problem, traditionally managed with diuretic therapy with no observable mortality benefit. Older studies on surgical intervention observed poor outcomes; however, this clinical reasoning predates current surgical approaches and novel transcatheter technology. The tricuspid apparatus is a complex structure that poses a technical challenge for surgeons and interventional cardiologists. Recent advances in surgical techniques and transcatheter therapy, particularly edge-to-edge repair devices, have demonstrated promising results for safety…

Ultrasound to reduce cognitive errors in the ED.

Emergency medicine setting is intrinsically prone to a greater risk of medical errors than other specialties. Cognitive errors are particularly frequent when the clinical decision-making process heavily relies on heuristics. These could be defined as "mental shortcuts," which enable physicians to rapidly overcome both time and efforts required by the normative reasoning. Our article demonstrates how emergency physicians' thinking may be affected by failed heuristics, through the description of 3 real clinical cases. We aimed to show how the proper use of a widespread and easy-learning technolo

Toward truly agentic radiology AI: A multimodal architecture framework and differentiated implementation roadmap.

Agentic artificial intelligence(AI) represents a structural transition from task-specific tools to autonomous systems capable of goal-oriented, multi-step reasoning. However, translating this potential into radiology requires a fundamental shift from text-dominated architectures to avision-first approachthat engages the specialty's diagnostic core. In this paper, we characterize the multimodal challenge distinguishing radiology from text-centric domains, where high-dimensional images serve as the primary diagnostic substrate rather than supplementary data. We reviewthree image-text coupling ar

Advising the ill-advised: Debiasing the conjunction fallacy through gamified training with outcome feedback.

Many real-world decisions involve uncertainty, yet some are governed by logical constraints that make certain outcomes objectively more or less probable. Despite this, people often commit reasoning errors such as the conjunction fallacy, i.e. judging a conjunction of events as more likely than a single constituent event. This fallacy has proven robust across populations and contexts, and difficult to fully mitigate. While previous research has shown that reformulating problems or providing explicit instruction can reduce such errors, these approaches often yield limited success and may not gen

Childhood sexual abuse in first-episode psychosis: The relevance in cognitive bias.

Cognitive biases and childhood trauma have been associated with psychotic outcomes; however, the combined effect of these two variables on psychosis has been minimally explored. This study investigates differences in cognitive biases and the presence of sexual abuse between people with first episode of psychosis (FEP) and healthy controls and explores the relationship between childhood sexual abuse and cognitive biases in individuals with FEP, controlling for sociodemographic and clinical confounders. A sample of 85 FEP patients and 87 healthy controls were assessed using the Spanish version o

The ethics of disclosing genetic diagnosis for Alzheimer's disease: do we need a new paradigm?

INTRODUCTION OR BACKGROUND: Genetic testing for rare Mendelian disorders represents the dominant ethical paradigm in clinical and professional practice. Predictive testing for Huntington's disease is the model against which other kinds of genetic testing are evaluated, including testing for Alzheimer's disease. SOURCES OF DATA: This paper retraces the historical development of ethical reasoning in relation to predictive genetic testing and reviews a range of ethical, sociological and psychological literature from the 1970s to the present. AREAS OF AGREEMENT: In the past, ethical reasoning ha

[A Practical Diagnostic Approach to Patients Presenting with "Dizziness": Evaluation and Differential Diagnosis Based on the ATTEST/TiTrATE Framework].

Dizziness is one of the most common complaints encountered in outpatient clinical practice, and is associated with a wide range of underlying conditions, ranging from peripheral vestibular disorders to cerebrovascular disease, cardiovascular abnormalities, and psychological factors. Traditional classifications based on the quality of dizziness symptoms have inherent limitations in diagnostic accuracy. In contrast, the ATTEST approach, which structures clinical reasoning according to temporal profiles and triggering factors, has been shown to be useful. In this article, we outline the practical…

Ontology-Aligned Representation of Adverse Events in a Multiple Myeloma Clinical Trial.

To explore the potential of ontologies in improving adverse event identification, we developed ontology-aligned representations of adverse events extracted from electronic health records (EHRs) of fifty patients participating in a multiple myeloma trial. We reused relevant biomedical ontologies that are built upon Basic Formal Ontology (BFO) and constructed representations in Resource Description Framework/Web Ontology Language (RDF/OWL). Adverse events for the selected cohort were extracted from both structured and unstructured EHR data stored in the Arkansas Clinical Data Repository (AR-CDR)

Acute Adrenal Hemorrhage as the Initial Manifestation of Metastatic Lung Adenocarcinoma: A Cautionary Tale.

Adrenal gland metastatic disease is the most commonly occurring malignancy of the adrenal glands. Although metastatic disease is common, adrenal hemorrhage is a rare but potentially fatal manifestation of malignancy. The objectives of this case report are to highlight the unusual presentation of metastatic lung adenocarcinoma as spontaneous adrenal hemorrhage in a 64-year-old female who was otherwise asymptomatic. As well as to support the reasoning that metastatic disease should be considered as a differential in patients with this clinical presentation as this may have altered this fatal out

Multicentric Castleman disease, Kaposi sarcoma, hemophagocytic syndrome, and a novel HHV8-lymphoproliferative disorder.

The case of a 45-year-old man with HIV disease with bulky lymphadenopathy, fevers, and weight loss is presented. Immune reconstitution inflammatory syndrome was initially believed to be the cause, but the patient was found to suffer from Kaposi sarcoma, multicentric Castleman disease, hemophagocytic syndrome, and a newly described lymphoproliferative disorder. The diagnostic reasoning related to the causes of the fulminant illness is discussed as well as the roles of Epstein-Barr virus, human herpesvirus 8, and hepatitis C virus in the patient's clinical presentation and the therapeutic choice
