[Claw plates--new implants for para-articular osteosynthesis].

The claw plate types used by us are manufactured according to the same principle. These plates are mostly applied in the treatment of multiple fragment fractures and in the fixation of osteotomies. A strong fixation of the plate, also to little fragments close to the joint, is achieved by the claws and one screw. The plates may be used for neutralization or as a support, distance piece, or compression plate. The shape of the variable plate as well as the claws can be formed. The masses of the maximum and minimum plate are adapted to the different sizes of bones. No special instrument is needed

Physeal injuries of the ankle in children: classification.

Seventy-one cases of ankle fractures in children were studied. An analysis of the fracture lines involving the distal tibial and fibular epiphyses were done. A new classification is proposed based on the position of the foot at the moment of trauma and the direction of the abnormal force. Four mechanisms were considered, grades were described for each mechanism and each grade always occurred in the same order. In almost half of the cases, the supination-inversion mechanism was responsible for the injury. A correlation with the physeal injury classification of Salter and Harris is imperative, s

Tibial fractures associated with civilian gunshot injuries.

Forty patients with 41 fractures of the tibia produced by civilian gunshot injuries were reviewed. The usual patient was male, 28 years of age, and had been shot by an unknown assailant. Thirty-two of the 41 fractures (78%) were the result of low-energy missiles (less than 500 foot pounds or 680 Nm) while six (15%) were from intermediate (500 to 1,200 foot pounds or 680-1,627 Nm) and three (7%) from high-energy missiles (greater than 1,200 foot pounds or 1,627 Nm). Characteristic fracture patterns for the low energy group consisted of the drill hole, unicortical, oblique, spiral, butterfly, an

Bipolar Clavicle Fracture: A Rare Entity of the Commonest Fracture - A Case Report.

INTRODUCTION: Bipolar clavicle fractures are a simultaneous injury to both ends of the clavicle and represent an exceptionally rare entity with an incidence of <1% of all clavicle fractures. Despite the clavicle being one of the most commonly fractured bone, involvement of both ends is frequently missed or misdiagnosed, leading to complications like persistent pain and compromised function. The rarity of these fractures has also led to a significant gap in established treatment guidelines. We present a case of a bipolar clavicle fracture with sternoclavicular joint dislocation treated successf

Anatomical evaluation of the modified posterolateral approach for posterolateral tibial plateau fracture.

OBJECTIVE: The study was undertaken to evaluate the efficacy and safety of a posterolateral reversed L-shaped knee joint incision for treating the posterolateral tibial plateau fracture. METHODS: Knee specimens from eight fresh, frozen adult corpses were dissected bilaterally using a posterolateral reversed L-shaped approach. During the dissection, the exposure range was observed, and important parameters of anatomical structure were measured, including the parameters of common peroneal nerve (CPN) to ameliorate the incision and the distances between bifurcation of main vessels and the tibial

Assessing fibular length using bimalleolar angular measurements.

Shortening of the fibula after fracture is common and often difficult to appreciate. Loss of lateral malleolar anatomy causes significant biomechanical changes in the ankle and correlates with poor clinical results. We studied angular measurements of distal fibular length to serve as a guide for assessing fibular reduction after ankle fracture. Mortise view X-rays of 50 normal ankles from 25 healthy volunteers were obtained. The average talocrural angle measured 78.5 degrees. However, individual variation was high with values ranging from 75 to 86 degrees. Comparing contralateral ankles demons

One-bone forearm formation using vascularized fibula graft for massive bone defect of the forearm with infection: case report.

Massive long-bone defects of greater than 6 cm are difficult to treat with conventional bone grafts, and other methods are sometimes recommended, such as vascularized bone grafts or bone transport using the Ilizarov external fixator. The combination of local infection with a massive bone defect exacerbates the problem, and provides an even more negative prognosis. The authors treated a large bone defect of the forearm with local infection, using a one-bone forearm formation with a large vascularized fibula graft. They attached an adequate amount of muscle fascia to the vascularized fibula, whi

Acute compartment syndrome and its predictors among patients with tibia fracture in Uganda: a multicenter prospective cohort study.

INTRODUCTION: Acute compartment syndrome (ACS) is a devastating and time-sensitive emergency that increases limb disabilities, amputations, … ncidence of ACS was found to be 8.5% (11/130). Among the 11 people with ACS, all had both tibia and fibula fractures, and 8/11 had closed fractures. Smoking (aRR = 2.189, CI = 1.614-7.812, P = 0.049) and swelling of the injured limb (aRR = 3.793, CI = 1.479-9.727, P = 0.008) were the independent predictors of ACS. CONCLUSION: The incidence of ACS in Uganda was 8. …

Intramedullary Nailing of Concurrent Bilateral Tibial Stress Fractures Followed by Unilateral Tension Plating for a Nonunion in a Vitamin D-Deficient Elite Football Player.

Stress fractures consist of a type of bone fracture that occurs due to repetitive mechanical stress instead of acute forceful injuries that cause common fractures. They are quite common among athletes at all competition levels and in army recruits who are expected to undergo extremely demanding exercises. While stress fractures can occur in any long bone, they are usually associated with the most common weight-bearing sites of lower extremities such as phalanges, metatarsals, tarsal bones, the tibia, and fibula. In this study, we report the surgical management of a 23-year-old African football

Primary diaphyseal tuberculosis of the tibia, presenting as a pathological fracture - A case report.

BACKGROUND: Isolated involvement of the shaft of a long bone is a rare presentation of skeletal tuberculosis (TB). To our knowledge, primary skeletal TB of the tibia manifesting as a pathological fracture has not been widely reported. We report a case of primary TB of the right tibia in an immunocompetent 85-year-old woman. CASE PRESENTATION: An immunocompetent 85-year-old woman with tuberculosis osteomyelitis of her right tibia and fibula, presenting as pathological fracture. Fibular ostectomy was performed followed by application of a bioabsorbable bone substitute and an Ilizarov external f

[Pilon fractures. Part 2: Repositioning and stabilization technique and complication management].

The management of complex pilon fractures with soft tissue injuries has seen many trends, with changes toward staged protocols of temporary external fixation followed by delayed open reduction and internal fixation (ORIF), minimally invasive percutaneous plate osteosynthesis (MIPPO) techniques and special implants, the benefits of negative pressure wound sealing and early "fix and flap" efforts to reconstruct soft tissue defects. Reduction and fixation must involve cautious management and careful handling of soft tissue in order to minimize the well-known complications of this difficult fractu

Acute paediatric ankle trauma: MRI versus plain radiography.

OBJECTIVE: To evaluate the diagnosis of acute physeal ankle fractures on plain radiographs using MRI as the gold standard. METHODS: Sixty consecutive children, 29 with a clinical diagnosis of lateral ligament injury and 31 with physeal ankle fractures, were examined using both radiographs and MRI in the acute period. The imaging data were reviewed by three "masked" radiologists. The fracture diagnosis and Slater-Harris classification of radiographs were compared with findings on MRI. RESULTS: Plain radiography produced five of 28 (18%) false negative and 12 of 92 (13%) false positive fractur

The Use of Pediatric Flexible Intramedullary Nails for Minimally Invasive Fibular Fracture Fixation.

Fibular fractures in the setting of an unstable ankle joint require surgical fixation; however, several factors contradict open surgical correction. Severe soft tissue compromise can delay adequate fracture reduction and preclude the standard incisional approach. The soft tissue envelope in the setting of obesity, diabetes, and/or peripheral vascular disease further complicates definitive treatment. Poorly timed open fixation can lead to delayed healing of the incision site, with wound breakdown and the potential for hardware failure. Proximal fibular fractures are also at unique risk of neuro
