The endemic fungi Histoplasma capsulatum, Coccidioides immitis, and Blastomyces dermatitidis tend to reside in specific geographic regions. The organisms are pathogenic in that they are able to produce clinical disease in both immunocompromised patients and in patients with normal immunity. These organisms have a variety of clinical presentations, some of which typically are seen in the normal host and others that are primarily encountered in persons with abnormal immunity. Although most of the cases are seen in endemic regions, they may occur in persons who at some time either resided in or t…

The treatment of the major systemic fungi encountered in the United States--Histoplasma capsulatum, Blastomyces dermatitides, and Coccidioides immitis--depends on an understanding of the natural history of disease. Clinical manifestations of these infections can range from an asymptomatic disease that requires no therapy to an illness that is incurable and requires long-term suppression with antifungal agents to produce a balanced host-to-parasite relationship. This article discusses the natural history of disease caused by H. capsulatum, B. dermatitides, and C. immitis and various clinical si…

A 39-year-old man presented with chronic headaches and intermittent blurred vision with previous neuroimaging and blood work that was reportedly normal. He had papilledema and further questioning elicited a history of extensive alcohol use, unexplained weight loss, and night sweats. Magnetic resonance imaging of the brain demonstrated communicating hydrocephalus and leptomeningeal enhancement. The patient underwent ventriculoperitoneal shunt placement and leptomeningeal biopsy, which was initially unrevealing. Cerebrospinal fluid eventually yielded positive titers for coccidioides, a diagnosis…

There is an increasing pool of immunocompromised patients who are at an increased risk to fungi infections, which now cause 8% of nosocomial infections. Premature infants and elderly, transplant, and HIV patients are prime candidates for invasive fungal infections. The genitourinary system can be a source or target of disseminated fungal infection. Although candidal species are the most frequent pathogen, other species such as aspergila, cryptoccoccus have become major pathogens. "Environmental fungi," which include blastomyces, coccidioides and histoplasma, have become more aggressive in the …

Workers in Coccidioides-endemic areas performing soil-disturbing work or exposed to windy and dusty conditions are at increased risk for coccidioidomycosis. Four occupational coccidioidomycosis outbreaks from 2007 to 2014 in California are described, involving construction workers in a number of excavation projects and an outdoor filming event involving cast and crew. These outbreaks highlight the importance of identifying industries and occupations at high risk for coccidioidomycosis, conducting targeted occupational health surveillance to assess the burden of illness, developing and implemen…

Coccidioidomycosis is an infection caused by inhalation of the Coccidioides fungus. Most infections remain subclinical or are confined to the pulmonary system. Disseminated disease is rare. Traditionally, a combination of imaging modalities has been used to determine disease extent. We suggest (18)F-FDG PET/CT as a single first-line imaging examination to assess disease extent. We present a case of disseminated coccidioidomycosis to the lung, mediastinum, soft tissues, and skeletal system. To our knowledge, no prior case reports demonstrate such widespread disease using PET/CT.

We present a preliminary study utilizing an enzyme immunoassay for the detection of antibodies in blastomycosis using nitrocellulose membrane as the solid support. The comparative dot enzyme-linked immunosorbent assay (CDE) utilizing Blastomyces dermatitidis, Histoplasma capsulatum, and Coccidioides immitis antigens allows rapid screening of specimens and determination of a serum profile with respect to anti-Blastomyces antibodies. Sensitivity (76% or greater) and specificity (approximately 12% crossreactivity) determinations indicated that this visual assay may be an alternative antibody dete…

IN THE PRESENT STUDY, THE ADMINISTRATION OF AN INTERFERON STIMULATOR, POLYINOSINIC: polycytidylic acid (In:Cn), increased the severity of both experimental Candida albicans and Coccidioides immitis infections in mice, as evidenced by earlier time of death and higher final mortality. In addition, kidney tissue levels of C. albicans at 3 and 5 days after infection were found to be about 40- to 300-fold higher in In:Cn treated mice than in control mice. Experimental Cryptococcus neoformans infection, however, was not significantly affected by In:Cn treatment.

Coccidioidomycosis is an invasive fungal disease caused by Coccidioides species, posing a significant risk of disseminated infection. Vertebral involvement, though rare, can lead to instability, epidural abscess, and neurological deficits. In such cases, the integration of antifungal therapy and surgical intervention becomes critical, yet no standardized surgical protocol exists. A 27-year-old pregnant woman presented with four months of progressive axial neck pain, neck stiffness, a left-sided cervical mass, and neurological deterioration consistent with upper cervical myelopathy. Coccidioidomycosis is caused by the dimorphic fungi Coccidioides species which is endemic in the Western hemisphere. Reports on the characteristics of travel-related disseminated coccidioidomycosis in immunocompetent patients are rare, especially in non-endemic regions. The multifaceted symptoms of this condition present a diagnostic challenge to clinicians. This study aimed to review immunocompetent patients diagnosed with disseminated coccidioidomycosis in a tertiary hospital in Eastern China and other non-endemic areas, and to emphasize the importance of combining travel history with clinic…

BACKGROUND: In California, state prison inmates are employed to fight wildfires, which involves performing soil-disrupting work. Wildfires have become more common, including areas where Coccidioides, the soil-dwelling fungus that causes coccidioidomycosis, proliferates. However, work practices that place wildland firefighters at risk for coccidioidomycosis have not been investigated.
METHODS: On August 17, 2017, the California Department of Public Health was notified of a cluster of coccidioidomycosis cases among Wildfire A inmate wildland firefighters. We collected data through medical recor…

Coccidioidomycosis, a fungal infection caused by inhaling spores of Coccidioides immitis/posadasii, is endemic to the southwestern states of the United States, Northern Mexico and some parts of Central and South America. It is primarily a pulmonary infection with less than 0.5% of symptomatic cases showing dissemination. Skin, lymph nodes and bone are the commonest sites. Neurological involvement is rare and commonly presents as strokes, abscesses or meningoencephalitis. We present the case of a previously healthy 23-year-old African American male, presented with a four-month history of progre…

Valley fever is endemic to the southwestern United States. Humans contract this fungal disease by inhaling spores of Coccidioides spp. Changes in the environment can influence the abundance and dispersal of Coccidioides spp., causing fluctuations in valley fever incidence. We combined county-level case records from state health agencies to create a regional valley fever database for the southwestern United States, including Arizona, California, Nevada, New Mexico, and Utah. We used this data set to explore how environmental factors influenced the spatial pattern and temporal dynamics of valley…

The number of infections caused by the fungal pathogen Coccidioides is rising. In tandem, the number and breadth of researchers studying this important dimorphic fungal pathogen are growing as well. This review focuses on recent advances in the field and identifies areas primed for ongoing rapid growth, with the goal to improve treatments for coccidioidomycosis as soon as reasonably possible. These areas include building genetic tools, high-throughput drug screening, developing immunomodulatory therapies for coccidioidomycosis, and incorporating research methodologies that can be performed out…

Coccidioidomycosis, although endemic to southwest regions of United States, has sporadic cases which can be encountered outside of the endemic areas. Clinicians must be aware of usual and unusual presentations of this disease entity and should keep a high level of suspicion for the diagnosis. We discuss an uncommon presentation for pulmonary coccidioidomycosis in a patient with a history of HIV/AIDS, who presented with pneumothorax due to a ruptured coccidioidal cavity. This report also illustrates the high importance of travel history in investigating rare disease presentations.

Coccidioidomycosis is a systemic mycosis with a variable clinical presentation. Misdiagnosis of coccidioidomycosis as bacterial pneumopathy leads to inappropriate prescription of antibiotics and delayed diagnosis. This report describes an outbreak among armadillo hunters in northeastern Brazil in which an initial diagnosis of bacterial pneumonia was later confirmed as coccidioidomycosis caused by Coccidioides posadasii. Thus, this mycosis should be considered as an alternative diagnosis in patients reporting symptoms of pneumonia, even if these symptoms are only presented for a short period, w…
