[Invasive infection with Haemophilus influenzae type b in spite of complete vaccination].

Five patients, 4 boys and 1 girl aged 13-41 months, developed invasive Haemophilus influenzae type b (Hib) disease (2 epiglottitis, 3 meningitis) despite full (or at least 3 times) vaccination. At admission as well during convalescence, 3 out of 5 had IgG anti Hib antibody levels < or = 5 U/ml. Serum immunoglobulin levels, including IgG subclasses, as well as complement were normal in all cases. In 2 of the 3, booster vaccinations with Hib conjugate vaccine elicited adequate antibody titres. Since the incorporation of the conjugated Hib polysaccharide tetanus toxoid vaccine (HibTT) in the Nati…

Recognition and emergency management of infectious causes of upper airway obstruction in children.

Infections represent one of the most common causes of upper airway obstruction in the pediatric age group. Because obstructive processes may lead to respiratory failure and subsequent cardiopulmonary arrest, prompt recognition is crucial. This discussion provides a brief review of the unique anatomic and physiological characteristics of the child's airway, which may contribute to obstructive processes, followed by basic principles of pediatric airway management. Finally, we review many of the infectious causes of upper respiratory obstruction, with emphasis on early recognition and acute manag

Laryngospasm-induced pulmonary edema.

We report the case of a 6-month-old child who developed acute pulmonary edema because of laryngeal spasm during orthopedic manipulations for congenital hip dysplasia. Laryngospasm was probably secondary to an unsuspected light level of anesthesia, maintained via face mask. No other predisposing factors, such as enlarged adenoid tonsils, laryngitis, epiglottitis, mechanical stimulation of the larynx or aspiration of foreign material were identified. Serious oxygen desaturation and bradycardia ensued, during inefficient attempts at positive pressure ventilation. After emergency intubation withou…

Retropharyngeal abscesses in children: a 10-year review.

Retropharyngeal abscess is a rather rare, deep-neck infection of children and may seriously compromise the airway and mimic other diseases. A retrospective review of 17 cases of retropharyngeal abscess presenting to The Children's Hospital, Denver, from 1976 to 1986 was performed. Nine children (56%) had stridor or airway obstruction. Seven patients (41%) had perforations of their hypopharynx or esophagus, including two neonates (most likely associated with intubation attempts). Two patients presented in the emergency department with a tentative diagnosis of "epiglottitis," while another refer…

Transoral Laser-Assisted Infrahyoid Supraglottic Laryngectomy for Selected Patients With Supraglottic Cancer.

Supraglottic laryngectomy has evolved from open to transoral endoscopic approaches with advancements in surgical techniques and instruments such as lasers, endoscopes, ultrasonic devices, and robotics. Transoral laser-assisted microsurgery has emerged as an effective treatment option, offering faster functional recovery and serving as an alternative to partial laryngectomy or non-surgical therapies. Traditional endoscopic supraglottic laryngectomy involves resection of both suprahyoid and infrahyoid supraglottic structures. However, in cases where the tumor is limited to the infrahyoid epiglot

Microelectrodes and radiofrequency for transoral horizontal supraglottic laryngectomy in T3 tumors.

The aim of this manuscript is to show the surgical technique for horizontal supraglottic laryngectomy in T3 carcinomas by a transoral approach using microelectrodes and radiofrequency. From 2009 to 2020, 11 selected cases of T3 laryngeal supraglottic carcinomas invading the pre-epiglottic space, with preserved vocal cords mobility underwent surgery. The technique is described step by step. Average time duration of the larynx surgery was 95 min. Nine percent of the patients presented a profuse hemorrhage 7 days postoperative; in 73% of patients the nasogastric feeding tube was withdrew 10 days

Calcinosis Universalis: Cause for a Hard Airway.

Calcinosis universalis is a rare subtype of connective tissue diseases known as calcinosis cutis. The disease is described as diffuse calcium salt deposits in subcutaneous, fibrous muscle structures, and tendons. Calcinosis cutis typically occurs secondary to a tissue damaging disease such as dermatomyositis, systemic lupus erythematosus, and Sjogren syndrome, presenting before age 20 and predominantly affecting women. Calcinosis universalis presents challenges for anesthesia providers as joint calcification can make positioning difficult, vascular access becomes increasingly difficult over ti

Spindle cell lipoma of the larynx: A case report.

INTRODUCTION: Lipomas are rarely found in primary mesenchymal tumors of the hypopharynx and larynx. When they do appear, they often macroscopically resemble a retention cyst or laryngeal nodule. The laryngeal spindle cell lipoma, a variant, is extremely rare. PATIENT CONCERNS: A 65-year-old woman presented with a 3-month history of pharyngeal paraesthesia. Laryngoscopy revealed the presence of a well-encapsulated, smooth-surfaced, yellowish, pedicled mass on the left epiglottis. Magnetic resonance imaging confirmed the epiglottic mass. DIAGNOSIS: Following excision of the mass, the diagnosis

Forgotten denture in a hemiplegic patient.

A 74-yr-old man developed left hemiplegia because of a right middle cerebral artery territory infarction and also had clinical features of dysphagia and speech difficulty. At that time, he complained of neck pain, but the symptom was ignored because he had a nasogastric tube and had been diagnosed with a huge epiglottic cyst that had already shown several symptoms such as severe hoarseness and throat discomfort. A videofluoroscopic swallowing study was planned to find out the cause and the type of dysphagia. On the videofluoroscopic swallowing study, a foreign body was found at the hypopharynx

[Electrocautery snare efficacy in head and neck lesion treatment].

The electrocautery snare has been widely used in gastroenterology and to remove bronchial and urinary bladder lesions, but rarely in head and lesion electrocautery. Since October 2006, we have used this instrument to treat 17 head and neck granuloma, papilloma, cyst, and cancer lesions under local or general anesthesia. Lesions were localized in the larynx, oropharynx, hypopharynx, or tracheostoma. The snare was used through a rhinolaryngovideoscope in most cases with a current of 15-30 watts produced by an electrosurgical generator. All procedures were easy, quick and successful, with minimal

Comparison of the Levitan FPS Scope and the single-use bougie for simulated difficult intubation in anaesthetised patients.

A randomised cross-over study was performed in 34 patients with no evidence of airway difficulties, following induction of general anaesthesia, to compare the efficacy of the Levitan FPS scope (LFPS) and the single-use bougie for tracheal intubation during simulated grade IIIa laryngoscopy. Success rates for intratracheal placement of the device, and the time required for insertion and tracheal intubation were recorded. Both devices were equally successful (31/34 for the LFPS vs 29/34 for the bougie) for insertion into the glottis. The mean insertion time for the LFPS was statistically longer

[Video-fluoroscopy in a graduated diagnostic program in oral cavity and pharyngeal carcinoma].

The role of videofluoroscopy in the preoperative evaluation of 38 carcinomas of the oral cavity and oro-/hypopharynx is described. The analysis compared videofluoroscopy with CT and endoscopic assessment, since videofluoroscopy is the only imaging method that can record dynamic motility changes during deglutition. The intraluminal part of the tumour is well visualised by high contrast resolution of the pharyngeal mucosa. The extent of tumorous infiltration can be evaluated via indirect criteria such as leaking, vallecular asymmetry, impaired epiglottic movement with insufficient vestibular clo

Compensatory mechanisms induced by high oropharyngeal airway resistance in rats.

Acid base balance changes were observed during 72 h following bilateral nasal obstruction in rats. Mouth breathing caused acute respiratory acidosis and marked aerophagia, leading to spontaneous death of the experimental animal 80 to 90 h postoperatively. Stenotic oropharyngeal airway, due to palatal-epiglottic approximation, is supposed to be responsible for respiratory insufficiency in the nose obstructed rats. The compensatory changes in respiratory mechanics caused by high oropharyngeal airway resistance, together with some possible reflex changes, may have caused either air swallowing or
