The ergoreflex: how the skeletal muscle modulates ventilation and cardiovascular function in health and disease.

The control of ventilation and cardiovascular function during physical activity is partially regulated by the ergoreflex, a cardiorespiratory reflex activated by physical activity. Two components of the ergoreflex have been identified: the mechanoreflex, which is activated early by muscle contraction and tendon stretch, and the metaboreflex, which responds to the accumulation of metabolites in the exercising muscles. Patients with heart failure (HF) often develop a skeletal myopathy with varying degrees of severity, from a subclinical disease to cardiac cachexia. HF-related myopathy has been a

Exercise intolerance in chronic heart failure: the role of cortisol and the catabolic state.

Chronic heart failure (CHF) is a complex clinical syndrome leading to exercise intolerance due to muscular fatigue and dyspnea. Hemodynamics fail to explain the reduced exercise capacity, while a significant skeletal muscular pathology seems to constitute the main underlying mechanism for exercise intolerance in CHF patients. There have been proposed several metabolic, neurohormonal and immune system abnormalities leading to an anabolic/catabolic imbalance that plays a central role in the pathogenesis of the wasting process of skeletal muscle myopathy. The impairment of the anabolic axes is as

Brachycephalic Obstructive Airway Syndrome: Grading, Assessment, and Treatment.

The anatomic abnormalities found in dogs with brachycephalic obstructive airway syndrome lead to changes in airway resistance and turbulent airflow. Altered airway resistance leads to secondary airway changes. All these changes cause airway noises, which may be present at rest or elicited during exercise. Exercise tolerance tests, careful respiratory auscultation, and identification of increased effort respiratory can help to grade brachycephalic dogs into different categories based on disease severity. Other categorization systems and multiple surgical procedures aim to stratify and alter the…

Improvement of diaphragmatic excursion in the supine position in a patient with idiopathic pleuroparenchymal fibroelastosis: A case report.

Idiopathic pleuroparenchymal fibroelastosis (iPPFE) is a rare form of idiopathic interstitial pneumonia. We report a case of a patient with iPPFE in whom postural changes improved diaphragmatic excursion (DE) and exercise tolerance. Chest radiography showed a greater elevation of the diaphragm at maximum expiration in the supine position than the standing position. DE measured by ultrasonography was higher in the supine position than the standing position. The findings may suggest greater diaphragm movement in the supine position, leading to successful rehabilitation and improved exercise endu…

Return to Cycling Following Brain Injury: A Proposed Multidisciplinary Approach.

Cycling is an important form of exercise, recreation, and transportation. Following traumatic brain injury, the benefits of cycling for health, fitness, and community mobility must be considered alongside potential risk for recurrent injury. In addition to medical concerns and exercise tolerance, key domains include motor function, attention, and visuospatial and executive function, which have previously been explored with regard to driving. Cycling skill is a combination of cognitive and motor function, and can be trained with appropriate education and intervention. We discuss the relationshi…

Temporary heat intolerance in a heatstroke patient.

A case history of recurrent heatstroke is presented. The first heatstroke was incurred during strenuous exercise following acute gastroenteritis; the second heatstroke happened 1 month later during a march in hot climatic conditions. Two heat tolerance tests were performed. On the first, 1 month after the second incidence, an intolerance to heat was observed which, on the second test 5 months after the incidence, had disappeared. It is proposed that acute intercurrent illness and heatstroke in itself cause temporary susceptibility to exercise in heat, and that every heatstroke patient should b

Characterization and Application of Novel Exercise Recovery Patterns That Reflect Cardiac Performance: A Substudy of the SEQUOIA-HCM Trial.

BACKGROUND: Post-exercise oxygen uptake recovery (VO2Rec) is slow in advanced heart failure. We sought to establish easily derived VO2Rec measures and evaluate their cardiospecificity and prognostic relevance in patients with dyspnea on exertion. We further sought to determine VO2Rec modifiability proportional to changes in cardiac function with disease-specific treatment of obstructive hypertrophic cardiomyopathy. METHODS: VO2Rec patterns were evaluated in relation to cardiac performance and the primary outcome of heart failure hospitalization or death in a referral cohort with dyspnea on ex

Are exercise booster sessions effective at preserving exercise-induced effects in people with multiple sclerosis? A randomised controlled trial: results from the MSBOOST trial.

OBJECTIVES: In people with multiple sclerosis (PWMS), exercise improves aerobic capacity and muscle strength which are key components of physical function and overall health status. However, the effects diminish after exercise cessation. Therefore, the present study aimed to investigate whether exercise booster sessions (EBS) could preserve the exercise-induced improvements. DESIGN: Multicentre RCT. METHODS: Ninety-four PWMS performed 12 weeks of aerobic-training (AT) or resistance-training (RT) and were subsequently randomised to receive EBS (two sessions every fifth week) (ATboost n = 24,

Plasma catecholamines in patients undergoing invasive cardiopulmonary exercise test for exercise intolerance.

BACKGROUND: Invasive cardiopulmonary exercise testing (iCPET) combines traditional cardiopulmonary exercise testing with invasive hemodynamic measurements to assess exercise intolerance, which can be caused by preload insufficiency (PI), characterized by low ventricular filling pressures and reduced cardiac output during exertion. We hypothesize that plasma catecholamine levels at rest and during exercise correlate with hemodynamic parameters in PI. METHODS: We included adult patients who underwent iCPET for exercise intolerance and had plasma catecholamines measured at rest and peak exercise

Are Aerobic Programs Similar in Design to Cardiac Rehabilitation Beneficial for Survivors of Stroke? A Systematic Review and Meta-Analysis.

Background Survivors of stroke face movement disability and increased cardiovascular disease and stroke risk. Treatment includes rehabilitation focused on functional movement with less emphasis on aerobic capacity. After rehabilitation, survivors of stroke must self-manage activity with limited appropriate community programs. Lack of structured activity contributes to sedentary behavior. The objective of this systematic review and meta-analysis is to review aerobic programs for stroke survivors similar in activity and dosage to cardiac rehabilitation programs to determine their efficacy for im

Physiological insights of exercise hyperventilation in arterial and chronic thromboembolic pulmonary hypertension.

BACKGROUND: Pulmonary hypertension (PH) patients show, during exercise, an excessive increase in ventilation (VE) compared to carbon dioxide output (VCO2), determining a high VE/VCO2 slope. There are several possible causes, including an elevated dead space ventilation (VD), VE/perfusion (Q) mismatch and/or an enhanced peripheral or central chemoreceptor activity. We evaluated the causes of exercise hyperventilation in PH patients. METHODS: Eighteen group I and IV PH patients underwent cardiopulmonary exercise test with blood gas analysis at every minute. VE, alveolar ventilation (VA) and VD

Nitric oxide exhalation correlates with ventilatory response to exercise in patients with heart disease.

AIMS: It is controversial whether or not pulmonary nitric oxide (NO) production, reflected in the end-tidal alveolar NO concentration, is diminished in patients with heart failure. Since pulmonary perfusion is regulated by NO production, decreased NO production in the pulmonary vasculature is assumed to result in diminished lung perfusion and further increases in ventilation-perfusion mismatch. The aim of this study is to investigate whether exhaled NO correlates with both exercise-induced hyperpnea and exercise tolerance in patients with heart disease. METHODS AND RESULTS: Forty-two patients…

Exercise-induced complete heart block in a patient with chronic bifascicular block.

Heart block induced by exercise or associated with symptomatic, chronic bifascicular block can progress to high-grade atrioventricular (AV) block and sudden death. The authors describe a case of exercise-induced AV block in a patient with chronic bifascicular block. Thirty seconds of ventricular asystole were observed during exercise treadmill testing. Myocardial ischemia was ruled out, and the cause was determined to be distal AV nodal block. The pathophysiology, diagnosis, and treatment of distal AV nodal block are reviewed. The role of exercise treadmill testing and electrophysiologic studi

Food-dependent exercise-induced anaphylaxis with negative allergy testing.

Food-dependent exercise-induced anaphylaxis (FDEIA) is a disorder where exercise following allergen ingestion triggers anaphylaxis although exercise and allergen exposure are independently tolerated. The diagnosis of FDEIA is based on a characteristic clinical history. The culprit allergen is usually confirmed through the use of skin prick testing (SPT) serum-specific IgE levels and a food-exercise challenge. We present a case of FDEIA suggested by clinical history and open food-exercise challenge with negative specific IgE levels and SPT that highlights the challenges involved in diagnosing a

Endurance Performance is Influenced by Perceptions of Pain and Temperature: Theory, Applications and Safety Considerations.

Models of endurance performance now recognise input from the brain, including an athlete's ability to cope with various non-pleasurable perceptions during exercise, such as pain and temperature. Exercise training can reduce perceptions of both pain and temperature over time, partly explaining why athletes generally have a higher pain tolerance, despite a similar pain threshold, compared with active controls. Several strategies with varying efficacy may ameliorate the perceptions of pain (e.g. acetaminophen, transcranial direct current stimulation and transcutaneous electrical stimulation) and
