Identifying opioid-related electronic health record phenotypes for emergency care research and surveillance: An expert consensus driven concept mapping process.

Background: Case identification for many areas of opioid research and surveillance in the emergency department (ED) is challenging as patients are often undifferentiated with nonspecific symptoms and diagnostic codes have proven to be inaccurate. Opioid-related phenotypes based on combinations of electronic health record data are a promising method to address this gap but lack a consensus-based conceptual framework to aid organization and prioritization. Methods: To achieve consensus around opioid-related phenotype topics in the ED, we used a hybrid scheme that employed modified Delphi and con

Enriching UMLS-Based Phenotyping of Rare Diseases Using Deep-Learning: Evaluation on Jeune Syndrome.

The wide adoption of Electronic Health Records (EHR) in hospitals provides unique opportunities for high throughput phenotyping of patients. The phenotype extraction from narrative reports can be performed by using either dictionary-based or data-driven methods. We developed a hybrid pipeline using deep learning to enrich the UMLS Metathesaurus for automatic detection of phenotypes from EHRs. The pipeline was evaluated on a French database of patients with a rare disease characterized by skeletal abnormalities, Jeune syndrome. The results showed a 2.5-fold improvement regarding the number of d

Quantifying Brain [18F]FDG Uptake Noninvasively by Combining Medical Health Records and Dynamic PET Imaging Data.

Full quantification of regional cerebral metabolic rate of glucose (rCMRglu) with [18F]fluorodeoxy-glucose ([18F]FDG) positron emission tomography (PET) imaging requires measurement of an arterial input function (AIF) curve, which is obtained with an invasive arterial blood sampling procedure during the scan. We previously proposed a non-invasive simultaneous estimation (nSIME) method that quantifies binding of a PET radioligand by combining individual electronic health records information and a pharmacokinetic AIF (PK-AIF) model. Initially applied only to [11C]DASB data, in this study we vali

Electronic Consultations in Allergy/Immunology.

BACKGROUND: Allergic condition management more often requires allergist guidance than allergy testing; necessary testing may be unavailable at initial drug allergy consultations. Electronic consultations (e-consults) provide expedited, problem-focused, potentially cost-saving care in other medical specialties, but have not yet been studied in Allergy/Immunology. OBJECTIVE: To describe e-consult use at an academic allergy/immunology practice. METHODS: E-consult data (August 10, 2016 through July 31, 2018) and in-person consult data (August 1, 2014 through July 31, 2018) were reviewed to deter

Between-days intra-rater reliability with a hand held myotonometer to quantify muscle tone in the acute stroke population.

A myotonometer can objectively quantify changes in muscle tone. The between-days intra-rater reliability in a ward setting for the acute stroke population remains unknown. This study aimed to investigate the device's between-days intra-rater reliability when used in a ward setting for acute stroke participants. Muscle tone of biceps brachii, brachioradialis, rectus femoris, and tibialis anterior was recorded in the ward at bedside by one physiotherapist on two consecutive days. This study included participants who were within 1 month of their first stroke occurrence. Participants who were medi

Medicalisation, smoking and e-cigarettes: evidence and implications.

There is debate in the tobacco control literature about the value of a medical model in reducing smoking-related harm. The variety of medical treatments for smoking cessation has increased, health professionals are encouraged to use them to assist smoking cessation and tobacco dependence is being described as a 'chronic disease'. Some critics suggest that the medicalisation of smoking undermines the tobacco industry's responsibility for the harms of smoking. Others worry that it will lead smokers to deny personal responsibility for cessation, create beliefs in 'magic bullets' for smoking cessa

Toward noninvasive quantification of brain radioligand binding by combining electronic health records and dynamic PET imaging data.

Quantitative analysis of positron emission tomography (PET) brain imaging data requires a metabolite-corrected arterial input function (AIF) for estimation of distribution volume and related outcome measures. Collecting arterial blood samples adds risk, cost, measurement error, and patient discomfort to PET studies. Minimally invasive AIF estimation is possible with simultaneous estimation (SIME), but at least one arterial blood sample is necessary. In this study, we describe a noninvasive SIME (nSIME) approach that utilizes a pharmacokinetic input function model and constraints derived from m

[Features of the electronic eikonometer for the study of binocular function].

After presenting the components of this electronic eikonometer (device schematic and organizational chart) for the analysis and measurement of perceptive effects of binocular disparity, we review the specifics (tests with incorporated magnifications seen in polarized light) and the advantages of this device as compared to existing eikonometers (absence of any intermediary optical system). We provide a list of available tests in the test library and their parametric characteristics: Ogle Spatial Test for Aniseikonia, Fixation Disparity Test: binocular nonius, and Linear and Random stereoscopic

Electron microscopy of chorionic villus samples for prenatal diagnosis of lysosomal storage disorders.

Some lysosomal storage disorders cause progressive prenatal accumulation of undegradable metabolites that manifest as membrane-bound vacuoles in endothelial cells, fibroblasts, and trophoblast, identifiable by electron microscopic examination of chorionic villus samples (CVS). There were 111 CVS, which had ultrastructural examination for suspected storage disorders at Great Ormond Street Hospital (1988-2005). There were 31 positive diagnoses, including glycogen storage disease type II, gangliosidosis type 1, mucopolysaccharidosis type 1, MPS not specified, Niemann-Pick type A, sialidosis/mucol

A comparison of selected phonatory behaviors of healthy aged and young adults.

Selected phonatory behaviors of healthy aged and young men and women were compared. Inverse-filtered air flow, electroglottograph (EGG), and intraoral air pressure signals were recorded as subjects phonated in each of four conditions: normal, soft, loud, loud/high pitched. Minimum flow offset, flow amplitude, air flow duty cycle, EGG duty cycle, estimated subglottal pressure, and fundamental frequency were derived from the recorded signals and compared among age/gender groups. Males had significantly greater flow amplitude than females regardless of age. Significant Age x Sex interactions were

An audio urine-glucose analyzer for blind diabetics.

At the instigation of one of the authors (J.M.)--a young man whose blindness is due to diabetic retinopathy--a urine-glucose analyzer was designed and constructed. Its purpose and function enable him to test his urine himself (free of dependence on thers for this service) and, thereby, he can then determine and administer his doses of insulin. He and other young blind diabetics may value the degree of freedom from dependence on others than the analyzer provides and feel it helps them to be able to work and care for themselves. A description of the instrument instructions for its use, and a sum

Nursing home-acquired pneumonia, dysphagia and associated diseases in nursing home residents: A retrospective, cross-sectional study.

BACKGROUND: Nursing home-acquired pneumonia (NHAP) is a common infection among nursing home residents. There is also a high prevalence of dysphagia in nursing home residents and they suffer more often from comorbidity and multimorbidity. This puts nursing home residents at higher risk of (mortality from) NHAP. Therefore it is important to gain more insight into the incidence of NHAP and the associated medical conditions in nursing home residents with dysphagia. OBJECTIVE: To investigate possible associations between NHAP and dysphagia in nursing home residents and to search for a medical risk

Using ICD Codes Alone May Misclassify Overdoses Among Perinatal People.

INTRODUCTION: As perinatal drug overdoses continue to rise, reliable approaches are needed to monitor overdose trends during pregnancy and postpartum. This analysis aimed to determine the sensitivity, specificity, positive predictive value, and negative predictive value of ICD-9/10-CM codes for drug overdose events among people in the MATernaL and Infant clinical NetworK (MAT-LINK) with medication for opioid use disorder during pregnancy. METHODS: People included in this analysis had electronic health record documentation of medication for opioid use disorder and a known pregnancy outcome fro

Deep learning-based segmentation, grading and genotype classification of glioma: a literature analysis.

BACKGROUND AND OBJECTIVE: Glioma represents the most prevalent and lethal primary brain tumor in adults, and its high mortality poses a significant challenge to clinical decision-making. While the conventional gold standard for diagnosis relies on the examination of surgically resected tumor tissue, recent advances in artificial intelligence, particularly deep learning, have demonstrated compelling performance in medical image analysis, offering the potential for non-invasive prediction of the genetic and histological features of glioma as well as automatic tumor segmentation. This review aims

Allergy alerting and overrides for opioid analogues across two health systems.

OBJECTIVES: Despite opioids comprising a majority of drug allergy alert (DAA) overrides, few studies have designed recommendations for clinical decision support (CDS) systems. We determined the frequency of opioid analogue DAA overrides and assessed DAAs overridden by patient and provider type, the most common allergic reactions documented in electronic health records (EHR), reaction severity, and associated hypersensitivity. METHODS: We conducted an observational cross-sectional study of DAAs in two geographically remote health systems. Patients were included if they were 18 years or older a
