AIM: The aim of this study is to highlight our experience about the use of open Abdomen's technique as strategy for the management of complicated colon diverticulitis with a delayed anastomosis or colostomy.
MATERIALS AND METHODS: Thirty patients, with III and IV Hinchey stage, have been undertaken to a surgical procedure with Open Abdomen technique and application of Ab-thera device. A second surgical look was made after 48-72 hours in order to evaluate the possibility to do an anastomosis or colostomy.
RESULTS: No deaths in patients with anastomosis were reported, but one case of leakage a

HISTORY AND ADMISSION FINDINGS: Within a few weeks a 78-year-old woman presented three times in hospital with dramatic blood loss in need of transfusion. At her admission to hospital she complained about general weakness and painless melena. Within the last few years the patient had been treated with aspirin and phenprocoumon for chronic atrial fibrillation and coronary heart disease.
INVESTIGATIONS: Laboratory findings revealed a very low hemoglobin of 6 g/dl when the patient presented at hospital. The esophagogastroduodenoscopy was normal. The first colonoscopy detected diverticula of the l

PURPOSE: This study aimed to evaluate the impact of surgery-associated variables on recurrence rates after sigmoid resection for diverticulitis.
METHODS: Patients who underwent elective sigmoid resection for uncomplicated diverticulitis between 1992 and 2000 at two tertiary referral centers were followed up for recurrent disease as the primary end point. Recurrence after surgery was defined as left lower quadrant pain, fever, and leukocytosis, with consistent CT and/or contrast enema findings on admission and after six weeks. A logistic regression of the following variables was undertaken: pa

BACKGROUND/AIMS: Non-adrenergic non-cholinergic inhibitory nerves are the most important nerves in the enteric nervous system of the human gut. Recently, it has been established that nitric oxide is released by stimulation of non-adrenergic non-cholinergic inhibitory nerves. Therefore, in order to evaluate the function of nitric oxide in the left-sided colon of patients with diverticular disease, we examined the enteric nerve responses in colonic tissues from patients with this disease, and also used the left-sided normal colon as a control.
METHODOLOGY: Colonic tissue specimens (the divertic

Colovesical fistula, a not infrequent complication of diverticulitis, can be difficult to demonstrate by commonly used diagnostic imaging modalities. We present four cases in which computed tomography was used to detect the presence of such a fistula. Computed tomography readily detected air within the bladder earlier and without equivocation when compared with other imaging techniques. It was useful also in the assessment of the extent and the degree of pericolonic inflammation, thus playing an important role in preoperative surgical planning and postoperative follow-up.

BACKGROUND: Percutaneous drainage can be a conservative option for abscess formation subsequent to acute inflammation of the sigmoid colon.
CASE REPORTS: Three patients, aged 36, 65 and 77 years, were hospitalized for abscesses in the peri-sigmoid region. All three were treated with echoguided percutaneous drainage. The infectious phenomena regressed rapidly allowing secondary left colectomy 6 to 8 dais later with immediate colorectal anastomosis.
DISCUSSION: Hartman's resection is indicated for perforated diverticules of the sigmoid colon with formation of pelvic abscess and must be followe

BACKGROUND: Diverticula of the gastrointestinal tract are common in older adults, most often affecting the colon. While typically asymptomatic, acute diverticulitis represents the most frequent complication and may lead to severe outcomes, including perforation, abscess formation, and diffuse peritonitis. Duodenal diverticula on the other hand is a rare clinical entity, and is frequently detected incidentally during proximal endoscopy. While most cases are asymptomatic, inflammation of periampullary duodenal diverticula may result in serious complications. Acute pancreatitis secondary to duode

Pylephlebitis is a rare complication of intra-abdominal and pelvic infections, occurring in the context of conditions such as acute appendicitis and diverticulitis, among others. It consists of septic thrombophlebitis of the portal vein and/or its tributary branches. Early diagnosis of this condition is crucial to prevent serious complications like sepsis and mesenteric ischemia. Contrast-enhanced abdominal CT scan is the test of choice for diagnosis and for evaluating potential complications, and the correct interpretation of radiological findings is essential to avoid unnecessary interventio

Minimally invasive surgery is gaining increased importance in colorectal surgery. Since 1993 we have performed 378 minimally invasive colorectal procedures (appendzcetomy excluded): right-sided (n = 70) and left-sided (n = 160) hemicolectomy, anterior rectal resections (n = 72), and colostomy (n = 12), Hartmann procedure (n = 17), take-down operation (n = 25), rectopexy (n = 21) and coloproctomucosectomy (n = 2). The indications were diverticulitis (n = 147), benign tumor (n = 40), malignant tumor (n = 135), rectal prolapse (n = 21), Crohn's disease (n = 8), intestinal diversion (n = 25), colo

Computed tomography (CT) of the colon is useful in assessment of patients with colonic disease because the entire bowel wall, extraluminal tissues, and adjacent solid organs can be directly visualized. Patients with advanced stage colorectal carcinoma can be reliably identified with CT. This information can assist in surgical planning for intraoperative irradiation and placement of a hepatic artery infusion catheter. CT is an important imaging modality in the postoperative evaluation for recurrent rectal cancer after abdominoperineal resection. CT-guided biopsy often can provide histologic pro

INTRODUCTION: Diverticular disease is a challenge in western countries. The occurrence of fistula complicating diverticulitis is uncommon. As a result, spontaneous and synchronous colo-vesical and colo-cutaneous is an even rarer situation.
CASE PRESENTATION: We report the case of a 68 years old patient with medical history of bilateral inguinal hernia surgery and diverticular disease. He was admitted for fecal fistula through to the left inguinal area and recurrent urinary tract infections evolving for 2 months. Clinical examination revealed fecaluria and colo-cutaneous fistula. Abdominal CT

During treatment for sigmoid colon diverticulitis, a 72-year-old man underwent abdominal computed tomography, which revealed a gastric tumor that was approximately 40mm in diameter. He then underwent a detailed examination after the remission of diverticulitis. The gastric mucosa was found to be unremarkable on gastrointestinal endoscopy. On ultrasonic endoscopy, however, a hypoechoic mass of approximately 40mm in diameter was detected in the fourth layer of the gastric fornix and was believed to be a gastric submucosal tumor(SMT). The tumor was laparoscopically resected, and histological anal

UNLABELLED: <p>Introduction: Fistulizing sigmoid diverticulitis, most commonly involving colovesical or colovaginal fistulas, is a rare but debilitating complication of chronic diverticular disease. Despite pathognomonic symptoms, diagnostic pathways are often inconsistent, contributing to delayed surgical treatment and increased patient burden. This study examines the preoperative diagnostic pathway of patients with fistulizing diverticulitis in a multicenter cohort in Germany.
METHODS: In this retrospective study, patients with colovesical, colovaginal, or combined fistulas due to chronic s

Segmental colitis associated with diverticular disease (SCAD) is a colonic inflammatory disorder with localized non-granulomatous inflammation at sigmoid colon, and associated with colonic diverticulosis. SCAD is an apparently uncommon disorder in Western. We experienced a rare case of SCAD in a 46-year-old woman who visited the hospital due to abdominal discomfort. Colonoscopic examination showed multiple sigmoid diverticula in association with a segment length colitis. Colonoscopic biopsies of the sigmoid colon demonstrated cryptitis and crypt abscess along with chronic inflammatory cells in

PURPOSE: The spontaneous passage per rectum of a full-thickness colon "cast" is a rare consequence of acute colonic ischemia. Previous cases have undergone surgery soon afterward because of intractable symptoms. We report a patient who was managed conservatively for 11 months but ultimately required definitive surgery.
METHODS: The clinical, radiographic, pathologic, and endoscopic findings were obtained from the case notes and compared with previously reported cases.
RESULTS: A 67-year-old obese patient underwent a Hartmann's procedure for a perforated diverticular abscess, which was revers
