Kenya is one of the British Army's main training areas outside of the UK and the flora and fauna present the physicians with several challenges. A 22 year old infantry soldier presented with a vesicular, linear painful rash across his chest caused by the chemical excreted from the Staphylinid or rove beetle, known locally as the Nairobi fly. Treatment included topical antibiotic and steroid creams to good effect. This report highlights the recognition, treatment and complications of the Nairobi Fly.

Footwear dermatitis is an important aspect of contemporary dermatology. The causative factors are constantly changing just as the footwear industry is continually changing. These range from the leather itself to rubber accelerators and from dyes to even metal trim.

Allergic contact dermatitis (ACD) is a T-cell mediated skin inflammation caused by repeated skin exposure to contact allergens. This review summarizes current knowledge on the immunology of ACD. Different phases in ACD are distinguished, i.e. sensitization, elicitation and resolution phases. We discuss contact allergen presentation and the central role of antigen presenting cells during sensitization phase. There is an extremely complex interaction of different kinds of immune cells, such as antigen presenting cells, T, B, NK lymphocytes, keratinocytes (KCs), endothelium, mast cells (MCs) and …

Retrospective studies demonstrate that the prevalence of skin sensitization does not significantly differ between atopic and non-atopic pati … aracterized patients are necessary to clarify the correlation between impaired skin barrier, atopic dermatitis and allergic contact dermatitis.

Seborrheic dermatitis is a chronic relapsing inflammatory skin condition characterized by scaling and poorly defined erythematous patches in areas rich in sebaceous glands. It is one of the most frequent skin disorders and may be socially embarrassing. Fungi of the genus Malassezia, lipid-dependent, ubiquitous skin residents, play a pathogenic role. Topical antifungal agents (e.g., ketoconazole) are the mainstay of treatment, and if used intermittently they can maintain remission. The vehicle itself may also play a relevant role. Improvements in diagnostic criteria, severity measures and outco…

The term 'immunotherapy' refers to treating diseases by inducing, enhancing or suppressing immune responses. As allergy is an excessive, detrimental immune reaction to otherwise harmless environmental substances, immunotherapy of allergic disease is aimed at the induction of tolerance toward sensitizing antigens. This article focuses on the historical developments, present state and future outlook for immunotherapy with haptens as a therapeutic modality for allergic contact dermatitis. Inspired by the effectiveness of immunotherapy in respiratory allergies, attempts were undertaken at curing a…

INTRODUCTION: The prevalence of atopic dermatitis (AD) is low in North Africa. We describe the epidemiology of this atopic condition among school children in Tunisia.
METHODS: We conducted a Cross-sectional survey study of 5 to 6-year-old schoolchildren from 21 primary schools of Sfax. The diagnosis of AD was based on the U.K. Working Party diagnostic criteria. A questionnaire including these criteria and some risk factors of AD was issued to the children. All children were examined by one dermatologist.
RESULTS: Among the 1617 examined children, ten had AD giving a one-year prevalence of 0.…

BACKGROUND: Because atopic dermatitis (AD) patients differ from non-AD patients in immunologic responses (they lack β-defensins, are more likely to react to protein allergens, and are harder to experimentally sensitize to the nonprotein allergens), patch-test responses to commercially available patch test series may differ in AD patients.
OBJECTIVE: To compare responses to conventional patch tests in AD and non-AD patients and to assess the importance of specific allergens in AD.
METHODS: AD patients were strictly defined by history of childhood flexural dermatitis. Patients with active dise…

Much progress has been made in understanding canine atopic dermatitis (AD), its triggering factors, and the best approach to its diagnosis and management. This article discussess the clinical features of AD, the pitfalls of allergy testing, and how recent research developments can influence the clinical management of this condition. Of particular interest is the accumulating evidence of skin barrier dysfunction, which should prompt clinicians to consider a shift from "reactive" management, classically focused on controlling and suppressing the inflammation, to a more "proactive" management of …

Seborrheic dermatitis is a recurrent, usually mild, skin disorder with typical clinical manifestations. As it most frequently involves exposed areas, such as the face and scalp, patients seek advice from a dermatologist in order to control their disease. This article will review the available treatments for this common dermatologic problem.

Vicks VapoRub (VVR) is a commonly used inhalant ointment that helps relieve symptoms of upper respiratory tract infections. It contains several plant substances, including turpentine oil, eucalyptus oil, and cedar leaf oil, which can potentially irritate or sensitize the skin, as well as camphor, menthol, nutmeg oil, and thymol. Although many reports describe allergic contact dermatitis (ACD) to the various constituents in VVR ointment, there are no cases of VVR directly causing ACD. We present a case of a patient who developed an ACD secondary to application of her VVR.

PURPOSE OF REVIEW: This review will focus on the diagnostic features of atopic keratoconjunctivitis (AKC), its relationship to atopic dermatitis, the immunopathogenesis, and therapy, and will include strategies used for the management of severe disease unresponsive to conventional therapy.
RECENT FINDINGS: Recent research has demonstrated the importance of various cytokines (IL-33), proteins (thymic stromal lymphopoetin) and effector cells (conjunctival epithelial cells, eosinophils and basophils) in the pathogenesis of chronic ocular inflammation. Current evidence supports the use of tacroli…

Cigarettes are widely known to contain potent carcinogens, and their smoke contributes to many chronic and potentially fatal diseases. Cigarettes may also represent an underreported and underrecognized cause of allergic contact dermatitis (ACD). Potential allergens from cigarettes can be found in the filters, paper, and tobacco. This article reviews the current literature on ACD from cigarettes to understand the clinical manifestation of ACD from cigarettes, to recognize components in cigarettes as potential sources of ACD, and to describe how to patch-test patients with suspected ACD from cig…

Dermatitis cruris pustulosa et atrophicans (DCPA) is a distinctive type of chronic superficial folliculitis, primarily affecting the lower limbs. It is characterized by symmetrical follicular pustules of both legs, with cutaneous edema, resulting in alopecia, atrophy and scarring. It was first described by Clarke, from West Nigeria, in 1952 and well illustrated in his book "Skin diseases in the African," under the initial label of "Nigerian shin disease." Subsequently, it was described in India as well, in 1964, and continues to be a problem in dermatology clinics across the country. It is pre…

Irritant and allergic contact dermatitis are common inflammatory skin diseases induced by repeated skin contact with low molecular weight chemicals, called xenobiotics or haptens. Although both diseases may have similar clinical presentations, they can be differentiated on pathophysiological grounds. Irritant contact dermatitis (ICD) is a non-specific inflammatory dermatitis brought about by activation of the innate immune system by the pro-inflammatory properties of chemicals. Allergic contact dermatitis (ACD) corresponds to a delayed-type hypersensitivity response with a skin inflammation me…
