Metacarpophalangeal portal safety. An anatomical study.

AIM: To quantify the risk of dorsal innervation injury when performing direct metacarpophalangeal joint portals of the second to fifth fingers. MATERIAL AND METHOD: An anatomical study of 11 upper limbs of fresh corpses was carried out. After placing them in a traction tower, the metacarpophalangeal portals were developed on both sides of the extensor tendon. The dorsal sensory branches were dissected and the distances between the portal and the nearest nerve were measured by a digital caliper. The portals of all the fingers were compared globally to assess the safest finger and two to two ra

Critical Angles of Deformity in Dupuytren's Contracture of the Little and Ring Fingers.

BACKGROUND: We aimed to determine the degree of contracture in the ring and little fingers at which hand function became importantly impaire … PIPJ and combined MCPJ and PIPJ angulation at 52.5, 67.5, and 75 degrees respectively. For the ring finger joint, the angulations were 52.5, 67.5 and 75 degrees respectively. CONCLUSIONS: This information will provide information for clinicians and patients as to when clinically important disability tends to occur in DC.

Cutaneous squamous cell carcinoma (SCC) arising in stump of amputated finger in a patient with resected glossal SCC.

BACKGROUND: Cutaneous squamous cell carcinoma (SCC) of the hands and fingers are sometimes locally aggressive; with higher rates of regional metastasis than other cutaneous SCC, although distant metastasis is rare. CASE PRESENTATION: We present the case of a 62-year-old Japanese man with double cancers: a tongue SCC and a cutaneous SCC. Swelling of the finger lesion developed gradually around the entire remaining middle finger after accidental amputation at the proximal interphalangeal joint. Histopathological examination of the tumor on the stump of the amputated finger indicated a well-diff

Patterns of proximal interphalangeal joint dislocations.

INTRODUCTION: The aim of the study was to understand finger proximal interphalangeal joint (PIPJ) dislocations better by reviewing the demographics and biomechanics of a cohort of PIPJ dislocations. METHODS: All cases of PIPJ dislocations presenting to our emergency department or neighbouring minor injury units over a 2 year period were reviewed. We recorded demographics, mechanism of injury, direction of the dislocation radiologically, concomitant injuries and rates of follow-up (FU). RESULTS: There were 74 dislocations in 74 adults with a median age of 46 (range 14-87) years. Fifty-six (76

Electromyographic Study on the Inhibitory Effects of Local Cold- and Hot-Water Bathing of the Upper Limb on Finger Flexor α-Motor Neuron Activity.

This study evaluated the inhibitory effects of cold and heat stimulation on α-motor neuron activity and the associated pain, exploring their clinical applications. Twenty-four healthy young adults participated, undergoing four conditions: (1) cold-water finger immersion, (2) warm-water finger immersion, (3) warm-water forearm immersion, and (4) a control condition. Assessments included grip strength, integrated electromyography (IEMG) of finger flexor muscles, skin temperature, and pain perception. Cold-water finger immersion involved three sets of 5-second ice water immersion with 2-second br

Ultrasound-Based Sensing Models for Finger Motion Classification.

Motions of the fingers are complex since hand grasping and manipulation are conducted by spatial and temporal coordination of forearm muscles and tendons. The dominant methods based on surface electromyography (sEMG) could not offer satisfactory solutions for finger motion classification due to its inherent nature of measuring the electrical activity of motor units at the skin's surface. In order to recognize morphological changes of forearm muscles for accurate hand motion prediction, ultrasound imaging is employed to investigate the feasibility of detecting mechanical deformation of deep mus

Functional Electrical Stimulation and Brain-Machine Interfaces for Simultaneous Control of Wrist and Finger Flexion.

Brain-machine interface (BMI) controlled functional electrical stimulation (FES) is a promising treatment to restore hand movements to people with cervical spinal cord injury. Recent intracortical BMIs have shown unprecedented successes in decoding user intentions, however the hand movements restored by FES have largely been limited to predetermined grasps. Restoring dexterous hand movements will require continuous control of many biomechanically linked degrees-of-freedom in the hand, such as wrist and finger flexion, that would form the basis of those movements. Here we investigate the abilit

Intra- and inter-rater reliability of goniometric finger range of motion using a written protocol.

INTRODUCTION: Goniometric finger range of motion (ROM) is the most common outcome measure used for functional evaluation of finger joints, but its reliability is not well-evaluated. This study aimed to investigate intra- and inter-rater reliability of goniometric finger ROM using a written protocol for active, passive, and composite movements in healthy adults. METHODS: The design was a single-center, cross-sectional, reliability study. Participants were 20 healthy adults (mean ± standard deviation, 36.4 ± 10.9 years). ROM for active, passive, and composite movements of the fingers was assess…

Modified extension-block K-wire fixation technique for the treatment of bony mallet finger.

This article describes the treatment of a bony mallet finger deformity using 2 extension-block Kirschner wires (K-wires) with a transarticular K-wire fixation technique for precise alignment of the terminal extensor tendon-bone relationship and effective immobilization of the distal interphalangeal joint. Twenty-nine patients (33 fingers) with a bony mallet finger deformity and fracture fragment involving more than one-third of the articular surface were treated surgically. The fracture fragment was fixed and the mallet finger deformity was corrected in all patients using modified extension-bl

Dactylitis, a term for different digit diseases.

Dorland's Illustrated Medical Dictionary gives the following definition of dactylitis (deltaalphachitauupsilonlambdaomicronsigma = digit): 'inflammation of a finger or toe'. Although any inflammatory process involving the fingers or toes may be called dactylitis, the term has entered in current use only in some well-defined entities. These differ in the involved tissue of the digit and in the type of involvement. Tuberculous dactylitis is the variant of tuberculous osteomyelitis affecting the short tubular bone of the hands and feet. Radiographs typically show a central, lytic, cystic, and exp

Using active or functional range of motion as a further indicator to detect hand osteoarthritis. A preliminary study.

PURPOSE: Propose further indicators for helping to detect hand osteoarthritis (HOA) using either active or functional ranges of motion (AROM or FROM). METHOD: Hand kinematics data of 16 hand joint angles from previous studies were used, taken from healthy participants and HOA patients with differently affected joints and distinct levels of compromise. Data consisted of: (i) AROM (extreme values and ranges); (ii) FROM while performing the Sollerman Hand Function Test (mean, extreme percentiles and ranges). Two linear discriminant analyses (stepwise method) were conducted, one per dataset (AROM

Arthrodesis of the digital joint using intraosseous wiring in patients with rheumatoid arthritis.

OBJECTIVE: Digital joints affected by rheumatoid arthritis often have severe deformity and/or dislocation, and arthrodesis in a functional position is required. METHODS: Arthrodesis was performed using intraosseous wiring (modified Lister's method) from January 2011 to December 2015, and we investigated the union rate, postoperative complications, and patient satisfaction with the operation at the final follow-up. The DASH score, grip power, and pinch power were also investigated before the operation and at the final follow-up. RESULTS: Arthrodesis was performed for 90 digital joints in 56 p
