Validation of a monoclonal antibody-based immunofluorescent assay to detect Burkholderia pseudomallei in blood cultures.

BACKGROUND: Identification of Burkholderia pseudomallei, the cause of melioidosis, using routine methods takes several days. Use of a monoclonal antibody-based immunofluorescent assay (IFA) on positive blood cultures may speed diagnosis. METHODS: We tested the diagnostic accuracy of the IFA on 545 blood cultures positive for Gram-negative organisms at Udon Thani Hospital, Thailand, between June 2015 and August 2016. RESULTS: Sensitivity of the IFA was 100% and specificity was 99.6%. The median decrease in time to pathogen identification between the IFA result and routine methods was 28 h (IQ

Age-related infertility.

Oocyte number and quality decrease with advancing age. Thus, fecundity decreases as age increases, with a more rapid decline after the mid-30s. Patients more than 35 years old should receive prompt evaluation for causes of infertility after no more than 6 months of attempted conception. Patients with abnormal tests of ovarian reserve have a poorer prognosis and may need more expedited and aggressive treatment. Although oocyte donation is the best method to overcome age-related infertility, other treatment options may help women proceed quicker toward pregnancy. Patients at an advanced age shou

[Focused cardiac ultrasound in internal medicine].

Focused cardiac ultrasound is a noninvasive technique which can be performed by the internist. It is capable of providing useful, rapid and accurate information about the structure and function of the heart. It can be used to complement anamnesis and physical examination at the bedside. Clinicians may be able to determine left and right ventricular diameter and function, detect the presence of significant pericardial effusion, identify important valvular defects and estimate the central venous pressure by measuring the diameter and collapsibility of inferior vena cava with good accuracy after

Outbreak of leptospirosis among white-water rafters--Costa Rica, 1996.

On October 15, 1996, a physician notified the Illinois Department of Public Health about five patients with an unknown febrile illness who had returned from a white-water rafting trip on flooded rivers in Costa Rica during September 27-28, 1996. The five patients had been members of a white-water rafting expedition involving 26 rafters from five states, the District of Columbia, and Costa Rica. This report summarizes the findings of the multistate investigation conducted by the Illinois Department of Public Health and by CDC in collaboration with the Ministry of Health of Costa Rica. The findi

Gastroschisis and reduced fetal heart-rate variability.

Most liveborn babies with gastroschisis do well after surgical repair, although about one-eighth of affected cases die late in utero. Our practice is to use weekly computerised cardiotocograph (CTG) analysis after week 34 of gestation in cases of gastroschisis. In a look-back at the records in 18 such singleton pregnancies, CTG showed 7 to be highly abnormal or preterminal. All but 1 of these 7 had a normal fetal heart rate. In all 7 cases, delivery was expedited. Only 1 infant had neurological sequelae, and in all the abdominal defect was successfully repaired. Monitoring of these high-risk p

Pulmonary Embolism Presenting as Backache After Appendectomy: An Atypical Presentation of a Common Diagnosis.

Pulmonary embolism (PE) is a disease with diverse clinical manifestations, which lacks specificity but may be life-threatening. We describe a 39-year-old male who presented with acute PE complicated by pulmonary infarction, confirmed by computed tomography pulmonary angiography 2 weeks following laparoscopic appendectomy. His initial symptom was right lower-back pain. This case underscores the diagnostic challenge posed by an atypical presentation of PE, in which isolated back pain was the sole initial manifestation. Furthermore, we delineate the pathophysiologic mechanisms that may account fo

Case Report: Pediatric nasopharyngeal carcinoma masquerading as benign lymphadenopathy: diagnostic pitfalls in two cases.

BACKGROUND: Diagnosis of pediatric nasopharyngeal carcinoma (NPC) is frequently delayed due to its rarity and histologic mimicry of benign lymphoproliferative disorders. CASE PRESENTATION: We report two pediatric cases of NPC presenting with isolated cervical lymphadenopathy, initially misdiagnosed as Kimura disease and classical Hodgkin lymphoma, respectively. Both patients were treated with multimodal chemoradiotherapy and remain under follow-up. CONCLUSIONS: These cases highlight the diagnostic challenges of pediatric NPC, particularly its mimicry of reactive or inflammatory conditions. P

Digitally guided alveolar nerve repositioning surgery: a novel approach to the atrophic posterior mandible.

Inferior alveolar nerve repositioning is an uncommon surgical procedure performed in oral surgery to safeguard the alveolar nerve during certain dental or reconstructive procedures. While this procedure is generally safe and effective, it is important to understand the potential risks and complications associated. This article presents a computer-guided surgical stent to expedite the surgical accuracy while reducing the risks of nerve injury during alveolar nerve repositioning using local anaesthesia alone. In conclusion, the use of a bone supported guide could improve surgical outcomes, altho

Femoral Neck Fracture in a Pediatric Patient with Primary Hyperparathyroidism.

CASE: A previously healthy 11-year-old girl underwent expedited surgical fixation of a femoral neck fracture sustained while jump-roping. After further work up, she was diagnosed with primary hyperparathyroidism. Parathyroidectomy of a hypertrophic adenoma proved curative. Now, five months post left hip surgery, the patient is pain-free and walks without a limp. CONCLUSION: We describe the first published case of primary hyperparathyroidism presenting as a pathologic hip fracture in a child. Although presentation with a fracture is exceedingly rare, bone pain is a frequent complaint of pediat

Penetrating Aortic Ulceration With Pseudoaneurysm and Intramural Hematoma: Emergency Department Management and Point-of-Care Ultrasound Diagnosis.

Penetrating aortic ulcer (PAU) complicated by an intramural hematoma is a rare and potentially life-threatening emergency department (ED) presentation that is defined by progressive ulceration through the intima layer into the media layer of the aorta. Symptomatic PAUs can be clinically indistinguishable from other life-threatening pathologies such as aortic dissection, acute coronary syndrome (ACS), intrabdominal catastrophes as well as less lethal processes such as musculoskeletal back pain. Given the potential of PAUs to result in lethal aortic rupture and dissection, the emergency provider

Organocatalytic synthesis of (Het)biaryl scaffolds via photoinduced intra/intermolecular C(sp2)-H arylation by 2-pyridone derivatives.

A unique N,O-bidentate ligand 6-oxo-1,6-dihydro-pyridone-2-carboxylic acid dimethylamide (L1) catalyzed direct C(sp2)-H (intra/intermolecular) arylation of unactivated arenes has been developed to expedite access to (Het)biaryl scaffolds under UV-irradiation at room temperature. The protocol tolerated diverse functional groups and substitution patterns, affording the target products in moderate to excellent yields. Mechanistic investigations were also carried out to better understand the reaction pathway. Furthermore, the synthetic applicability of this unified approach has been showcased via

State of the Art-High-Sensitivity Troponins in Acute Coronary Syndromes.

In cases of suspected acute coronary syndrome (ACS), rapid and accurate diagnosis is essential to establish effective evidence-based medical treatment. Patients' history, clinical examination, 12-lead electrocardiogram, and cardiac biomarkers are cornerstones in initial management. Since high-sensitivity cardiac troponins were established, they have markedly expedited and revolutionized rule-in and rule-out pathways of patients with ACS and changed our everyday clinical practice. Thus, they have become an indispensable tool in daily routine in emergency units. This review focuses on historical

Differential Diagnosis of Diarrhea in Patients With Neuroendocrine Tumors.

Patients with neuroendocrine tumors (NETs) and carcinoid syndrome experience diarrhea that can have a debilitating effect on quality of life. Diarrhea also may develop in response to other hormonal syndromes associated with NETs, surgical complications, medical comorbidities, medications, or food sensitivities. Limited guidance on the practical approach to the differential diagnosis of diarrhea in these patients can lead to delays in appropriate treatment. This clinical review and commentary underscore the complexity in identifying the etiology of diarrhea in patients with NETs. Based on our c

Bilateral cataracts as the presenting feature of type 1 diabetes in an 11-year-old girl.

Paediatric cataracts can present a diagnostic dilemma to ophthalmologists. Next-generation DNA sequencing (NGS) has been promoted as a tool to expedite the diagnosis of an underlying cause in such cases.1 The authors present an unusual case of bilateral cataracts in an 11-year-old girl as the first presenting feature of new type 1 diabetes mellitus. Prompt diagnosis and subsequent management of this potentially life-threatening condition were achieved through careful history taking and targeted biochemical testing. The authors feel this case highlights the significance of simple measures such

Surgical Approaches to Chronic Pancreatitis: Indications and Techniques.

There are a number of surgical strategies for the treatment of chronic pancreatitis. The optimal intervention should provide effective pain relief, improve/maintain quality of life, preserve exocrine and endocrine function, and manage local complications. Pancreaticoduodenectomy was once the standard operation for patients with chronic pancreatitis; however, other procedures such as the duodenum-preserving pancreatic head resections and its variants have been introduced with good long-term results. Pancreatic duct drainage via a lateral pancreaticojejunostomy continues to be effective in ameli

Point-of-Care Diagnosis of Cardiac Tamponade Identified by the Flow Velocity Paradoxus.

The presentation of cardiac tamponade is a spectrum from occult to extreme. The clinical history, physical exam, electrocardiogram, and radiographic findings of tamponade have poor sensitivities and even worse specificities. We use a clinical scenario to demonstrate how point-of-care cardiac ultrasound can diagnose impending cardiac tamponade in a clinically stable patient. The ultrasound finding we recommend is the flow velocity paradoxus, in which respiratory variation causes significant changes in transvalvular inflow velocities, which are exaggerated when tamponade is present. The manageme

CAP--advancing the evaluation of preclinical Alzheimer disease treatments.

If we are to find treatments to postpone, reduce the risk of, or completely prevent the clinical onset of Alzheimer disease (AD), we need faster methods to evaluate promising preclinical AD treatments, new ways to work together in support of common goals, and a determination to expedite the initiation and performance of preclinical AD trials. In this article, we note some of the current challenges, opportunities and emerging strategies in preclinical AD treatment. We describe the Collaboration for Alzheimer's Prevention (CAP)-a convening, harmonizing and consensus-building initiative to help s

Drug resistance analysis by next generation sequencing in Leishmania.

The use of next generation sequencing has the power to expedite the identification of drug resistance determinants and biomarkers and was applied successfully to drug resistance studies in Leishmania. This allowed the identification of modulation in gene expression, gene dosage alterations, changes in chromosome copy numbers and single nucleotide polymorphisms that correlated with resistance in Leishmania strains derived from the laboratory and from the field. An impressive heterogeneity at the population level was also observed, individual clones within populations often differing in both gen

Disseminated Nocardia paucivorans infection in an immunocompetent host.

Nocardia paucivorans is a recently discovered species that has been shown to have a predilection for CNS involvement in cases of disseminated infection. We present a 50-year-old man with one year of weight loss admitted to the hospital with cough and confusion. Imaging revealed pulmonary and iliopsoas masses as well as innumerable ring-enhancing brain lesions. N. paucivorans was eventually identified in a subcarinal lymph node biopsy. The diagnosis was expedited by utilizing 16s rRNA gene sequencing on the biopsy tissue, resulting in species-level identification several weeks prior to culture
