Surgical treatment of pathological subtrochanteric fractures due to benign lesions in children and adolescents.

Twelve children and adolescents with pathological subtrochanteric fractures due to benign tumors or tumorlike lesions were treated by curettage, autologous cancellous bone graft, and internal fixation. The healing of the lesion and the fracture in nine patients was without complications. In the remaining three, the healing was complicated. In one, with nonunion and shortening due to 35 degrees varus deformity following conservative treatment, a 1-cm shortening due to 15 degrees varus deformity remained after surgery. In the same patient, a small recurrence was curetted when removing the plate

Lytic lesions of the femoral neck in rheumatoid arthritis simulating pigmented villonodular synovitis or malignancy.

Large lytic lesions, relatively asymptomatic, involving the femoral neck and the base of the head are described in two patients suffering from a classical seropositive rheumatoid arthritis. Histological examination failed to reveal signs of malignancy, infection or pigmented villonodular synovitis. There were no rheumatoid nodules but a chronic hypertrophic villous synovitis was found. Rheumatoid synovium may invade the superior extremity of the femur; this fact is important in the differential diagnosis of destructive lesions of the femoral neck in RA.

Subchondral metastasis: report of five cases.

Subchondral metastasis is a rare occurrence and poses a diagnostic dilemma as initial films may show a lytic lesion in the subchondral region often misinterpreted as being benign. We present five cases of subchondral metastasis as well as a review of the literature. In our cases, we present subchondral metastasis in the elbow, shoulder, and hip joints. All patients had pain over the affected joint and most presented with a lytic lesion in the subchondral bone. Three patients have died since presentation and two are doing well at last follow up visit. Subchondral metastasis is a rare entity, bu

Vascularized fibular graft for bone reconstruction of the extremities after tumor resection in limb-saving procedures.

We treated 18 patients (8 males and 10 females) with bone or soft tissue tumors in the extremities by vascularized fibular grafts (VFGs). The average age was 29 years. Two had malignant soft tissue tumors in the forearm (angiosarcoma and synovial sarcoma) and the other 16 had bone tumors [osteosarcoma (8), chondrosarcoma (3), angiosarcoma (1), adamantinoma (1), and giant cell tumor (3)]. Affected sites of the bone tumor cases were tibia (5), femur (5), humerus (3), radius (2), and ulna (1). According to the surgical staging system of Enneking et al. 1 patient was in stage III, 12 in IIB, 2 in

Feasibility of percutaneous cementoplasty combined with interventional internal fixation for impending pathologic fracture of the proximal femur.

PURPOSE: To evaluate the feasibility of percutaneous cementoplasty and interventional internal fixation for stabilization of impending pathologic fracture of the proximal femur. MATERIALS AND METHODS: From May 2012 to August 2013, six consecutive patients (three men and three women; median age, 58.33 y ± 21.45; age range, 18-78 y) who underwent percutaneous cementoplasty plus interventional internal fixation for the treatment of metastases to the proximal femur were retrospectively analyzed. The Karnofsky performance status (KPS) and visual analog scale (VAS) score for pain were assessed befo

Atypical femoral fracture with early PET/CT changes from denosumab for metastatic breast cancer and transition from denosumab to zoledronic acid.

Atypical femoral fractures (AFFs) are potential complications arising from long-term use of bone modifying agents such as denosumab. We describe a case of bilateral AFFs that was treated surgically in a middle-aged woman receiving oncologic doses of denosumab for treatment of metastatic breast cancer for seven years. In retrospect, early cortical change was present on scout computer tomography (CT) and positron emission tomography-computer tomography (PET-CT) prior to her left AFF. This case highlights the potential for earlier opportunistic AFF radiologic screening during cancer surveillance.

[Thymoma with primary osseous and pulmonary metastases. Case report and review of the literature].

Bone metastases can be the first symptom of a tumor. The case of a 65-year-old patient with a thymoma exhibiting primary metastases in the lung and bone is presented. Initially, a tumor was suspected because of an osteolytic destruction of the right proximal femur posing a high risk of fracture. Further diagnostic procedures detected a partially sclerosing mediastinal bulk and bilateral lung metastases. The histological investigation of the resected proximal femur and needle biopsy of the mediastinal bulk showed an extrathoracically metastasized polygonal cell thymoma. This metastasizing patte

Oncogenic osteomalacia: two case reports with surprisingly different outcomes.

Oncogenic osteomalacia is a rare paraneoplastic syndrome of acquired hypophosphatemic osteomalacia, resulting from a deficit in renal tubular phosphate reabsorption, in which fibroblast growth factor 23 (FGF23) seems to be implicated. This condition is usually associated with a phosphaturic mesenchymal tumor of mixed connective tissue located in the bone or soft tissue. The clinical and the radiologic findings are the same as those seen in osteomalacia, and the biochemical features include renal phosphate loss, low serum phosphate and 1,25-(OH)(2) vitD(3) levels, increased alkaline phosphatase

Surgery for metastatic lesions of the femur: good outcome after 245 operations in 216 patients.

We report our experience with surgery for femoral metastatic lesions, based on 216 patients who underwent a total of 245 operations for femoral metastatic lesions. The median age was 66 (30-94) years, and the most common diagnosis breast cancer, followed by prostate cancer. All patients had pain on weight bearing, 196 had pain at rest, 147 were unable to walk preoperatively, and 148 were confined to a health-care facility. The patients were operated with bipolar hip prosthesis (n=7), total hip replacement (THR) with Harrington reconstruction of the acetabulum (n=42), ordinary THR (n=108), intr

[Loosening of a total hip arthroplasty due to metastasis].

A case report of a metastatic caused loosening of a total hip arthroplasty one year after primary implantation is presented. A primary cancer was unknown at surgery. Due to suspected low-grade infection, a revision surgery was performed and the diagnosis of a metastatically caused loosening could be made. A bronchial carcinoma was identified as primary cancer. In spite of the rare incidence of the described metastatic loosening, the significance of a differential diagnostic strategy and the importance of a histological examination in revision surgery is depicted.

What Are the Complications, Reconstruction Survival, and Functional Outcomes of Modular Prosthesis and Allograft-prosthesis Composite for Proximal Femur Reconstruction in Children With Primary Bone Tumors?

BACKGROUND: Proximal femur reconstruction after bone tumor resection in children is a demanding surgery for orthopaedic oncologists because of the small bone size and possible limb-length discrepancy at the end of skeletal growth owing to physis loss. The most commonly used reconstruction types used for the proximal femur are modular prostheses and allograft-prosthesis composites. To our knowledge, there are no previous studies comparing the outcomes after modular prosthesis and allograft-prosthesis composite reconstruction of the proximal femur in children with primary bone tumors. QUESTIONS

Is It Appropriate to Treat Sarcoma Metastases With Intramedullary Nailing?

BACKGROUND: Patients with primary bone and soft tissue sarcoma are at risk for skeletal metastases. Although uncommon, these metastases can result in impending or pathologic fractures. Intramedullary nailing traditionally has been an accepted form of palliative treatment for patients with metastatic carcinoma, but we could find no studies that report specifically on intramedullary nailing of metastatic sarcoma lesions. QUESTIONS/PURPOSES: We asked: (1) What is the survival of patients with an impending or pathologic fracture from a sarcoma metastasis? (2) What proportion of patients treated w

The co-expression of p53 protein and P-glycoprotein is correlated to a poor prognosis in osteosarcoma.

Multidrug resistance (MDR), mediated by P-glycoprotein (P-gp), is an in vitro phenomenon observed within tumour cells, suggesting cross-resistance to unrelated drugs, and expression of P-gp may therefore affect the prognosis and incidence of recurrence after treatment. The mutant p53 protein causes reduced tumour suppression. Co-expression of p53 and P-gp is related to short survival, increased tumour activity and drug resistance. The purpose of this study was to measure the expression of p53 protein and P-gp in osteosarcoma tissue and assess its prognostic significance. Fifty-two tumour speci

Large tumor endoprostheses and extracortical bone-bridging: 28 patients followed 10-20 years.

Aseptic loosening is a common cause of failure in large tumor endoprostheses. The concept of extracortical bone-bridging was developed to tackle the problem of loosening. New bone which forms across the junction of the bone-prosthesis junction is believed to improve fixation by controlling the transfer of stresses across the junction as well as by giving additional stability to the prosthesis. We present the long-term experience with this concept following major reconstruction after tumor and non-tumor conditions in 31 patients. The overall function was good for upper and lower limb prostheses

Cementless fixation for primary segmental bone tumor endoprostheses.

To combat the high incidence of aseptic loosening for young patients and for patients with failed implants after resection for bone tumors, intramedullary cementless fixation of massive tumor implants was investigated. These implants consist of a hydroxyapatite coated titanium stem. To date, 47 of these prostheses have been inserted for the treatment of primary bone tumors. Radiographs indicate that the stems are osseointegrated. Radiolucent lines have not been seen between the implant and the bone. Bone remodeling changes have been observed. In several cases in which the implant was not seate
