Transcatheter arterial embolotherapy: a therapeutic alternative in obstetrics and gynecologic emergencies.

Transcatheter arterial embolization has become a major treatment modality in a variety of clinical applications, including management of bleeding related to a broad spectrum of obstetric and gynecologic disorders. Embolotherapy has a well-documented role in the management of pelvic and genital tract hemorrhage in the postpartum and postoperative/postcesarean setting. It is also an integral part in the treatment armamentarium of abdominal and cervical ectopic pregnancy, arteriovenous malformation, and gynecologic neoplasms, including more recently, uterine leiomyomata. Based on experiences accu

Thoracic aorta emergencies: is the endovascular treatment the new gold standard?

BACKGROUND: The endovascular treatment for acute traumatic aorta rupture has been proven to be safe in emergency. Technical aspects concerning the device fixation, the materials' durability, and the patients' selection criteria are still under discussion. We present our clinical experience and the mid-term follow-up. METHODS: Between January 2001 and January 2005, twenty-five patients have been diagnosed with acute traumatic aorta wall rupture in our emergency department. An angio computed tomography scan (CT-scan) was performed to confirm the diagnosis and to localize the aortic lesions. Eig

Perianesthesia care following obstetric emergencies at risk for multisystem organ dysfunction.

Perianesthesia care provided to obstetric patients is on the rise due to current obstetric practice habits, changes in the maternal population, and the increased desire for scheduled childbirth. Both scheduled and emergent cesarean deliveries create risk, yet the use of general anesthesia increases maternal morbidity and mortality significantly. Obstetric emergencies make up the majority of emergent cesarean deliveries. Detrimental events during pregnancy and childbirth may be categorized into hemorrhagic, septic, or anaphylactic shock. Excessive loss of circulating volume with subsequent loss…

Esophageal emergencies: things that will wake you from a sound sleep.

Esophageal emergencies are a common problem facing practicing gastroenterologists and it is important to know what therapies are indicated for different situations. Patients ingesting caustic agents should be monitored intensively for signs of perforation and ultimately for signs of stricture development. Foreign bodies impacted in the esophagus should be removed promptly to prevent perforation. Although esophageal perforations are generally managed surgically, conservative management of localized perforations has become more common especially with improved antibiotics and the use of nonsurgic…

Non-traumatic abdominal emergencies: imaging and intervention in acute biliary conditions.

Imaging is the standard method for the evaluation of emergency bile ducts and gallbladder diseases. Imaging may help to treat the patient also. In acute cholecystitis, association of clinical and sonographic data is accurate for the diagnosis, even when the patient is examined by a junior radiologist. Computed tomography may be required for those patients with unusual presentation such as emphysematous cholecystitis, perforation, or abscess. Acalculous cholecystitis is a challenging problem. It sometimes requires percutaneous cholecystostomy for diagnosis or treatment purposes. In patients wit

Recognition and treatment of endocrine/metabolic emergencies in children: part I.

A wide variety of endocrine/metabolic emergencies exists that may be isolated occurrences, the initial manifestation of an endocrine/metabolic disorder (eg, diabetes mellitus or hypoparathyroidism), or an acute abnormality in a child with known endocrine/metabolic disease as a result of intercurrent illness, emotional stress, or noncompliance with medications. The pediatric emergency medicine provider is faced with a difficult task when evaluating a child with a suspected endocrine or metabolic disorder, especially the child with no known underlying condition, since the signs and symptoms of s…

Anthropometric assessment of nutritional status in adolescent populations in humanitarian emergencies.

OBJECTIVE: To outline the difficulties and suggest potential solutions in anthropometric assessment of adolescents during humanitarian emergencies. DESIGN: Literature review. SETTING: Multiple settings in which the nutritional status of adolescents has been assessed using anthropometric measurements. SUBJECTS: Adolescents in multiple populations. INTERVENTIONS: None. RESULTS: The use of anthropometry may be more difficult in adolescents than in other age groups because anthropometric indices in normally nourished adolescents change with age and sexual development. Moreover, survey and ref…

Prolonged thiopentone infusion for neurosurgical emergencies: usefulness of therapeutic drug monitoring.

Serial serum thiopentone concentrations were measured during and following completion of an intravenous infusion of thiopentone in 20 patients with neurosurgical emergencies. The concentration data from a further 55 patients who had had some such measurements were reviewed retrospectively. The patients received an infusion for longer than 24 hours at a rate adjusted to maintain EEG burst suppression. The data were interpreted in terms of thiopentone pharmacokinetics and used to produce statistical models relating to clinical outcomes. In these patients, the one-month mortality rate following c…

[Asthma emergencies: can we lower the rate of readmission after discharge?].

INTRODUCTION: The rate of readmission among asthmatic emergency patients varies. In 1991 we observed a 9% rate of readmission following emergency room release. Studies of the number of readmissions or request for medical care are used as the basis for recommendations for releasing patients from hospital emergency care. No studies have assessed disease stability following release or factors related to stability. OBJECTIVES: To assess the course of disease and clinical stability of patients in the period immediately following release from emergency room care. To determine factors that might pre

Gastrointestinal emergencies in patients with acute intestinal graft-versus-host disease.

Acute intestinal graft-versus-host disease (GVHD) develops in about 30-50% of allogeneic bone-marrow transplant recipients: 10-20% have gastrointestinal emergencies (hemorrhage or perforation). Mortality reaches 30-60% in patients with acute, grade 2-4 GVHD. We studied 36 bone marrow recipients in whom acute intestinal GVHD developed. Seven had gastrointestinal emergencies: 4 severe gastrointestinal bleeding and 3 acute peritonitis. Three patients with gastrointestinal bleeding and one patient with peritonitis responded to medical therapy. Three needed surgery: one with bleeding and two with p…

Comparison of fentanyl/midazolam with ketamine/midazolam for pediatric orthopedic emergencies.

OBJECTIVE: Emergency management of pediatric fractures and dislocations requires effective analgesia, yet children's pain is often undertreated. We compared the safety and efficacy of fentanyl- versus ketamine- based protocols. METHODOLOGY: Patients 5 to 15 years of age needing emergency fracture or joint reduction (FR) were randomized to receive intravenous midazolam plus either fentanyl (F/M) or ketamine (K/M). Measures of efficacy were observational distress scores and self- and parental-report. Measures of safety were frequency of abnormalities in and need for support of cardiopulmonary f

Urologic emergencies. Conditions affecting the kidney, ureter, bladder, prostate, and urethra.

Prompt, thorough evaluation is needed when patients present with symptoms of renal colic, acute urinary retention, prostatitis, pyelonephritis, and other urologic emergencies. Primary care physicians have an important role in initial workup and treatment. Once the diagnosis is determined, urologic consultation may be necessary for definitive treatment.

[ENT emergencies in a tertiary hospital. Study of frequency and etiology].

We have reviewed 19,061 emergencies attended in the ENT Department of our Hospital, over a period of 3 years. And analyzed the aetiologic distribution and the frequency of the cases, finding out the most frequent pathology were the external otitis followed by epistaxis. In the paper are also considered their relation with other parameters such as sex (no differences), age (25-35 years) and seriousness, this latter not being of gravity. Finally, we've studied the amount and degree of appropriteness of the visits attended and found out that near half of the cases could be attended in a less spec…

Hematologic emergencies. Management of hyperleukocytic syndrome, DIC, and thrombotic thrombocytopenic purpura.

Three hematologic emergencies are reviewed in this article: the hyperleukocytic syndrome, disseminated intravascular coagulopathy (DIC), and thrombotic thrombocytopenic purpura (TTP). The hyperleukocytic syndrome most commonly occurs in patients with acute myelogenous leukemia. It commonly affects the lungs and may cause intracranial hemorrhage. Long-term control is accomplished only by inducing a remission of the leukemia. Patients with DIC may have excessive clotting, uncontrolled bleeding, or both. Infections are the most common cause; cases in obstetric patients are usually due to abruptio…

Eosinophilic enteritis presenting as surgical emergencies: a report of six cases.

Eosinophilic enteritis is now commonly diagnosed in northern Queensland, and cases have been seen elsewhere. The patients present with acute abdominal symptoms, which may mimic appendicitis, and many cases are managed by surgery. To facilitate the diagnosis of this unusual syndrome, six surgically treated cases are reported here. Blood eosinophilia may be a helpful diagnostic feature, but in some cases it does not develop until after the acute abdominal episode. Most cases resolve satisfactorily with conservative management.

Parenteral chlorpromazine and frusemide: safe and effective treatment for hypertensive emergencies.

Hypertensive emergencies present a difficult problem of management. Although many treatment regimens have been described over the years, their application has presented problems of adverse effects and all have required detailed and intensive supervision of patients. After favourable results obtained in a preliminary study using a combination of parenteral chlorpromazine and frusemide, a 5-year prospective study was conducted using this treatment to produce rapid reduction in blood pressure in patients with acute onset severe hypertension (blood pressure greater than 225/130 mmHg). The patients…

[The role of computed axial tomography in pancreatic emergencies (author's transl)].

The role of CT in acute diseases of the pancreas is discussed, in the light of personal experience (25 cases). In acute pancreatitis (22 cases), CT was useful in particular for: differential diagnosis between the necrotic-hemorrhagic type and the edematous type; detection of extra-pancreatic collections and evaluation of their extent; post-operative follow-up of necrotic-hemorrhagic pancreatitis. In the other more rare empergencies (3 cases), CT could recognize respectively the rupture of an infected pseudo-cyst, and intra-cystic hemorrhage, a mediastinitis due to the spread of ascitic fluid c
