Embryos created for research purposes.

The creation of embryos for research use has drawn a great deal of criticism. It is difficult to defend an ethical distinction between what one can do to "spare" embryos and what one can do to "Исследование" embryos. The strongest ground on which to argue against the creation of embryos for research is a symbolic one, having to do with respect for human life. Ronald Dworkin's work in Life's Dominion on the symbolic meaning of the abortion debate throws a helpful light on this dispute. By understanding the basic question to be, Does the creation of research embryos weaken or insult our communal res

Access barriers to in vitro diagnostics in Cambodia, Indonesia, Lao PDR, and the Philippines: programmatic lessons from NEDL field missions.

BACKGROUND: Access to essential in vitro diagnostics (IVDs) remains limited in many low- and middle-income countries, where persistent challenges exist in financing, human resources, infrastructure, procurement, supply chain management, and health insurance arrangements. Despite growing regional initiatives, such as the Association of Southeast Asian Nations (ASEAN) Essential Diagnostics List Initiative, country-specific evidence on barriers to IVD access remains limited. This article summarizes barriers to accessing IVDs and related lessons identified from field missions conducted as part of

Next-generation sequencing and bioinformatics capacity: findings from a multi-country survey to guide the genomics costing tool 2.0.

Next-generation sequencing (NGS) and bioinformatics are critical to infectious disease surveillance, outbreak detection and response, and the research and development of medical countermeasures. Achieving sustainable genomic surveillance requires countries to develop costed national strategies that integrate financial planning and budgeting across sequencing and bioinformatics activities. The genomics costing tool (GCT) was initially developed to estimate the costs of SARS-CoV-2 sequencing and associated bioinformatics. In response to growing country demand, the tool has now been expanded to s

Inequity in access to medications among communities in Eastern Ethiopia: a decomposition analysis.

BACKGROUND: Access to affordable essential medicines, including antibiotics, remains a barrier to achieving universal health coverage in poorer nations because of financial and nonfinancial challenges. Despite evidence of disparities in healthcare access and associated poorer health outcomes in Ethiopia, no previous study has thoroughly investigated inequity in access to needed medications. Hence, we aimed to assess inequity in access to needed medications among communities in eastern Ethiopia. METHODS: We conducted a cross-sectional study among 2,256 households, which were sampled using a str

Towards contextualized complex systems approaches to scaling-up hepatitis B birth-dose vaccination in the African region: a qualitative systematic review.

BACKGROUND: Despite the longstanding implementation of universal hepatitis B infant vaccination programs, the World Health Organization African region (WHO AFRO) maintains the highest prevalence (2.5%) of chronic hepatitis B virus (HBV) infection among children ≤5 years of age. Scaling-up hepatitis B birth-dose (HepB BD) vaccination could avert mother-to-child transmission of HBV infection and advance regional progress towards eliminating viral hepatitis. OBJECTIVE: To describe whether - and how - complexities within the health system or intervention influence the performance of HepB BD vacci

Effects of implementing free maternity service policy in Kenya: an interrupted time series analysis.

BACKGROUND: Maternal and perinatal mortality is a major public health concern across the globe and more so in low and middle-income countries. In Kenya, more than 6000 maternal deaths, and 35,000 stillbirths occur each year. The Government of Kenya abolished user fee for maternity care under the Free Maternity Service policy, in June of 2013 in all public health facilities, a move to make maternity services accessible and affordable, and to reduce maternal and perinatal mortality. METHOD: An observational retrospective study was carried out in 3 counties in Kenya. Six maternal health output i

Health worker perspectives on barriers and facilitators of tuberculosis investigation coverage among index case contacts in rural Southwestern Uganda: a qualitative study.

BACKGROUND: In 2012, the World Health Organization recommended screening and investigation of contacts of index tuberculosis patients as a strategy to accelerate detection of tuberculosis (TB) cases. Nine years after the adoption of this recommendation, coverage of TB contact investigations in Uganda remains low. The objective of this study was to examine health care providers' perceptions of factors influencing coverage of TB contact investigations in three selected rural health facilities in Mbarara district, southwestern Uganda. METHODS: This study identified provider opinions on the barri

Vouchers as demand side financing instruments for health care: a review of the Bangladesh maternal voucher scheme.

OBJECTIVES: Demand side financing (DSF) mechanisms transfer purchasing power to specified groups for defined goods and services in order to increase access to specified services. This is an important innovation in health care systems where access remains poor despite substantial subsidies towards the supply side. In Bangladesh, a maternal health DSF pilot in 33 sub-districts was launched in 2007. We report the results of a rapid review of this scheme undertaken during 2008 after 1 year of its setup. METHODS: Quantitative data collected by DSF committees, facilities and national information sy

Global Recognition of Traumatic Brain Injury as a Chronic and Notifiable Condition: A Post-WHA78 Advocacy Commentary.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of disability but one of the least recognized health problems in the world, affecting up to 69 million people annually. The associated lifelong disability in survivors, the loss of economic productivity, and being a risk factor for dementia consume 0.5% of global economic activity. Yet TBI is still largely invisible in national surveillance systems and not well represented in chronic disease frameworks. Consequently, governments are not equipped to provide proportional financing of acute care and long-term care of survivors, nor to bu

Strengthening the pipeline of Aboriginal and Torres Strait Islander doctors into and through general practice training in Queensland, Australia: a scoping review.

BACKGROUND: Aboriginal and Torres Strait Islander peoples experience significant health disparities relative to non-Indigenous Australians. Notwithstanding growing policy recognition of the link between Aboriginal and Torres Strait Islander health professionals and improved community health outcomes, their representation remains below parity in medical practice. This is particularly evident in general practice, where the general practitioner (GP) workforce shortfall intensifies nationally, and GP roles have become the hardest to fill across Aboriginal Community Controlled Health Organisations.

To what extent is equity entrenched in HIV/AIDS-related policy documents in Ethiopia? A policy content analysis.

INTRODUCTION: It is important to assess the extent of mentions of equity in policy and strategic documents for several reasons. First, it helps us understand the root causes of inequities, revealing both failures and successes. Second, it shows the readiness of stakeholders and leaders to take actions towards equity. Third, it identifies areas of improvement to plan strategies towards equity. The objective of this study was to assess the degree to which equity is incorporated in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS)-related policy documents in Ethiopia.

Bridging the digital pathology divide: reciprocal learning between high-resource and low-resource settings.

Digital pathology (DP) represents a major advance in global health, integrating digital whole-slide imaging, telepathology and clinical-grade artificial intelligence into cohesive workflows that can improve diagnostic accuracy and educational access. Realising these gains, however, depends on local infrastructure, governance, reimbursement and workforce capacity. The COVID-19 pandemic accelerated DP uptake in some settings, but adoption remains highly uneven between high-resource settings (HRS) and low-resource settings (LRS), reflecting unequal infrastructure, workforce, policy and financing.

Nouakchott Declaration: Promoting Organ Donation and Transplantation in Africa.

BACKGROUND AND HYPOTHESIS: Africa faces a growing crisis of end-stage organ failure driven by the rising burden of non-communicable diseases and fragile health systems. Behind these figures are preventable deaths, disrupted families, and widening inequities in access to lifesaving care, an inequity that organ transplantation could address. However, access to transplantation remains extremely limited across the continent. According to the 2024 Global Observatory on Donation and Transplantation (GODT) report, only 544 kidney transplants were performed in Africa, representing approximately 0.5% o
