Somatosensory evoked potentials correlate with neurological outcome in rabbits undergoing cerebral air embolism.

BACKGROUND AND PURPOSE: Somatosensory evoked potentials (SSEPs) have been used as an outcome measure in models of cerebral air embolism despite the lack of studies correlating SSEPs with other measures of neurological injury. We examined the relationship between SSEPs and neurological impairment in the setting of cerebral air embolism. METHODS: Anesthetized New Zealand White rabbits received either 0, 50, 100, or 150 microL/kg of air into the internal carotid artery. SSEPs were recorded at intervals for the subsequent 2 hours. After the final recording the anesthetic was discontinued, and the…

Objective detection of hemifield and quadrantic field defects by visual evoked cortical potentials.

AIMS/BACKGROUND: An objective method for detecting hemifield and quadrantic visual field defects has been developed using steady state visual evoked cortical potentials (VECPs), an adaptive noise canceller (ANC), and Hotelling's t2 statistic. The purpose of this study was to determine the sensitivity and specificity of the technique. METHODS: Nine subjects (mean age 44 years) were investigated with field loss due to a variety of causes including both anterior and posterior visual pathway lesions. Dynamic perimetry was performed by means of a Goldmann or Tübingen perimeter. VECP recordings wer

Brainstem evoked potentials in three groups of prisoners after release from detention camp.

Brainstem evoked potentials (BAEPs) were determined in three groups of male prisoners of war (POWs) released from detention camps and a control group. The first group comprised 21 POWs in whom BAEPs were determined 10-60 days after release (group I). The second group comprised 24 POWs in whom BAEPs were determined 6-9 months after release (group II), and the third group comprised 22 POWs in whom BAEPs were determined 12-18 months after release (group III). The control group comprised 32 subjects. The following changes were found in relation to the control group: in group I significantly longer…

Effect of intrathecal opioids on somatosensory-evoked potentials during spinal fusion in children.

STUDY DESIGN: Intraoperative recording of somatosensory-evoked potentials is useful for monitoring spinal cord tolerance during spinal fusion with instrumentation. Volatile anesthetic agents are known to have prominent suppressive effects on somatosensory-evoked potentials. This study evaluates the effect of intrathecal administration of opioid, consisting of morphine sulfate and sufentanil, on somatosensory-evoked potential monitoring. OBJECTIVES: To study the effect of different anesthesia techniques on somatosensory-evoked potentials during spinal fusion. SUMMARY OF BACKGROUND DATA: The e…

Brainstem auditory evoked potentials (BAEPs) pathology in topographical diagnosis of posterior fossa lesions.

The authors tried to determine the level of the brainstem where possible generators of various BAEPs waves could be situated by correlating clinical neurological symptomatology and/or cranial computed tomography findings of 195 patients with posterior fossa lesions with distinct BAEPs waves pathology. Results of statistical analysis showed a significant dependence between the levels of lesions as revealed by clinical symptomatology and/or cranial computed tomography and distinct BAEPs waves I-V pathology. Only indirect proof exists for the supratentorial origin of waves VI and VII. Because of

Abnormal somatosensory evoked potentials in patients with classic galactosemia: correlation with neurologic outcome.

In classic galactosemia, long-term neurologic sequelae can include low cognitive functioning and a curious neurologic syndrome with tremors, dysmetria, and ataxia. An abnormal white-matter signal on cerebral magnetic resonance imaging (MRI) is present in almost all patients; some have mild cerebral or cerebellar atrophy and focal white-matter lesions. The present study was undertaken to assess the integrity of myelinated pathways by recording somatosensory evoked potentials. Results were correlated with age at diagnosis, severity of illness, age at evoked potentials, neurologic examination, MR…

Long-lasting depression of motor-evoked potentials to transcranial magnetic stimulation following exercise.

We used transcranial magnetic stimulation to study the modulation of motor cortex excitability after rapid repetitive movements. Eleven healthy subjects aged 24-32 years were evaluated. Serial motor-evoked potential (MEP) recordings were performed from the right thenar eminence every 5 min for a period of 20 min at rest and for a period of 35 min after repetitive abduction-adduction of the thumb at maximal frequency for 1 min. All subjects presented distinct changes in MEP amplitude after exercise with an approximately 55% mean maximal decrease compared with basal conditions and complete recov

The effects of lead on otoacoustic emissions and auditory evoked potentials in monkeys.

Auditory functioning was assessed in two groups of adult rhesus monkeys (11 years of age). One (n = 11) received modest exposure to lead early in life and the other (n = 8) served as controls and did not receive any lead supplementation. Two lead-exposed monkeys had abnormal distortion product otoacoustic emissions (DPEs) and smaller amplitude or absent evoked potentials. These monkeys had abnormal distortion product otoacoustic emission (DPEs) and smaller amplitude or absent evoked potentials. These monkeys had the highest blood levels recorded in their respective groups. For the remaining le…

Visual evoked potentials in phenylketonuria: association with brain MRI, dietary state, and IQ.

At separate institutions, pattern reversal visual evoked potentials (VEPs) were recorded in children and older patients with phenylketonuria and compared with MRI of the brain. In nine patients aged less than 14 years, who were still on a diet low in phenylalanine, VEPs were clearly abnormal in only one and the abnormalities seen on MRI were mild. In 27 patients aged 14-31 years VEPs were abnormal in more than 80%, with significant reduction of amplitude and prolongation of latency despite the general absence of visual symptoms and abnormalities on routine neuro-ophthalmological examination. A…

Influence of age on cerebral potentials evoked by oesophageal balloon distension in humans.

The recording of viscerosensory cerebral evoked potentials is a new field in the research on gastrointestinal perception. The aim of our study was to assess the relationships between age and peak amplitudes and latencies of cerebral potentials evoked by balloon distension of the human oesophagus. In 16 healthy volunteers (aged 21-59 years), cerebral evoked potentials were recorded from a midline scalp electrode, using a series of 50 rapid balloon inflations with 13 ml of air. Peak to peak amplitudes (N1-P1, P1-N2) and peak latencies (N1, P1, N2) were assessed. Inverse correlations were found b…

Modification of cortical stimulation for motor evoked potentials under general anesthesia: technical description.

This pilot study presents a possible modification of direct cortical electrical stimulation technique for the recording of motor evoked potentials under general anesthesia. The exposed primary motor cortex was stimulated by a short train of anodal rectangular pulses at high frequency (300-500 Hz), while the compound muscle action potentials were recorded from the forearm and hand muscles. When compared with the traditional way of eliciting movement of the extremities by applying a train of pulses at lower frequency (50-60 Hz), muscle responses were obtainable at an intensity of much lower char…

Giant somatosensory evoked potentials in a patient with the anterior spinal artery syndrome.

We studied a previously healthy 25-year-old woman with the anterior spinal artery syndrome, a rare thoracocervical myelopathy with multiple potential etiologies. Quantitative and clinical sensory examination showed dissociated loss of pin-prick and temperature discrimination below the level of the lesion, with normal light touch, vibratory, and position sense. Magnetic resonance imaging was consistent with cervical spinal cord infarction. Median SEPs showed normal Erb's potential with absent spinal N13- and normal scalp N20- latency. Tibial SEPs showed normal lumbosacral responses and normal s

Tethered cord after spina bifida aperta: a longitudinal study of somatosensory evoked potentials.

Progressive neurological deterioration may occur after meningomyelocele repair. Magnetic resonance imaging almost invariably demonstrates a conus medullaris in an abnormally low position, whether neurological symptoms develop or not. Surgery of a secondary tethered cord is indicated when progression of neurological symptoms is documented. We performed a longitudinal study of posterior tibial nerve somatosensory evoked potentials (SSEPs) in children and adolescents after neonatal meningomyelocele repair. All patients were able to walk. Declining or negative posterior tibial nerve SSEPs were rec…

Surface and epidural lumbosacral spinal cord evoked potentials in chronic spinal cord injury.

Nine patients were examined in the chronic stage of spinal cord injury (12 to 56 months postinjury). Surface lumbosacral spinal cord evoked potentials (LSEPs) were obtained using electrodes placed over the S1, L2, L4, and T12 vertebral levels, referenced to a T6 surface electrode. Epidural LSEPs were obtained using a multielectrode lead placed percutaneously into the epidural space for evaluation of the efficacy of spinal cord stimulation for modification of pain and spasticity. The LSEPs resulting from supramaximal stimulation of the tibial nerve at the popliteal fossa were composed of propag…

[Visual electrically evoked potentials in healthy subjects and patients with optic nerve atrophy].

Visual electrically evoked potentials (VEEP) were measured by the method developed by the author in 14 healthy subjects (14 eyes) and 20 patients with optic atrophy (22 eyes). The normative values were established. VEEP with slightly decreased amplitude, otherwise unchanged, were found in the patients in all the cases with visual acuity of at least 0.03. Comparative study of VEEP and flash VEP has lead the author to a hypothesis that retinal ganglion cells and their axons are a source of VEEP (provided suprathreshold electrical stimuli are employed). Comparison of VEEP, flash and pattern VEPs …

[Normal values and age-related changes in magneto-electric evoked compound muscle potentials].

A number of 57 normal subjects was investigated using transcranial magnetic stimulation of the motor cortex and transcutaneous magnetic stimulation of the spinal nerve root in order to obtain normative data for central and peripheral motor latencies. Under standardized conditions (site of stimulation, stimulus intensity, degree of voluntary tonic background activation) muscle compound action potentials were recorded from different muscles of the upper and lower extremity: M. biceps brachii, M. extensor carpi radialis, M. interosseus dorsalis I, M. vastus medialis, M. tibialis anterior, and M.

Evoked potentials from direct cerebellar stimulation for monitoring of the rodent spinal cord.

Although the assessment of spinal cord function by electrophysiological techniques has become important in both clinical and research environments, current monitoring methods do not completely evaluate all tracts in the spinal cord. Somatosensory and motor evoked potentials primarily reflect dorsal column and pyramidal tract integrity, respectively, but do not directly assess the status of the ventral funiculus. The present study was undertaken to evaluate the use of evoked potentials, elicited by direct cerebellar stimulation, in monitoring the ventral component of the rodent spinal cord. Twe…

Evoked CA3 field potentials corresponding to both EPSPs and IPSPs in hippocampal slice.

Biphasic field potentials were recorded in the CA3 distal dendritic region in response to both antidromic (fornix) and orthodromic (mossy fiber) stimulation in guinea pig hippocampal slices in vitro. The positive component (P1) corresponded to intracellularly recorded excitatory postsynaptic potentials. The negative component (N1) appears to be due to GABAA-mediated inhibitory postsynaptic potentials (IPSPs) since it corresponded to the fast IPSP recorded intracellularly, was blocked by bicuculline and penicillin and augmented by barbiturates. The amplitude of N1 and the duration of P1 are sen
