Six patients are described who experienced a prodromal period suggesting an infective process, followed by a distinctive polymorphous cutaneous eruption. The most striking lesion, present in all of them and giving the eruption a distinctive herpetiform appearance, included closely grouped vesicles resting on an erythematous base. Histopathological examination of these lesions showed features of dermal and epidermal types of erythema multiforme but with prominent leukocytoclasis. A 77-year-old man presented with a cutaneous rash of 3 days' duration. Seven days before onset, the patient reported a bronchopulmonary infection treated with amoxicillin. Physical examination revealed multiforme cutaneous lesions, involving the armpits, pubis, genitals and lower back. In the lower back area, lesions were erythematous, purplish targetoid-like with multiple concentric circles. In places, bullae and postblistering erosions could be seen. Examination of oral cavity lesions poses various diagnostic challenges and thus a thorough examination with history can help to reach a diagnosis. Human cytomegalovirus infection usually proceeds asymptomatically in immunocompetent patients. In symptomatic forms, mononucleosis syndrome is the most common manifestation. However, atypical cases of cytomegalovirus infections in immunocompetent subjects are reported in the literature.
