The term cloacal malformation is commonly used to describe the classic cloacal malformation where there is a single common urogenital and intestinal channel located at the expected site of the urethra. There is, however, a spectrum of cloacal abnormalities that differ from this classic type and are less well discussed in the radiologic and surgical literature. The aim of this pictorial essay is to familiarize radiologists with the anatomy, appropriate terminology and key prenatal imaging findings that differentiate the six entities that constitute the spectrum of cloacal abnormalities.

Female omphalopagus conjoined twins were successfully delivered vaginally and required emergency surgical separation shortly after birth for gastroschisis. Shared tissue included conjoined bowel; one twin also had a complex cloacal abnormality and patent urachus. A 2-year follow-up is presented. A review of the relevant literature confirms that this is the first example of gastroschisis conjoining omphalopagus twins.

Most researchers believe that hypospadias arises from malformation of the penile urethra. However, this concept has been recently rejected, and it has been suggested that the opening of the urethra is "pushed forward" by growth of the perineum. Pregnancy terminated for a severe oligoamnios and renal dysplasia. Chromosomal, gonadal, internal genitalia sexes are female. There is a cloacal dysgenesis, with caudal appendice and hypoplastic external genitalia of male type. Single umbilical artery and congenital cardiac malformation (complete atrio ventricular communication) are associated. Embryopathologic explanation for this female pseudo-hermaphroditism is proposed.

The kinetics of amiloride action on the isolated epithelium of the hen coprodaeum are reported. Tissues were taken from birds fed on low salt diets for 9-10 days, conditions which induce a high resting short circuit current due to sodium and sensitive to amiloride. The relation between the inhibition of amiloride sensitive short circuit current and blocker concentration obeyed simple mass laws with an apparent stoichiometry of 1:1 between amiloride and the sodium entry sites. The use of sugar for treating wounds which are difficult to heal and positive to resistant pathogens has already been documented. The authors describe the successful treatment by direct instillation of granular sugar in the antibiotic-resistant infected surgical site wound of a child with sarcoma. Among the 2,105 Enterobacteriaceae tested in a survey done in Portugal, 165 were nonsusceptible to carbapenems, from which 35 (26 Klebsiella pneumoniae, 3 Escherichia coli, 2 Enterobacter aerogenes, and 3 Enterobacter cloacae isolates and 1 Klebsiella oxytoca isolate) were confirmed to be carbapenemase producers by the presence of 30 Tn4401d-blaKPC-3, 4 intI3-blaGES-5, and one intI1-blaVIM-2 gene, alone or in combination with other bla genes. The dissemination of blaKPC-3 gene carried by an IncF plasmid suggests lateral gene transfer as a major mechanism of dissemination.

Mycotic pseudoaneurysm of the subclavian artery is uncommon and its therapeutic strategy has not been established. We report a case of 81-year-old woman with mycotic pseudoaneurysm in the right subclavian artery. Blood culture showed Enterobacter cloacae. Because of hemoptysis and acute expansion of the pseudoaneurysm, emergent coil embolization was performed, but failed. The patient underwent urgent operation for an en-bloc resection of the pseudoaneurysm after aorto-right common carotid artery bypass followed by omentum packing. We examined the effects of elevated temperatures and biocides on survivability of food isolates of Cronobacter spp. (C. sakazakii) and concomitant enterobacteriaceae obtained in microbiological control of infant nutrition products. A 3-yr-old, female captive golden lion tamarin (Leontopithecus rosalia) developed an abscess initially within the oral cavity that progressed to the cheek. Treatment with amoxicillin/clavulanic acid, enrofloxacin, and sulfamethoxizole/trimethoprim resulted in temporary resolution of the abscess. Complex cloacal malformations are associated with a wide spectrum of anatomic defects involving the gastrointestinal, urinary, and gynecologic tracts; the variety and complexity of these defects complicate surgical planning and repair. Rotational fluoroscopy with 3-D reconstruction provides precise anatomic detail, particularly regarding length of the common channel, appearance/location of the vagina(s) and bladder, which facilitates surgical planning and determination of prognosis.
