[Recognition of unilateral auditory neuropathy in children].

Objective: This study aimed to compare the audiological characteristics between children with unilateral auditory neuropathy (UAN) and single-sided deafness (SSD) to establish a valid basis for the differential diagnosis of children with UAN. Methods: A retrospective analysis was conducted on audiological and imaging evaluations of children with UAN and SSD who were treated at Beijing Children's Hospital of Capital Medical University between May 2015 and June 2023. There were 17 children with UAN, comprising 10 males and 7 females, with an average age of 4.7 years. Additionally, there were 43

[Sensory and autonomic neuropathies and pain-related channelopathies].

Loss of pain perception can result from neurodevelopmental defects, degeneration of nociceptive fibers, or altered excitability of sensory neurons. Hereditary neurodegeneration leading to pain loss is classified as sensory and autonomic neuropathy (HSAN). Mutations in approximately 15 genes have been identified in the group of HSAN disorders. Hallmark of the disease is a liability to injury because of impaired acute pain as a warning system to prevent harm. The clinically overlapping "congenital insensitivity to pain (CIP)" is caused by mutations in voltage-gated sodium channels, which control

[Assessment of mesopic and contrast vision for driving licences: which cut-off values, which methods are appropriate?].

BACKGROUND: Hitherto recommendations and thresholds for contrast tests are available for mesopic but not for photopic methods. While mesopic tests are widespread in ophthalmology, in occupational medicine photopic contrast tests are often used. With regard to the attachment 6 of the German Fahrerlaubnisverordnung (FeV) which is relevant since July 2011 we tested the specificity and sensitivity as well as the test-retest reliability of available test devices and defined cut-off values. METHODS: We examined patients with medium opacities, healthy volunteers and a sample of employees. Optovist E

Influence of estradiol on micturition thresholds in the rat: involvement of the hypogastric nerve.

Studies have shown that the severity of bladder hyperreflexia induced by acute bladder inflammation varies with the ovarian cycle. These results suggest that the hyperreflexia is modulated by ovarian hormones. Other studies have suggested that such modulation involves the bladder's sympathetic innervation. These hypotheses were tested by assessing the development of bladder hyperreflexia in urethane-anesthetized rats subjected to different hormonal manipulations with or without bilateral hypogastric neurectomy (HYPX). The groups included sham ovariectomy (sham OVX), ovariectomy (OVX), OVX with

Human S-antigen determinant recognition in uveitis.

PURPOSE: Soluble antigen (S-Ag) is a member of the arrestin family of protein with which it shares a high level of homology. It is an immunologically privileged retinal antigen that can elicit experimental autoimmune uveitis (EAU) and is thought to be a target for ocular inflammatory diseases. This study was conducted to identify in humans, the immunogenic determinants of human S-Ag and to establish whether a specific response profile occurs in particular ocular inflammatory conditions. METHODS: Peripheral blood lymphocyte responses were measured against a panel of 40 overlapping synthetic pe

A study of skin responses to follow-up, rechallenge and combined effects of irritants using non-invasive measurements.

Irritant contact dermatitis is a common clinical problem. Primary irritation can be easily recognized, but cumulative irritation by daily exposure is hard to be diagnosed and the condition may fail to be clear even away from work. The mechanism of irritant dermatitis produced by repeated or combined exposure to clinical or subclinical doses of irritants is still poorly understood. In order to find out whether the subclinical doses of irritants affect each other by repeated or combined exposure according to their concentrations, non-invasive measurements, transepidermal water loss and laser Dop

Core temperature thresholds for hyperpnea during passive hyperthermia in humans.

Humans have higher ventilation when they are hyperthermic but it is not known whether core temperature thresholds for ventilation exist, nor has a physiological rationale been presented for this response. To examine this question, ventilation was studied in relation to core temperatures in humans rendered hyperthermic in a warm bath. Seven subjects [mean (SE), 23.3 (1.4) years] wearing only shorts and a thick felt hat with ear flaps were immersed to the neck in a bath at 41 (0.5) degrees C for 25 min. Tympanic (Tty), esophageal (Tes), thigh skin and forehead skin temperatures, heart rate, insp

Screening of vibration-induced disorders in the building industry using digital tactilometry. Results of a field study.

As occupational physicians in the building industry, we observed among these workers a high frequency of vibration exposure, during different tasks. We intended to study vibration exposure effects on vibration perception thresholds measured by digital tactilometry in this population of construction workers. A cross-sectional field study was made, 405 subjects were examined; each of them answered a questionnaire, underwent a medical examination and performed a test measuring his vibration perception thresholds, 150 subjects constituted the reference group. A close relationship between age and t

Non-invasive brain stimulation reveals reorganized cortical outputs in amputees.

EMG responses to non-invasive electromagnetic brain stimulation (EMS) were recorded from arm muscles of congenital amputees. Responses were obtained with lower thresholds on the amputated than on the intact side and were evoked from a larger cortical area. Contracting muscles showed increased responses to EMS; the increase was more pronounced on the amputated side. Similar findings were obtained in one traumatic amputee who suffered an early amputation, but not in another patient with a late amputation. We conclude that in congenital amputees there is substantial reorganisation of the corticos

[Distorted Freiburg Speech Test].

In 107 patients with different types of hearing loss (air conduction, sensorineural and combined) we ascertained the monaural discrimination loss of monosyllables with the help of the normal and distorted Freiburg speech test. It was evident that the quotient obtained from the measured discrimination is a very good and useful measure to characterise the ability to analyse the peripheral hearing system. Compared with normal hearing individuals and patients with air conduction hearing loss or combined loss of hearing, persons with perception hearing loss show a much greater and more divergent qu

Heart rate and respiratory rate in predicting risk of serious bacterial infection in febrile children given antipyretics: prospective observational study.

Clinical algorithms used in the assessment of febrile children in the Paediatric Emergency Departments are commonly based on threshold values for vital signs, which in children with fever are often outside the normal range. Our aim was to assess the diagnostic value of heart and respiratory rate for serious bacterial infection (SBI) in children after temperature lowering following administration of antipyretics. A prospective cohort of children presenting with fever between June 2014 and March 2015 at the Paediatric Emergency Department of a large teaching hospital in London, UK, was performed

Conditional Ablation of Glucocorticoid and Mineralocorticoid Receptors from Cochlear Supporting Cells Reveals Their Differential Roles for Hearing Sensitivity and Dynamics of Recovery from Noise-Induced Hearing Loss.

Endogenous glucocorticoids (GC) are known to modulate basic elements of cochlear physiology. These include both noise-induced injury and circadian rhythms. While GC signaling in the cochlea can directly influence auditory transduction via actions on hair cells and spiral ganglion neurons, evidence also indicates that GC signaling exerts effects via tissue homeostatic processes that can include effects on cochlear immunomodulation. GCs act at both the glucocorticoid receptor (GR) and the mineralocorticoid receptor (MR). Most cell types in the cochlea express both receptors sensitive to GCs. The

Subversion of effector CD8+ T cell differentiation in acute hepatitis C virus infection: the role of the virus.

In a companion study, we showed a dichotomy between the expansion of central memory (CCR7(+)) hepatitis C virus (HCV)-specific CTL and the incomplete memory effector differentiation in patients with acute HCV infection. Indeed, effector cells were unable to perform immediate functions, despite expressing the tissue-homing phenotype of effector memory cells (CCR7(-); semi-effectors). However, since they promptly differentiated into full-effectors upon IL-2 contact, we suggested that the inhibitory effect by environmental (possibly viral) factors on IL-2 production may have a pivotal role in gen

Distinct activation thresholds of unmyelinated C-fiber afferents by dorsal root ganglion and peripheral nerve stimulation.

OBJECTIVES: Sensitization of C-fiber nociceptors plays a critical role in spontaneous and ongoing pain in patients with chronic pain. We recently demonstrated that C-fiber afferents can be reversibly blocked through activity-dependent conduction slowing, suggesting that selective activation of C-fiber afferents may represent a novel strategy for pain relief. We hypothesized that electrical peripheral nerve stimulation (ePNS) and dorsal root ganglion (DRG) stimulation exhibit distinct activation thresholds for unmyelinated C-fiber afferents. MATERIALS AND METHODS: We characterized the activati

Deep sequencing reveals underestimated burden of integrase inhibitor resistance mutations in people living with HIV: a global individual patient data meta-analysis.

Integrase strand transfer inhibitors (INSTIs) have emerged as cornerstone agents in global antiretroviral therapy, though growing drug resistance presents clinical challenges. Conventional Sanger sequencing may underestimate the burden of low-frequency variants. This global meta-analysis systematically assesses the actual INSTI resistance burden among people living with HIV (PLWH) through deep sequencing technology. Our comprehensive search across Web of Science, PubMed, Cochrane Library, Embase, and Scopus (PROSPERO ID: CRD42023495535) identified studies utilizing deep sequencing for HIV inte

The role of quantitative tumor burden based on [68 Ga]Ga-DOTA-NOC PET/CT in well-differentiated neuroendocrine tumors: beyond prognosis.

PURPOSE: We aimed to elucidate the role of quantitative tumor burden based on PET/CT of somatostatin receptors in well-differentiated neuroendocrine tumors (NETs). METHODS: This study enrolled patients with [68 Ga]Ga-DOTA-NOC PET/CT-positive advanced NETs who did not receive medical treatment prior to PET/CT. Tumor burden was calculated using methods based on the background threshold and relative fixed threshold values (30%, 40%, and 50%). The prognostic value of the measured tumor burden in reference to overall survival (OS) and progression-free survival (PFS) on treatment with octreotide lo

A whole-joint, unidimensional, irreversible, and fine-grained MRI knee osteoarthritis severity score, based on cartilage, osteophytes and meniscus (OA-COM).

OBJECTIVE: To develop a whole-joint, unidimensional, irreversible, and fine-grained MRI knee osteoarthritis (OA) severity score, based on cartilage, osteophytes and meniscus (OA-COM), and to predict progression across different severity states using OA-COM as outcome and clinical variables as predictors. METHODS: Population-based knee pain cohort aged 40-79 was assessed at baseline and 7-year follow-up. OA-COM score was defined as the sum of MRI scores for cartilage, osteophytes and menisci, measured at 6, 8 and 6 sites, total score 0-54. To anchor severity levels, we fit cross-sectional logi
