Laboratory Administration of Transcutaneous Auricular Vagus Nerve Stimulation (taVNS): Technique, Targeting, and Considerations.

Non-invasive vagus nerve stimulation (VNS) may be administered via a novel, emerging neuromodulatory technique known as transcutaneous auricular vagus nerve stimulation (taVNS). Unlike cervically-implanted VNS, taVNS is an inexpensive and non-surgical method used to modulate the vagus system. taVNS is appealing as it allows for rapid translation of basic VNS research and serves as a safe, inexpensive, and portable neurostimulation system for the future treatment of central and peripheral disease. The background and rationale for taVNS is described, along with electrical and parametric consider

Spinal cord stimulation for failed back surgery syndrome and other disorders.

Chronic pain is a complex condition that requires a multi-disciplinary approach to management. Spinal cord stimulation (SCS) has evolved into a relatively easily implemented, reversible technique with low morbidity for the management of chronic, intractable pain in selected patients. Percutaneous placement of electrode arrays, under local anaesthesia. supported by programmable, implanted electronics has been a major technical advance. Multicenter prospective studies were conducted and demonstrated that SCS. as a neuromodulation procedure, is indeed a superior method for treatment of chronic pa

Clinical applications of functional electrical stimulation.

Functional Electrical Stimulation (FES) allows the restoration of controlled muscle contractions, and hence limb function via computer control, in patients with irrecoverable upper motor neuron lesions. Wide experience has been recorded in spinal cord injured paraplegics and tetraplegics and to a lesser extent in cerebrovascular accident victims. The electrodes, either surface or implanted, stimulate muscles electrically through a stimulator activated by a control source which is in turn activated by the remaining functions of the user. Future advances in electrode technology and control and c

Laser tympanic neurectomy for post-cochlear implant pain: A new technique.

OBJECTIVES: To present the case histories and management by LASER tympanic neurectomy of two patients who presented with pain as their only symptom after cochlear implantation, avoiding the need for device removal. CLINICAL PRESENTATION: Two of our patients presented with otalgia, which appeared 6 months after cochlear implantation and resulted in their refusing to use the device. The pain was not controlled by repeated remapping or medical management. Neither patient showed evidence of infection or inflammation around their device. X-rays showed that there were no extracochlear electrodes or

Defibrillation shocks over epicardial patches produce sympathetic neural dysfunction in man.

INTRODUCTION: The purpose of this study was to determine the effect of direct current (DC) shocks on cardiac sympathetic innervation in humans using I-123-metaiodobenzylguanidine (MIBG) scintigraphy. Decreased efferent sympathetic neural function has been demonstrated following > 10-J DC shocks delivered through epicardial patch electrodes in dogs. To evaluate the effect of DC shocks on cardiac sympathetic innervation in humans, we performed MIBG scintigraphy in 11 patients (ages 46 to 75 years) prior to and after receiving shocks from an implantable cardioverter defibrillator (ICD). METHODS

Integration of High-Charge-Injection-Capacity Electrodes onto Polymer Softening Neural Interfaces.

Softening neural interfaces are implanted stiff to enable precise insertion, and they soften in physiological conditions to minimize modulus mismatch with tissue. In this work, a high-charge-injection-capacity iridium electrode fabrication process is detailed. For the first time, this process enables integration of iridium electrodes onto softening substrates using photolithography to define all features in the device. Importantly, no electroplated layers are utilized, leading to a highly scalable method for consistent device fabrication. The iridium electrode is metallically bonded to the gol

Status dystonicus is a distinct state characterized by pallidal beta-band activity.

Status dystonicus (SD) is a poorly known neurological emergency requiring urgent interventions, including deep brain stimulation (DBS) targeting the globus pallidus interna (GPi). The sensing capabilities of DBS electrodes provide an opportunity to study the pathophysiology of SD. Here, we study local field potentials (LFPs) from GPi-DBS electrodes implanted in a cohort of 10 children longitudinally during SD, recovery and relapse (recording range 11-1155 days). During SD, we report an increase in the periodic component of the power spectrum within the beta-band along with increases in burst a

Unraveling tactile categorization and decision-making in the subregions of supramarginal gyrus via direct cortical stimulation.

OBJECTIVE: This study aims to investigate the potential of direct cortical stimulation (DCS) to modulate tactile categorization and decision-making, as well as to identify the specific locations where these cognitive functions occur. METHODS: We analyzed behavioral changes in three epilepsy patients with implanted electrodes using electrocorticography (ECoG) and a vibrotactile discrimination task. DCS was applied to investigate its impact on tactile categorization and decision-making processes. We determined the precise location of the electrodes where each cognitive function was modulated.

Deep Brain Stimulation in Pathological Aggression.

BACKGROUND: We describe a case of deep brain stimulation in a patient with severe aggression refractory to conservative treatment. The targets of the stimulation were set bilaterally in the posteromedial hypothalamus and - in a second procedure due to its ineffectiveness - in the nucleus accumbens (NAcc) bilaterally. METHODS: In the first stage, we implanted electrodes into the posteromedial hypothalamus, after obtaining consent of the Bioethics Committee. In the early postoperative period, we observed significant improvement in the range of 50-90% in terms of quantity and quality of the atta

Ictal intracranial infraslow EEG activity.

OBJECTIVE: To investigate seizure onset baseline shifts and their relationship to background infraslow activity (ISA: 0.01-0.1Hz). METHODS: Archived intracranial EEG recordings of 61 seizures from 12 patients were evaluated. All had extensive coverage of several lobes with implanted grid, strip and/or depth electrodes prior to potential epilepsy surgery. The data had been obtained on a Nihon-Kohden system (acquisition range 0.01-1000Hz). Data analysis was carried out with the BESA software. RESULTS: When the high pass filter was set at 0.01Hz ictal baseline shifts, comparable to those report

Gastrointestinal myoelectric activity in a child with gastroschisis and ileal atresia.

Gastroschisis is frequently associated with intestinal atresia and alterations in gastrointestinal function. The authors studied gastric and small bowel myoelectric activity in a child who had a complex course and prolonged inability to tolerate oral intake after staged repair of gastroschisis and an associated ileal atresia. The child remained unable to tolerate oral intake after repair of the atresia and was reexplored 3 months later to rule out a partial small bowel obstruction, with simultaneous placement of serosal electrodes on the stomach and proximal small bowel. Persistent gastric dys

Micro- and nanotechnology for neural electrode-tissue interfaces.

Implantable neural electrodes can record and regulate neural activities with high spatial resolution of single-neuron and high time resolution of sub-millisecond, which are the most extensive window in neuroscience research. However, the mechanical mismatch between conventional stiff electrodes and soft neural tissue can lead to inflammatory responses and degradation of signals in chronic recordings. Although remarkable breakthroughs have been made in sensing and regulation of neural signals, the long-term stability and chronic inflammatory response of the neural electrode-tissue interfaces st

Additional lead improves defibrillation efficacy with an abdominal 'hot can' electrode system.

BACKGROUND: Although the left prepectoral site is preferred for "hot can" placement, this site is unavailable in some patients. We evaluated the influence of electrode location on defibrillation thresholds with alternative hot can and transvenous lead configurations. METHODS AND RESULTS: Three interrelated studies were performed. In group 1, the importance of hot can location was investigated by pairing a right ventricular lead to five different hot can placement sites in seven pigs. The defibrillation energies for right pectoral, left pectoral, left subaxillary, and right and left abdominal

Defibrillation electrode configurations developed from cardiac mapping that combine biphasic shocks with sequential timing.

Previous canine mapping studies of the transvenous defibrillation lead configuration of right ventricle (RV) to left R2 patch (P) revealed regions of low potential gradient in the left ventricular apex (A) and the right ventricular outflow tract (O). Thus 16 new lead configurations were tested in eight dogs, which incorporated electrodes in A and O to raise the gradient. When used in conjunction with two sequential biphasic shocks, the average defibrillation threshold energy from these configurations was 57% lower than that produced by a single biphasic shock delivered through RV-->P (phase 1
