Mechanisms of Resistance to Oncogenic KRAS Inhibition in Pancreatic Cancer.

KRAS inhibitors demonstrate clinical efficacy in pancreatic ductal adenocarcinoma (PDAC); however, resistance is common. Among patients with KRASG12C-mutant PDAC treated with adagrasib or sotorasib, mutations in PIK3CA and KRAS, and amplifications of KRASG12C, MYC, MET, EGFR, and CDK6 emerged at acquired resistance. In PDAC cell lines and organoid models treated with the KRASG12D inhibitor MRTX1133, epithelial-to-mesenchymal transition and PI3K-AKT-mTOR signaling associate with resistance to therapy. MRTX1133 treatment of the KrasLSL-G12D/+; Trp53LSL-R172H/+; p48-Cre (KPC) mouse model yielded

Nanomedicine strategies for overcoming multidrug resistance in breast cancer: recent advances and future perspectives.

Multidrug resistance (MDR) remains a major challenge in the effective treatment of breast cancer, often leading to therapeutic failure, tumor recurrence, and poor clinical outcomes. Multiple mechanisms contribute to MDR, including overexpression of ATP-binding cassette (ABC) transporters, evasion of apoptosis, epithelial-mesenchymal transition (EMT), and the persistence of cancer stem cells (CSCs). In recent years, nanomedicine has emerged as a promising strategy to overcome these resistance pathways by enhancing drug delivery, improving intracellular drug accumulation, and enabling targeted a

Antimalarial efficacy monitoring after nearly 20 years of artemisinin-based combination therapy in Africa: recalibrating guidance.

Timely identification of resistance to antimalarials is necessary to ensure effective malaria case management. The backbone of antimalarial resistance surveillance strategies is the therapeutic efficacy study (TES). Originally designed to be easily deployable, in recent years, TES have become increasingly complex and slow to yield results. Concurrently, advances in molecular surveillance techniques, the emergence and spread of artemisinin partial resistance in Africa, and advances in antimalarial drug stewardship through multiple first-line therapies have changed the context in which TES are i

Combinations of antibiotics and nonantibiotic drugs enhance antimicrobial efficacy.

Combinations of antibiotics are commonly used in medicine to broaden antimicrobial spectrum and generate synergistic effects. Alternatively, combination of nonantibiotic drugs with antibiotics offers an opportunity to sample a previously untapped expanse of bioactive chemical space. We screened a collection of drugs to identify compounds that augment the activity of the antibiotic minocycline. Unexpected synergistic drug combinations exhibited in vitro and in vivo activity against bacterial pathogens, including multidrug-resistant isolates.

Insulin resistance and exendin-4 treatment for multiple system atrophy.

See Stayte and Vissel (doi:10.1093/awx064) for a scientific commentary on this article. Multiple system atrophy is a fatal sporadic adult-onset neurodegenerative disorder with no symptomatic or disease-modifying treatment available. The cytopathological hallmark of multiple system atrophy is the accumulation of α-synuclein aggregates in oligodendrocytes, forming glial cytoplasmic inclusions. Impaired insulin/insulin-like growth factor-1 signalling (IGF-1) and insulin resistance (i.e. decreased insulin/IGF-1) have been reported in other neurodegenerative disorders such as Alzheimer's disease. I

Nanoantibiotics: A Novel Rational Approach to Antibiotic Resistant Infections.

BACKGROUND: The main drawbacks for using conventional antimicrobial agents are the development of multiple drug resistance due to the use of high concentrations of antibiotics for extended periods. This vicious cycle often generates complications of persistent infections, and intolerable antibiotic toxicity. The problem is that while all new discovered antimicrobials are effective and promising, they remain as only short-term solutions to the overall challenge of drug-resistant bacteria. OBJECTIVE: Recently, nanoantibiotics (nAbts) have been of tremendous interest in overcoming the drug resis

Human Tuberculosis II. M. tuberculosis Mechanisms of Genetic and Phenotypic Resistance to Anti-Tuberculosis Drugs.

The great progress of knowledge of both M. tuberculosis physiology and how human host and bacilli interact has provided fertile ground for improving diagnosis and cure of TB infection. Once M. tuberculosis has infected humans, it elaborates strategies for evading the risk to killing by the cells of the host immune system and by the anti-tuberculosis (anti-TB) agents employed to cure infection. These strategies give rise to a bacterial multidrug resistance (MDR) status. This stems firstly from genetic mutations targeting a constellation of drug-processing mechanisms that still need full identif

Tackling Threats and Future Problems of Multidrug-Resistant Bacteria.

With the advent of the antibiotic era, the overuse and inappropriate consumption and application of antibiotics have driven the rapid emergence of multidrug-resistant pathogens. Antimicrobial resistance increases the morbidity, mortality, length of hospitalization and healthcare costs. Among Gram-positive bacteria, Staphylococcus aureus (MRSA) and multidrug-resistant (MDR) Mycobacterium tuberculosis, and among the Gram-negative bacteria, extended-spectrum beta-lactamase (ESBLs)-producing bacteria have become a major global healthcare problem in the 21st century. The pressure to use antibiotics

Malaria chemotherapy: recent advances in drug development.

Malaria is a disease caused by parasitic protozoa of the genus Plasmodium. Despite significant advances in understanding the disease and the parasite biology, malaria still remains one of the leading causes of morbidity and mortality, particularly in malaria-endemic regions of the world. The main factor hampering malaria control is the high degree of resistance developed by Plasmodium species against several classes of drugs. Artemisinin-based Combination Therapy (ACT) is the most rapidly acting antimalarial treatment effective against multi-drug resistant strains, and is, at present, the only

Monitoring and deterring drug-resistant malaria in the era of combination therapy.

As chloroquine and sulfadoxine-pyrimethamine (SP) are replaced by more effective artemisinin-based combination therapies (ACTs), strategies for monitoring (and, if possible, deterring) drug-resistant malaria must be updated and optimized. In vitro methods for measuring resistance will be critical for confirming and characterizing resistance to ACTs. Molecular markers are useful for tracking the emergence and dissemination of resistance and guiding treatment policy where resistance is low or moderate. Genomic approaches may help identify molecular markers for resistance to artemisinins and thei

Evolutionary dynamics of drug resistance in MDR-TB: Heterogeneous minor variants and extensive compensatory mutations.

Multidrug-resistant Mycobacterium tuberculosis complex (MTBC) remains a major global health challenge, yet the drug-specific evolutionary pathways through which resistance accumulates are not fully understood. Heterogeneous minor resistant variants (hSNPs) and compensatory mutations are increasingly recognized as important drivers of the emergence, stability, and transmission of drug-resistant strains. We analyzed 143 clinical MTBC isolates using whole-genome sequencing combined with phenotypic drug susceptibility testing to characterize lineage distribution, resistance-associated mutations, h

Targeting HIV-1 integrase: A review of options, challenges and future directions.

HIV-1 infects CD4+ T-cells, causing immunodeficiency and, if untreated, progression to AIDS. Global antiretroviral therapy scale-up has reduced deaths and new infections by nearly half over the past decade, yet lifelong treatment and emerging drug resistance remain critical challenges. This review provides an overview of the HIV-1 lifecycle, with emphasis on the structure, function, and multiple roles of IN in viral replication. We discuss the development, mechanisms of action, clinical utility, and resistance profiles of current integrase inhibitors, underscoring the need for innovative appro

Cannabinoids in hematologic malignancies: mechanisms and therapeutic potential for overcoming drug resistance.

Hematologic malignancies are highly heterogeneous diseases in which the emergence of therapy resistance remains a major obstacle to durable clinical benefit. Cannabinoids, acting through the endocannabinoid system, have drawn increasing interest because of their potential to modulate multiple resistance-associated pathways, yet the available evidence remains fragmented across disease entities, experimental models, and mechanistic frameworks. In this narrative review, we examine the molecular mechanisms by which cannabinoids may counteract drug resistance in leukemia, lymphoma, and multiple mye
