Design thinking during a health emergency: building a national data collection and reporting system.

BACKGROUND: Design thinking allows challenging problems to be redefined in order to identify alternative user-center strategies and solutions. To address the many challenges associated with collecting and reporting data during the 2014 Ebola outbreak in Guinea, Liberia and Sierra Leone, we used a design thinking approach to build the Global Ebola Laboratory Data collection and reporting system. MAIN TEXT: We used the five-stage Design Thinking model proposed by Hasso-Plattner Institute of Design at Stanford in Guinea, Liberia and Sierra Leone. This approach offers a flexible model which focus

Meeting report: Considerations for trial design and endpoints in licensing therapeutic HPV16/18 vaccines to prevent cervical cancer.

Cervical cancer is a major cause of morbidity and mortality globally with a disproportionate impact on women in low- and middle-income countries. In 2021, the World Health Organization (WHO) called for increased vaccination, screening, and treatment to eliminate cervical cancer. However, even with widespread rollout of human papillomavirus (HPV) prophylactic vaccines, millions of women who previously acquired HPV infections will remain at risk for progression to cancer for decades to come. The development and licensing of an affordable, accessible therapeutic HPV vaccine, designed to clear or

Preventing childhood obesity: what works?

Rates of overweight in North American children and adolescents have increased dramatically since the 1970s. Childhood obesity has reached epidemic proportions and calls for prevention and treatment programs to reverse this trend have been made. However, the evidence base needed for effective action is still incomplete, especially for childhood obesity prevention programs. This paper focuses on primary prevention of childhood obesity and has three aims: (1) to briefly describe current primary prevention approaches for childhood obesity and the evidence for their impact; (2) to elucidate promisi

Prenatal exposure to pesticides and the risk of male genital malformations: A scoping review.

INTRODUCTION: Hypospadias and undescended testis (UDT) are common genital malformations with multifactorial origins. Endocrine-disrupting chemicals, including pesticides, are suspected contributors, but prior reviews are outdated, and none has mapped evidence for both conditions together. OBJECTIVE: To map evidence on the association between prenatal pesticide exposure and hypospadias/UDT, with particular attention to underexplored exposures such as pyrethroids and household pesticides, and to identify research gaps. METHOD: Following PRISMA-ScR guidance, we searched PubMed, Web of Science,

No Causal Relationship Between Helicobacter Pylori Infection and Rosacea: A 2-Sample Mendelian Randomization Study.

BACKGROUND: Previous studies have shown that patients with rosacea have a higher prevalence of Helicobacter pylori (H. pylori) infection. However, whether H. pylori infection contributes to the development of rosacea remains unclear, and no genetic association studies between the two have been conducted to date. Genome-wide association studies (GWAS) database is a public resource that stores and shares data that aims to identify genetic links to complex diseases, physiological traits, or drug responses. Mendelian randomization (MR) is an epidemiological approach to investigate the effect of th

Variability of urinary metal in short-, mid-, long-term periods and its optimal sampling strategy: A novel epidemiological insight to exposure classification.

The health effects of metals are well-documented, but relying on single urine samples may inadequately reflect short-, mid-, or long-term exposure, leading to potential misclassification. Variability in urinary metal concentrations and its implications for exposure assessment across different timeframes and epidemiological study designs remain underexplored. Identifying optimal sampling strategies and minimum sample sizes is crucial for exposure assessment of enhancing environmental health research. In a two-year repeated-measures study of healthy adults across 4 visits (2019-2021), first morn

Understanding the interaction between cytomegalovirus and tuberculosis in children: The way forward.

Over 1 million children develop tuberculosis (TB) each year, with a quarter dying. Multiple factors impact the risk of a child being exposed to Mycobacterium tuberculosis (Mtb), the risk of progressing to TB disease, and the risk of dying. However, an emerging body of evidence suggests that coinfection with cytomegalovirus (CMV), a ubiquitous herpes virus, impacts the host response to Mtb, potentially influencing the probability of disease progression, type of TB disease, performance of TB diagnostics, and disease outcome. It is also likely that infection with Mtb impacts CMV pathogenesis. Our

An inflammatory biomarker as a differential predictor of outcome of depression treatment with escitalopram and nortriptyline.

OBJECTIVE: Major depressive disorder has been linked with inflammatory processes, but it is unclear whether individual differences in levels of inflammatory biomarkers could help match patients to treatments that are most likely to be beneficial. The authors tested the hypothesis that C-reactive protein (CRP), a commonly available marker of systemic inflammation, predicts differential response to escitalopram (a serotonin reuptake inhibitor) and nortriptyline (a norepinephrine reuptake inhibitor). METHOD: The hypothesis was tested in the Genome-Based Therapeutic Drugs for Depression (GENDEP)

Pediatric incidental brain tumors: a growing treatment dilemma.

OBJECT: Rising numbers of MRI studies performed during evaluations for pediatric disorders have contributed to a significant increase in the number of incidentally found brain tumors. Currently, there is very little literature on the nature of and the preferred treatment for these incidental brain tumors. In this paper the authors review their experience diagnosing and treating these lesions in children as well as the current literature on this topic. METHODS: Records from 2 centers were reviewed for incidentally found brain tumors, treatment approaches, and outcomes for both surgical and non

Fast food restaurants and food stores: longitudinal associations with diet in young to middle-aged adults: the CARDIA study.

BACKGROUND: A growing body of cross-sectional, small-sample research has led to policy strategies to reduce food deserts--neighborhoods with little or no access to healthy foods--by limiting fast food restaurants and small food stores and increasing access to supermarkets in low-income neighborhoods. METHODS: We used 15 years of longitudinal data from the Coronary Artery Risk Development in Young Adults (CARDIA) study, a cohort of US young adults (aged 18-30 years at baseline) (n = 5115), with linked time-varying geographic information system-derived food resource measures. Assessing prostate cancer risk: results from the Prostate Cancer Prevention Trial.

BACKGROUND: Prostate-specific antigen (PSA) testing is the primary method used to diagnose prostate cancer in the United States. Methods to integrate other risk factors associated with prostate cancer into individualized risk prediction are needed. We used prostate biopsy data from men who participated in the Prostate Cancer Prevention Trial (PCPT) to develop a predictive model of prostate cancer. METHODS: We included 5519 men from the placebo group of the PCPT who underwent prostate biopsy, had at least one PSA measurement and a digital rectal examination (DRE) performed during the year befo

Considerations for first field trials of low-threshold gene drive for malaria vector control.

Sustainable reductions in African malaria transmission require innovative tools for mosquito control. One proposal involves the use of low-threshold gene drive in Anopheles vector species, where a 'causal pathway' would be initiated by (i) the release of a gene drive system in target mosquito vector species, leading to (ii) its transmission to subsequent generations, (iii) its increase in frequency and spread in target mosquito populations, (iv) its simultaneous propagation of a linked genetic trait aimed at reducing vectorial capacity for Plasmodium, and (v) reduced vectorial capacity for par

The development of the Human Health Exposure Analysis Resource (HHEAR) Data Repository for environmental epidemiology research.

Implementation of the exposome paradigm is a critical aspect of the next generation of environmental health research studies. To spur exposomics research, the U.S.-based Human Health Exposure Analysis Resource (HHEAR) provided scientific investigators access to both laboratory and statistical analyses aimed at incorporating and expanding the breadth of biological markers of environmental exposures within their research. To extend the benefits of this program to the broader scientific community, the HHEAR Data Center established a public data repository to facilitate pooling and sharing of data
