[Bisphosphonates in pharmacotherapy of bone diseases].

Bisphosphonates represent a new class of drugs that has been developed in the past three decades for the treatment of various bone diseases and calcium metabolism disorders. Possessing high affinity to the bone-forming minerals, these substances can be used as inhibitors of the ectopic calcification and bone resorption. Bisphosphonates not only prevent from the loss of bones in the case of osteoporosis of various types (e.g., after the menopause), but provide for an increase in the bone mineral density as well. Therefore, these drugs offer an important additional means of therapy in cases of o

Effects of tetracyclines on bone metabolism.

The anti-resorptive properties of tetracyclines (TCs) and their non-antimicrobial, chemically modified analogues (CMTs) have enormous therapeutic potential in medicine and dentistry. Osseous destructive diseases associated with excessive mammalian collagenase (matrix metalloproteinase) activity and collagen breakdown include malignancy, arthritis, and periodontitis. However, apart from the significant antimatrix metalloproteinase effects of TCs, TCs/CMTs are also potent inhibitors of osteoclast function (i.e., anti-resorptive). Thus, TCs can affect several parameters of osteoclast function and

Disseminated coccidioidomycosis associated with hypercalcemia.

This article describes a patient in whom disseminated infection with coccidioidomycosis was associated with hypercalcemia. The patient had a low level of 1,25-dihydroxyvitamin D and a suppressed parathyroid hormone value, an indication that the hypercalcemia was not mediated by vitamin D or parathyroid hormone. The episode resolved readily with administration of pamidronate, an outcome suggesting that this is effective treatment of hypercalcemia of this origin. On follow-up of the patient while he was receiving antifungal therapy for Coccidioides immitis, calcium values remained normal.

[Preventive drug therapy of postmenopausal osteoporosis].

Osteoporosis, one of the most important disorders associated with aging, needs active prevention. Estrogen therapy prevents the early phase of bone loss and decreases the incidence of subsequent osteoporosis related fractures. All estrogens are probably not equally effective against post menopausal bone loss. In USA conjugated equine estrogens are the most used. In Europe estrogen therapy is usually applied with 17 beta estradiol with different ways of administration. If estrogen therapy cannot be used, there are limited datas about the use of bisphosphonates or calcitonins in the prevention o

The role of bone scanning in osteomalacia.

The presence of eight "metabolic features" was assessed on the bone scintigrams of ten patients with osteomalacia. In all of these bone images, sufficient features were present to strongly suggest a metabolic disorder. There scintiphotos were included in a controlled blind study using 30 normal bone scans and 20 scans of metastatic disease. Nine of the ten metabolic bone images were correctly identified by two independent observers. Skeletal uptake of radiotracer, expressed as bone-to-soft tissue ratio, was significantly higher in the osteomalacic patients than in a group of 80 controls.

Bisphosphonate-induced Esophagitis Dissecans Superficialis: A Rare Case Report.

Esophagitis dissecans superficialis (EDS), also referred to as "sloughing esophagitis," is an uncommon benign condition characterized by the desquamation of the superficial layer of the esophageal squamous mucosa. It is often underdiagnosed due to its rarity and nonspecific clinical presentation. Although several factors such as caustic ingestion, thermal injury, autoimmune disorders, and mechanical trauma have been implicated, medication-induced mucosal injury remains one of the most significant causes. Among these, bisphosphonates - a class of drugs widely prescribed for osteoporosis and oth

Myeloma bone disease: A constant problem in the changing landscape of myeloma management.

Outcomes for patients with multiple myeloma have improved markedly in recent years due to the introduction of highly effective immune-mediated anti-myeloma therapies in both newly diagnosed and relapsed patients. Conversely, while patients are living longer, myeloma bone disease continues to contribute significantly to morbidity and mortality. Routine incorporation of anti-resorptive therapies into patient management is recommended by consensus guidelines; however, patients continue to sustain skeletal-related events, including pathological fractures. In this review, we discuss the diagnosis a

Efficacy and Safety of Bisphosphonates in Chronic Non-bacterial Osteomyelitis of the Mandible: A Systematic Review.

Chronic non-bacterial osteomyelitis (CNO) of the mandible, often called diffuse sclerosing osteomyelitis (DSO) in Maxillofacial and Dental literature, is a rare condition characterized by sterile osteomyelitis affecting the mandible. This condition is part of the chronic recurrent multifocal osteomyelitis (CRMO)/synovitis, acne, pustulosis, hyperostosis, and osteomyelitis (SAPHO) spectrum. However, because mandibular involvement may present as unifocal disease, it deserves special attention. Unfortunately, the rarity of this disorder, along with a general lack of awareness, has led to numerous

Expert Perspective: How, When, and Why to Potentially Stop Antiresorptive Drugs in Osteoporosis.

Osteoporosis is a chronic disease, and antiresorptive treatments are often continued for many years. Despite their established efficacy in reducing fracture risk, the most commonly used antiresorptive treatments, bisphosphonates and denosumab, have short- and long-term risks that, coupled with their benefits and other unique characteristics, influence consideration for at least periodic discontinuation. Bisphosphonates retain their effects even after discontinuation due to their prolonged skeletal retention, whereas denosumab discontinuation is associated with a rapid rebound in bone turnover

Therapeutic strategy for atypical ulnar fracture in long use of bisphosphonate: A systematic review.

BACKGROUND: Atypical fractures are caused by the combined effects of severe suppression of bone metabolism (SSBT) due to long-term bisphosphonate therapy and chronic repetitive bone microdamage. Atypical ulnar fractures (AUFs) due to SSBT are rare, and there is no standard treatment strategy for such fractures. The relevant literature was reviewed, and the treatment strategy for AUF is discussed. METHODS: A systematic review was conducted. All studies on ulnar fractures in individuals with a history of bisphosphonate use were included, and the data were extracted and analyzed from the perspec

Oral Bisphosphonates Are Associated With Increased Risk of Severe Acute Kidney Injury in Elderly Patients With Complex Health Needs: A Self-Controlled Case Series in the United Kingdom.

Although oral bisphosphonates (BP) are commonly used, there is conflicting evidence for their safety in the elderly. Safety concerns might trump BP use in older patients with complex health needs. Our study evaluated the safety of BP, focusing on severe acute kidney injury (AKI), gastrointestinal ulcer (GI ulcer), osteonecrosis of the jaw (ONJ), and femur fractures. We used UK primary care data (Clinical Practice Research Datalink [CPRD GOLD]), linked to hospital (Hospital Episode Statistics [HES] inpatient) and ONS mortality data. We included all patients aged >65 with complex health needs an
